The aim of this study was to identify variables associated with early childhood caries (ECC) in 5-year old Brazilian children to allow their monitoring at both regional and national levels. It is a population-based cross-sectional study with epidemiological and socioeconomic data of 7.217 children included in the National Oral Health Survey (SBBrasil 2010). The dependent variable was untreated caries (component c of cpod). Independent variables were grouped into individual and contextual ones, and multilevel models of Poisson regression analysis were applied. In the country, 49,8% of children aged 5 years presented untreated caries. Among the 5 national regions, prevalence of untreated caries was higher in the North (64,9%) and lower in the Southeast (43,7%). National cpod index was 2,45 (CI 95%: 2,20-2,71) and component c was 2,05 (CI 95%: 1,81-2,29). There was a significant difference between North (3,11; CI 95%: 2,69-3,53) and Southeast (1,68; CI 95%: 1.40-1,95) regions. At individual level, low family income was associated with higher prevalence of untreated dental caries (CI 95%:  2,05-2,88). At contextual level, there was higher prevalence of untreated caries in areas with less access to fluoridated water (CI 95%: 1,04-2,25), less access to primary oral health care (CI 95%: 0,93-0,98), higher population/dentist ratio (CI 95%:  1,04-1,33), and higher percentage of children living in low-income families (CI 95%:  1,18-1,78). Our results indicate that socioeconomic factors were determinant for ECC prevalence, increasing dental treatment needs.

Objectives: Calculate the prevalence and severity of dental caries and periodontal health among school students aged 12-18 years old and their attitudes toward dental health and oral hygiene at Al-Mafraq governate- Jordan.Materials and methods: Demographic and oral behavioral information were collected; students aged 12-18 years old from six public schools distributed over three districts in Al-Mafraq governate over a six-month period had a full oral and dental examination. DMFT index (Decay, Missing, Filling, Teeth) was used to measure caries experience. Plaque index (PI) and gingival index (GI) of Löe and Silness were used to evaluate the oral health status. T - test was used for statistical evaluation. A P-value < 0.05 was considered statistically significant.Results: 1165 students were examined, 51% never had been to a dentist or had any previous dental treatment, 39.6% of the students examined never brushed their teeth and 61.85 % had dental caries. The (SiC) was 4.72 with a decay representing 93.53%. Females brush their teeth more frequent than males with a statistically significant difference; p< 0.001. Females showed lower DMFT index (1.81) and compared to males (2.25) with a statistically significant difference. Females showed lower SiC (4.164) compared to males (5.186)   with a statistically significant difference. There was no statistically significant difference between males and females in PI p= 0.590, and GI; p= 0.852.Conclusions: The prevalence of dental caries among students in Al-Mafraq governate and the significant caries index were substantially higher than the target goals of WHO/ FDA of year 2015 with more than 90 % of unmet treatment needs. Girls showed better oral hygiene habits compared to boys and exhibited less dental caries experience. However, periodontal status did not differ significantly between both genders.

