Stroke is the third most common cause of mortality in Westernised countries and Accounts for 12% of all deaths in the UK. The economic cost of stroke is enormous. Twelve per cent of first strokes occur in patients under 45 years of age, of which approximately 50% are ischaemic in nature. Stroke in young adult poses a major health problem. The causes of ischaemic stroke in young adults are many and diverse.Such patients usually require more extensive investigations in order to find an underlying cause than more elderly patients. Principal causes are cardioembolism, premature atherosclerosis, haematological and immunological disorders and migraine. Thrombophilic factors have been implicated in 4-8% of the young adult strokes worldwide. Protein S deficiency is a rare cause of ischemic stroke in young population. Only a few sporadic cases have been described in the literature. We are reporting a case of protein S deficiencyrelated ischemic stroke in a 40-year-old man. Early diagnosis and targeted approach can help such patients to prevent recurrent thrombotic episodes.Bangladesh Journal of Neuroscience 2012; Vol. 28 (1): 59-62

We have reported a rare case of DVT in a 25-year-old patient with a known case of Protein S deficiency with INR within therapeutic index. He was seen in the ED and transferred to the Medical ward after diagnosis in the ED using doppler US. The patient was successfully treated with heparin and warfarin with a higher than usual optimal INR. This case adds to the growing evidence that PS deficiency is one of the rare causes of DVT, and also raises awareness that thrombosis can still occur in the current optimal INR for such patients. This case report necessitates the revision of what should be the optimal INR for patients with Protein S deficiency who develop thrombosis in the current optimal INR.

Filariasis is a major health problem in the Indian Subcontinent. Due to its nocturnal periodicity it may be difficult even to demonstrate in the blood. In heavy parasitic load they may appear in the blood, urine with chyle and at times in scrotal aspirates. It is very rare and unusual to find microfilaria in thyroid aspirate. Herpes zoster virus (HZV) reactivation is commonly reported in elderly or immune compromised patients. However, certain presentations are rarely reported in immune competent patients. Here we report a rare case of recalcitrant HZV infection, including 7 reactivations over 3 years, in an immune competent patient. Due to the pain experienced, this patient self-referred to our clinic, and thus bypassed other specialties. As pain management specialists, it is critical to be aware of such presentations, the clinical implications involved, and management options to consider. The COVID-19 pandemic continues to be a major health problem worldwide. Timely diagnosis of co-infections mimicking COVID-19, such as malaria, might be challenging particularly in non-endemic areas. We report the first case of COVID-19 and Plasmodium ovale malaria co-infection from our region aiming to highligt the importance of travel history and prophylaxis in malaria management in the context of pandemic. The galloping sound can sometimes be a harbinger of zebra besides the horse.
