Primary duodenal lymphoma is very atypical, although the gastrointestinal (GI) tract is the most prominent site of extra nodal lymphoma. Here we present a case of 25 years old male, presented with upper abdominal pain and vomiting and eventually diagnosed as a case of duodenal non Hodgkin's lymphoma. The particular interest in this report is the fact that, it is important to have a high index of suspicion to diagnose primary intestinal lymphoma as it has high mortality which requires early diagnosis and treatment.    doi: 10.3329/jom.v10i2.2829   J MEDICINE 2009; 10 : 124-127

Background: Ocular myocysticercosis is rare and a high index of suspicion is required for its diagnosis. Objective: To describe clinical characteristics and treatment outcome of ocular myocysticercosis. Cases: We describe a series of three patients who had different clinical presentations of ocular myocysticerocosis namely diplopia, restricted ocular motility and subconjunctival cyst. The treatment with oral albendazole and prednisolone was effective in all three cases. Conclusion: Favorable outcomes can be achieved with a high index of suspicion, early diagnosis and treatment with oral albendazole and prednisolone in patients with ocular myocysticercosis.DOI: http://dx.doi.org/10.3126/nepjoph.v4i2.6551 Nepal J Ophthalmol 2012; 4 (2): 315-318

Little attention has been paid for the renal involvement in endometriosis, a rare and silent disorder which ultimately lead to renal failure. Involvement most commonly may be limited to single ureter (left one) and it is usually involvement extrinsically. Although cases have been reported in the literature, true incidence of ureteral involvement is still not known. The diagnosis is difficult as the disease has non-specific symptoms. Only high index of suspicion with radiological support would be helpful in early diagnosis. Early cases may be benefited with progestin or Anti-aromatase therapy, most cases need surgery, either laparoscopically or laparotomy.

Laparoscopic cholecystectomy is usually performed for gallstones or polyp of the gallbladder. Multiseptate gallbladder is a rare congenital malformation. Although several asymptomatic cases have been described, patient usually present with right upper abdominal pain. We present a 29-year-old female patient with multiseptate gallbladder, cholecystectomy was performed, and her abdominal pain and gastrointestinal complaints have resolved.

Subglottic and tracheal malignancies are the least frequently encountered laryngeal cancers. When these tumors are encountered, they have often reached advanced size prior to producing symptoms. Once symptoms are brought to the attention of the otolaryngologist, rapid and accurate diagnosis is essential to effect the earliest possible treatment and to minimize morbidity and mortality associated with this problem. Illustrative cases are presented including a highly unusual case that presented with cardiac-like chest pain. A review of the literature including the pertinent anatomy, pathophysiology, and methods of diagnosis and treatment is presented. Increased familiarity with this disease process and a high index of suspicion on the part of the otolaryngologist may lead to improved survival patterns in patients with these malignancies.

Opportunistic fungal infections of the upper limb are rare and require a host who is immunologically compromised. A review of these rare mycotic infections is presented. The hand surgeon should be aware of these infections and should have a high index of suspicion in order to establish an early diagnosis and institute an appropriate management.

Introduction: Scrotal tuberculosis (TB) is rare and may present as painful scrotal swelling with ulceration and discharging sinus. Case Report: A 28 years male with 2 months history of swelling and pain over left scrotum. Developed ulcer over the scrotal region with multiple sinus associated with pus discharge. Conclusion: The clinical presentation of TB scrotal ulcer can be atypical and a high index of suspicion is required for early diagnosis. Diagnosis is by using ultrasonography, microbiology, and biopsy. Treatment requires prolonged ATT for 6 months.