Introduction: From 1999–2004 to 2011–2014, untreated dental caries prevalence decreased among US children aged 2 to 5 y, regardless of family income. Policies were concurrently initiated for children to increase access to preventive dental services in dental, primary, and community settings and to restorative care in dental settings. Objectives: We aimed to examine 1) whether changes in prevalence and severity of untreated and treated caries between the periods varied by family income and 2) to what degree increased past-year dental visit (PYDV) contributed to the changes. Methods: We used data for 3,822 children in the National Health and Nutrition Examination Survey 1999 to 2004 and 2011 to 2014. Caries prevalence included prevalence of untreated caries with ≥1 decayed teeth (dt) and prevalence of treated caries with ≥1 filled teeth (ft). Caries severity included number of dt and ft among those with ≥1 dt or ft. We estimated changes in caries outcomes among low- and higher-income children with models—one controlling for sociodemographics and another controlling for sociodemographics and PYDV. Significant changes ( P < 0.05) becoming insignificant after controlling for PYDV provide insight on the contribution of PYDV to changes in outcomes. Results: Prevalence of untreated caries decreased for low- and higher-income children, with a slightly larger decrease for low-income children; dt decreased only for low-income children; and estimated decreases did not vary by model. An increase in prevalence of treated caries was observed only among low-income children but became minimized and insignificant after controlling for PYDV. Similarly, after controlling for PYDV, the increase in ft among low-income children lost significance, whereas the increase among higher-income children remained. Conclusion: Untreated caries among children aged 2 to 5 y declined from 1999–2004 to 2011–2014, with larger declines among low-income children. While changes in PYDV contributed to increases in treated caries, particularly for low-income children, additional factors appear to have contributed to decreased untreated caries. Knowledge Transfer Statement: For young children, the degree and direction of changes in caries over the last decade varied by outcome measure (e.g., untreated or treated) and family poverty status. Examining the effect of increased dental utilization on changes in untreated and treated caries outcomes can help identify those policies that contribute to changes in these outcomes and highlight the potential role of the different caries assessment criteria used in dental offices versus those in a population-based survey.

Abstract Introduction Sugar ingestion is the most important isolated factor related to dental caries. Contact with sugary foods at an early age may represent a risk to the oral health of preschool children. Objective To evaluate the timing of the introduction of sugar in the diet of preschoolers and its association with early childhood caries. Material and method Cross-sectional population-based study with 888 five-year-old preschoolers from public and private schools. Parents answered a questionnaire on sociodemographic data, eating habits and oral health. The variables timing of sugar introduction (>1 year and ≤1 year) and dental caries experience were dichotomized and data were analyzed using descriptive statistics and Poisson regression (p<0.05). Result The introduction of sugar in the diet occurred during the first twelve months of life for 73.8% of preschoolers. There was no association between the timing of sugar introduction and early childhood caries (p>0.05). The interruption of exclusive breastfeeding and low family income increased the prevalence of preschoolers having early contact with sugar by 32% and 23%, respectively (p<0.05). The prevalence of early childhood caries was 42.3% and was associated with a lower level of maternal education, low income, increased frequency of ingestion of candy and the use of nonfluoridated toothpaste (p<0.05). Conclusion The introduction of sugar in the diet occurred during the first year of life in most preschoolers and there was no association with early childhood caries.

Introduction: Dental caries and periodontal disease are among the most common infectious diseases in the world. DMFT (Decayed, Missing, Filled, Teeth), CPITN (Community Periodontal Index of Treatment Needs), and plaque indices are applied as suitable criteria for evaluating these two diseases and oral health. Given the significance of dental caries and periodontal disease, the objective of this study was to determine the incidence of dental caries and periodontal disease in the Iranian city of Shiraz. Material and Methods: The samples were evaluated in the four categories of medical, manufacturing, educational, and office centers within Shiraz, and then the DMFT, CPITN, and plaque indices in addition to nutrition status were all measured among them. Results: Having considered the DMFT to be below 15% and the CPITN less than 1.5 (the variables’ mean in the population under study), respectively, as resistant to dental caries and periodontal disease while at the same time taking into account the plaque index of less than 1.25 (mean) as the low level of this very index not disregarding the nutritional status of the population under study: the resistant individuals ratio to the two diseases equaling 10%, those individuals prone to these two diseases equaling 55%, the ratio of resistant people to periodontal disease and, simultaneously, prone to dental caries equaling 14%; and, lastly, the ratio of individuals resistant against dental caries simultaneously prone to periodontal disease equaling 20% (estimated). Conclusion: The research was performed in medical, manufacturing, educational, and office centers in Shiraz, Iran. There was no significant difference in terms of periodontal disease and dental caries between the four groups. [GMJ. 2012;1(2):66-71]

