The oroantral communication is considered one of the most frequent complications in oral surgery, especially in extractions of posterior teeth in the maxilla, treatment of cystic lesions, neoplasms, osteomyelitis and trauma in the region of the maxillary sinus, forming fistulas. However, ectopic third molar and lesions such as odontomas are rarely found simultaneously in the maxillary sinus. This report shows a successful treatment performed in a specific case of oroantral communication, associated to an underdiagnosed complex odontoma and an impacted ectopic third molar into the maxillary sinus and discuss the requirement of intervention. The treatment strategy involved clinical and surgical management through the oroantral communication, avoiding additional bone access to the maxillary sinus. This proposal presented a satisfactory result, and at six months after surgery, the patient has an excellent evolution, proving to be a successful strategy.

A dentigerous cyst or follicular cyst is a form of odontogenic cyst. It is believed that it forms during the development of the tooth and is associated with pressure exerted by the crown of an unerupted (or partially erupted) tooth on the fluid within the follicular space. Typically, dentigerous cysts are painless and discovered during routine radiographic examination. However, they may be large and result in a palpable mass. Additionally, as they grow they displace adjacent teeth. They almost exclusively occur in permanent dentition. The cyst is lined by stratified squamous non-keratinizing epithelium. About 70% of dentigerous cysts occur in the mandible and 30% in the maxilla. Dentigerous cysts associated with ectopic teeth within the maxillary sinus are very rare. We report radiologic and pathologic features in a rare case of infected dentigerous cyst of maxillary sinus arising from an ectopic third molar in a 21-year-old female patient.

ABSTRACT Ectopic eruption of teeth into a region other than the oral cavity is rare although there have been reports of teeth in the nasal septum, mandibular condyle, coronoid process, palate, chin and maxillary sinus. Eruption of ectopic teeth in maxillary sinus usually is found incidentally on routine radiological investigations. We present a case of an ectopic maxillary third molar tooth with dentigerous cyst that caused chronic purulent sinusitis in left maxillary sinus. Resumo: Inúmeras complicações podem ocorrer durante procedimentos cirúrgicos odontológicos, sendo a comunicação oro-antral uma das mais abordadas na literatura. Esta condição é resultante, principalmente, em exodontias de elementos dentários superiores posteriores, visto sua proximidade com o seio maxilar. O seu diagnóstico é feito basicamente por meio da manobra de Valsalva e análise do fragmento, contudo exames complementares podem evidenciar essa condição. A literatura considera, também, que a conduta de tratamento pode ser bastante diversificada. Objetivo: Apresentar por meio de uma revisão narrativa da literatura, as possibilidades técnicas de condutas clínicas para casos de comunicação oro-antral. Metodologia: A pesquisa bibliográfica foi online, com inclusão de artigos em português, inglês e espanhol, publicados nos últimos 10 anos, exceto àqueles fundamentais a respeito do tema, com tempo maior de publicação. Foram excluídos, também, cartas ao editor e editoriais. Foi, então, confeccionada uma análise descritiva dos artigos e os dados foram organizados de forma a subsidiar os profissionais da odontologia a intervir de forma segura diante desta complicação. Considerações finais: A comunicação oro-antral é uma realidade que pode ocorrer no consultório odontológico. Suas consequências são bastante variadas, cabendo ao Cirurgião-Dentista, além de evitar e diagnosticar, saber intervir quando necessário de forma eficaz. Haja vista que suas complicações podem limitar a qualidade de vida do paciente. Palavras-chave: Seio maxilar; Cirurgia bucal; Procedimentos Cirúrgicos Bucais. Abstract: Numerous complications can occur during dental surgical procedures, and oroantral communication is one of the most discussed in literature. That condition results mainly from extractions of posterior superior dental elements, given its proximity to the maxillary sinus. Its diagnosis is basically made through the Valsalva maneuver and analysis of fragment, however complementary exams can demonstrate that condition. The literature also considers the treatment approach can be quite  diversified. Objective:   Topresent, through a narrative review of literature, technical possibilities of clinical approaches for cases of oroantral communication. Methodology: The bibliographic research was online, including articles in Portuguese, English and Spanish, published in the last 10 years, except for those fundamental on the topic, with longer publication time. Letters to editor and editorials were also excluded. A descriptive analysis of articles was then carried out and the data were organized in order to support dentistry professionals to intervene safely in face of that complication. Final Considerations: Oroantral communication is a reality that can occur in the dental office. Its consequences are quite varied, and it is up to the dental surgeon, in addition to avoiding and diagnosing, to know how to intervene effectively when necessary, given that its complications may limit the patient's quality of life. Keywords: Maxillary sinus; Oral surgery; Oral Surgical Procedures.

This case series describes an alternative approach for removing ectopically erupted tooth in the maxillary sinus using an improved endoscopic approach known as modified endoscopic-assisted sinus surgery (MESS). We reviewed medical charts and radiographs of patients who underwent surgical removal of an ectopic tooth in the maxillary sinus using MESS. The surgical technique consisted of creating a bony window in the maxillary sinus wall with a pre-adapted microplate, which was repositioned after the tooth was removed. For all the patients included in this study, there were no postoperative complications clinically and radiographically. At the time of microplate removal, satisfactory bone regeneration was observed around the bony window margins of the maxillary sinus wall. MESS is an effective technique that modifies and integrates the techniques of the conventional Caldwell-Luc procedure (CLP) and functional endoscopic sinus surgery (FESS) and can be used to remove ectopic tooth in the maxillary sinus by avoiding the risk of postoperative sinus-related complications.

The maxillary sinus is a pneumatic space that, when inside it contains foreign bodies, can trigger infections or inflammations of different severities, causing the appearance of cysts, tumors or the development of chronic sinusitis. Imaging is essential for the correct diagnosis of the case, as well as surgical planning for the removal of the foreign body. The present work aims to describe a clinical-surgical case of a patient with an ectopic third molar in the maxillary sinus, emphasizing the clinical, radiographic characteristics, therapeutic conduct and surgical technique, as well as showing the importance of theoretical and practical knowledge regarding the subject. A 49-year-old female patient attended the university’s dental clinic for surgical removal using the Caldwell-Luc technique of the ectopic upper third molar in the left maxillary sinus. The surgical removal of the ectopic element in the maxillary sinus was effective in the case presented, preventing the patient from future pathologies that could affect the sinus, as well as the symptomatological reduction of sinusitis.
