Introduction: Cecal volvulus (CV) is rare cause of intestinal obstruction especially in children. There is no established protocol for the treatment of CV due to its rarity. Case Presentation: We present a rare case of CV in a 9-year-old girl. Discussion: The spectrum of intestinal malrotation may give rise to a mobile cecum that may result to CV. Constipation could be a predisposing factor. The clinical presentation which may include abdominal pain, distension, constipation and vomiting depends on the time of presentation. There may be associations with chromosomal abnormalities. Conclusion: Although CV is a rare cause of intestinal obstruction, awareness and high index of suspicion is needed by clinician for early diagnosis and treatment.

Hernia arising from obturator canal is rare and it contributes to about less than 1% of incidence of all hernias. Diagnosing an obturator hernia clinically is a challenging one and nearly impossible. These hernias usually present as an intestinal obstruction as more than 50% of obturator hernias goes in for strangulation. Here, we report an unusual presentation of an obturator hernia in a 70-year-old woman who presented to emergency room with acute abdomen and uncomplicated reducible inguinal hernia. Radiological imaging showed obstructed inguinal hernia while on diagnostic laparoscopy, a strangulated and perforated obturator hernia of Richter’s type was seen in addition to an uncomplicated inguinal hernia. Obturator hernia, although very rare, is associated with high morbidity and mortality as it is often underdiagnosed as in our case. Laparoscopy bailed us out from missing out a perforation from an occult obturator hernia.

BACKGROUND: Internal hernias are uncommon causes of acute abdomen and intestinal obstruction. Internal herniation due to appendices epiploicae is very rare with only six cases reported in the literature.CASE REPORT: We, herein, present the report of a 64-year-old female who presented with features of intestinal obstruction due to internal herniation of a loop of small intestine through an orifice formed by the fusion of two appendices epiploicae. The band was divided into release the entrapped loop of bowel.CONCLUSION: A high index of suspicion with prompt surgical intervention will improve outcome.

Abstract Background Ocular cysticercosis is a disease which rarely involves cutaneous skin and the optic nerve. Patients with clinical presentation of subcutaneous nodules and papilledema should always be evaluated for cysticercosis. Case presentation We report a rare case of ocular cysticercosis with multiple disseminated subcutaneous nodules and papilledema in both eyes. A 22-year-old Brahmin man presented with complaints of gradual loss of vision in both eyes and multiple small masses all over his body. On clinical evaluation, multiple subcutaneous nodules were seen on his face, mandibular area, elbow, arm, and abdomen. A fundus evaluation showed bilateral blurred disc margin. The case was managed with steroids and anti-parasitic drugs. Conclusion This case report highlights the importance of ruling out neurocysticercosis in cases with multiple disseminated subcutaneous nodules and papilledema.

Background:This study was done at Niloufer hospital and institute of child health, Hyderabad with an aim of evaluating the clinical presentation, diagnostic evaluation, management and outcome of malrotation of intestines in the neonates.Methods: Cases of neonatal small intestinal obstruction due to malrotation presenting to the Department of Pediatric Surgery, Niloufer hospital over a period of two years were evaluated.Results: A total of 38 newborns presented to our department with intestinal obstruction due to malrotation. 3/38 patients presented with extensive gangrene of midgut. 4/38 patients died during the course of treatment.Conclusions: Malrotation is a relatively common cause of neonatal small bowel obstruction. A high index of suspicion is needed in neonates presenting with bilious vomiting. Early laparotomy prevents fatal complication of extensive gangrene due to midgut volvulus.

Idiopathic pneumomediastinum is a form of spontaneous pneumomediastinum without predisposing factors and precipitating factors. The purpose of this publication is to present a rare case of this pathological condition. Case presentation: A 21year-old female with spontaneous pneumomediastinum was revealed. Medical history, physical examination, imaging and interventional diagnosis couldn’t determine any predisposing and precipitating factors for pneumomediatinum: case of idiopathic pneumomediastinum with unusual widespread subcutaneous emphysema. Conclusion In conclusion, this publication refers to one relatively rare condition in the thoracic pathology – idiopathic pneumomediastinum. Being a case of idiopathic pneumomediastinum, there was an unusual clinical presentation of widespread subcutaneous emphysema. Despite widespread subcutaneous emphysema, the presenting case confirms the good prognosis of idiopathic pneumomediastinum.

Abstract Background We report a rare case of primary abdominal cocoon with bilateral cryptorchidism. Case presentation The patient had a history of laparoscopic surgery for bilateral cryptorchidism 6 years earlier. He was admitted to the hospital again due to intestinal obstruction. Surgery was performed on the patient after the failure of conservative treatment. The patient was diagnosed with primary abdominal cocoon. Instead of the greater omentum, many cocoon-like tissues surrounding the bowel were seen during operation. Abdominal surgery can increase the risk of intestinal adhesion, which is one of the main causes of intestinal obstruction, especially in patients with abdominal cocoon. We hypothesize that the surgery 6 years earlier to address transabdominal bilateral cryptorchidism accelerated the patient’s intestinal obstruction. Conclusion This case implies that it is important for urologists to evaluate whether their patients exhibit abdominal cocoon before cryptorchidism surgery, to choose better surgical methods and reduce the risks of poor prognosis.

Introduction: Cysticercosis is a disease of public health importance which causes significant morbidity and mortality in developing countries including India. The clinical presentation of cysticercosis is variable and involves various body tissues. Isolated muscle involvement of cysticercosis is relatively rare. Case Presentation: We report a case of 28 year old female with complaints of mid back pain. Investigations revealed cysticercosis of the para-spinal muscles. Due to non-resolution of symptoms with oral medication, patient was managed surgically and the lesion was removed. Conclusion: Cysticercosis should always be included in differential diagnosis of muscle swelling in countries endemic for Taenia infection. Surgical resection may be required in muscular form of cysticercosis if significant improvement is not observed with conventional oral treatment.

Teratoma are bizarre tumors of germ cell and are usually present in testis and ovaries and contains structures like hairs, teeth, bone, thyroid tissue or more complex organs. Retroperitoneal occurrence is an uncommon site of presentation. They are usually present in childhood and rarely in adults. Here we are discussing a rare case presentation of middle age female, presented with abdominal fullness and pain which later diagnosed as retroperitoneal teratoma on CT scan. Clinical presentation, workup, surgical management, and outcome is discussed in this study.