Background: Although they are not life threatening, dental caries and periodontaldisease are the most predominant and widely spread oral diseases throughout theworld. The aims of the study included the investigation of the prevalence andseverity of dental caries, gingivitis and dental plaque in relation to gender,furthermore, nutritional status was assessed in relation to oral health condition(dental caries).Materials and Methods: This oral health survey was conducted among primaryschool children aged 9 years old in Dewanyiah city in Iraq. The total samplecomposed of 600 child (320 males and 280 females) selected randomly fromdifferent school in Dewanyiah city. Diagnosis of dental caries was according tothe criteria described by WHO (1987). Plaque index of Silness and Loe (1964)was used for plaque assessment, gingival index of Loe and Silness (1963) wasfollowed for recording gingival health condition. Nutritional status was assessedaccording to body mass index (BMI) indicator using anthropometric measurement(height and weight).Results: Results showed that the prevalence of dental caries was 85% for 9 year-oldschool children. Regarding primary and permanent dentition, dental caries washigher among females compared to males with statistically significant difference(P<0.05) for primary dentition, on the other hand, males showed higher values offilled surfaces compared to females with statistically significant difference(P<0.05) for primary dentition and highly significant difference (P<0.01) forpermanent dentition. Finding of this study revealed that 100% of the children hadgingival inflammation. Furthermore, the values of plaque and gingival indiceswere higher among males compared to females with statistically highly significantdifferences (P<0.01). In current study, the prevalence of malnutrition described bythe BMI indicator was 5.3%. For total samples no significant difference wasrecorded in dmfs /DMFS values among wasting and well nourished children(P>0.05).Conclusion: A high prevalence of dental caries and gingivitis were recorded.Improvement in the prevention educational programs is needed among schoolchildren.

Abstract Background Children suffer a significant burden of oral diseases which impacts their quality of life, and general health. Many epidemiological studies have looked at oral health status and treatment needs among children in urban and rural areas across different age groups. There was limited literature assessing the oral health status among children in tribal areas, which presents unique challenges in terms of: topography, availability of health services, and complex socio-cultural health practices. Methods Cross-sectional study was conducted among 655 children across six schools between 12-15 years in Jawadhi Hills, Tamil Nadu, India. World Health Organization's, Oral Health Surveys were used for clinical and risk factor assessments. Clinical examination was conducted, and self-assessed questionnaire was administered to the children to assess the risk factors to poor oral health. Since dental fluorosis was common, drinking water samples were collected to assess fluoride levels in the community. Results Prevalence of dental caries and gingivitis (gum disease) was found to be 22.3% and 87.3% respectively. Dental Pain was reported by 57% of children as the most common reason to visit the dentist. Prevalence of dental fluorosis was found to be 53.6%. Multiple logistic regression showed: gender, dental fluorosis, diet, and maternal education significantly associated (p &lt; 0.005) with dental caries. Fluoride levels in water was found to be higher than permissible limits (&gt;1.5 ppm). Conclusions Children requiring treatment (∼ 200) were referred to the local hospital, and treatment was done free-of-cost. School authorities were informed about the current oral health status of children and advised to conduct regular health education & dental camps; the importance of girls' education was emphasized. Local authorities were alerted about the fluoride-rich water situation in the community and educated about defluoridation methods that the community can employ at household level. Key messages Evidence-based burden of oral diseases was presented to the local authorities for the first time, with a call for action. A referral system was established between the schools and the local hospital, to ensure continuity of oral health care for the children.

Aim: Smoking induces changes in salivary inflammatory biomarker levels associated with oral diseases. This study status and explored association among salivary Interleukin-1β, oral diseases and smoking. Methods: Data of male smokers of a private institute recruited for a randomized clinical trial is analyzed for this paper. Demographic and systemic information were collected. Oral disease status was examined and saliva sample collected for IL-1β levels. IL-1β levels and other study variables were analyzed with respect to smoking status categorized into smoking years and cigarettes per day. The student's t-test and one-way ANOVA were used for statistical analysis using SPSS version 22) with significance level of p≤0.050. Results: Analysis of baseline data of seventy-eight smokers showed elevated levels of IL-1β with increasing smoking, higher BMI. Smoking was higher among aged, married and low-income individuals. Pearson partial correlation analysis, after controlling age, marital status, education, income, and BMI, demonstrated a positive significant relationship of smoking per day with smoking years; dental caries with missing teeth and calculus; gingivitis with missing teeth and calculus; periodontitis with dental caries, calculus and gingivitis. Conclusion: This analysis demonstrates that levels of IL-1β were raised in smokers, however; there was no association with oral disease parameters. Large studies may be conducted to observe status and association of smoking, oral disease and salivary biomarkers. Keywords: Smoking, Oral Health, IL-1β, Males

This study assessed oral health outcomes (perceived dental treatment need, untreated dental caries, gingival bleeding, periodontal pockets, and pain in teeth and gums), in relation to color/race inequalities among adolescents in each Brazilian region. The database included dental examination and interview of 16,833 15-19-year-old adolescents, surveyed by the Brazilian health authority, from May 2002 to October 2003, in accordance with international diagnostic criteria standardized by the World Health Organization. Prevalence ratios estimated by Poisson regression, and controlled by socioeconomic status and access to fluoridated piped water, assessed oral health differentials among color/race groups and country's regions. Except for periodontal pockets, prevalence figures were higher in the North and Northeast: perceived dental treatment needs, untreated dental caries, gingival bleeding at probing and pain in teeth and gums varied between 80-83%, 75-76%, 38-43%, and 17-18%, respectively, in these regions. Adolescents living in the Southeast - the richest Brazilian region - presented a better general profile of oral health than their counterparts living in the remaining regions; they had a lower prevalence of untreated dental caries (54%) and unfavorable gingival status (29%). However, the Southeast presented color/race inequalities in all oral health outcomes, with a poorer profile systematically affecting browns or blacks, depending on the oral health condition under consideration. These results reinforce the need for expanding the amplitude of health initiatives aimed at adolescent oral health. Socially appropriate health programs should concurrently aim at the reduction of levels of oral disease and its inequalities.

Objectives: To assess oral health status and practices of 5- and 12-year-old Tribal school children.Methods: A total of 418, 5-year-old children and 327, 12-year-old children were enrolled. Information on demographic characteristics of participants along with oral health behavior was collected. Clinical data were collected on dental fluorosis, periodontal status, dental caries and treatment needs. Dean's index criterion was used to assess dental fluorosis. Community Periodontal Index (CPI) for periodontal conditions and Dentition status and treatment needs for dental caries were recorded. Results: Between meal sugar consumption was high (100%). None of the children in both the age groups had visited trained health personnel for dental treatment. Dental fluorosis prevalence in 5- and 12-year olds was 11.9% and 22.9% respectively. Bleeding on probing and calculus was common between both the age groups. A low mean number of healthy sextants were found and this decreased with age. Mean dmft/DMFT values for 5- and 12-year olds were 4.13 ± 3.90 and 1.15 ± 1.62. Significant caries index (SIC) scores for 5- and 12-year olds were 7.17 ±4.30 and 3.78 ± 3.21 respectively. Conclusion: The present study reveals high sugar consumption, dental fluorosis, poor oral hygiene, and untreated dental disease of tribal children. Under these circumstances, the implementation of preventive programs including restriction of sweets in school premises for the tribal children is the key to good oral health.

Oral diseases, including dental caries, periodontitis, and edentulism (toothlessness), affect about 3.5 billion people worldwide. A cross-sectional study was conducted to assess the oral health status and treatment needs of government employees employed at an organization in New Delhi. Oral health information was recorded using the standard World Health Organization’s (WHO) Oral Health Assessment Tool. A total of 476 employees at various levels of administration were screened at their worksite. Dental caries and periodontal disease were present in more than half of the participants. Around 56% had decayed teeth, 20% had missing teeth, and 16% had filled teeth. Bleeding from gums was seen in 71% and periodontitis in 59% of participants. Preventive treatment and oral health promotion was required in at least 41% of the screened individuals. Annual workplace oral examinations may help in decreasing the oral disease burden and create awareness on the oral health among employees.
