EFFECTIVENESS OF "SWIM-UP" AND "GRADIENT" METHODS IN SPERM PREPARATION FOR ARTIFICIAL INSEMINATION

Objectives: Artificial insemination with sperm preparation and inseminate to the uterus is a common method of infertility treatment. Currently two methods used for sperm washing is “swim-up” and “gradient”. Materials and methods:cross-sectional description in 166 cycles of artificial insemination in Hue University Hospital from April, 2012 to March, 2013 in order to compare effectiveness between “swim-up” and “gradient” methods. Samples were collected randomly into two methods. Results:Results of sperm preparation in both methods are equivalent in terms of the following parameters total sperm count, total number of progressive sperm, total good morphology sperm. However, “gradient” method results in higher number of progressive sperm in case with slow motility (38.3% vs. 26.1%) or abnormal morphology (34.9% vs. 19.7 %) compared with “swim-up” method. Pregnancy rates after artificial insemination were similar between two preparation methods. Conclusion:In case of slow motility sperm and abnormal morphology, “gradient” method should be used to increase the number of progressive sperms. Pregnancy outcome depends on many factors other than preparation methods. Key words: Artificial insemination, sperm preparation, infertility treatment

In recent years, microfluidic chip-based sperm sorting has emerged as an alternative tool to centrifugation-based conventional techniques for in vitro fertilization. This prospective study aims to compare the effects of density gradient centrifugation and microfluidic chip sperm preparation methods on embryo development in patient populations with astheno-teratozoospermia. In the study, the semen samples of the patients were divided into two groups for preparation with either the microfluidic or density gradient methods. Selected spermatozoa were then used to fertilize mature sibling oocytes and the semen parameters and embryo development on days 3 and 5 were assessed. While the density gradient group was associated with a higher sperm concentration, motility (progressive and total) was significantly higher in the microfluidic chip group. No significant differences were observed in the fertilization rates or grade 1 (G1) and grade 2 (G2) proportions of the third-day embryos. Furthermore, while the proportions of the poor, fair and good blastocysts on day 5 did not differ significantly, excellent blastocysts (indicating high-quality embryos) were observed in a significantly higher proportion of the microfluidic chip group. When compared to the classical density gradient method, the microfluidic chip sperm preparation yielded sperm with higher motility and higher quality blastocysts at day 5; in patients with astheno-teratozoospermia.

Conjugate gradient method is verified to be efficient for nonlinear optimization problems of large-dimension data. In this paper, a penalized linear and nonlinear combined conjugate gradient method for the reconstruction of fluorescence molecular tomography (FMT) is presented. The algorithm combines the linear conjugate gradient method and the nonlinear conjugate gradient method together based on a restart strategy, in order to take advantage of the two kinds of conjugate gradient methods and compensate for the disadvantages. A quadratic penalty method is adopted to gain a nonnegative constraint and reduce the illposedness of the problem. Simulation studies show that the presented algorithm is accurate, stable, and fast. It has a better performance than the conventional conjugate gradient-based reconstruction algorithms. It offers an effective approach to reconstruct fluorochrome information for FMT.

The spermatotoxic effect of carbaryl in adult and young male rats has been examined. Carbaryl 50 and 100 mg/kg b.wt. Male fed 5 d/week for 60 days, caused dose and age- dependent decline in epididymal sperm count and sperm motility, an increase in sperm with abnormal morphology. The dose of 25 mg/kg/d was a 'No observed effect level' for the indices studied. Young animals in comparison to adults exhibited pronounced spermatotoxic effects.

Conjugate gradient methods are well-known methods which are widely applied in many practical fields. CD conjugate gradient method is one of the classical types. In this paper, a modified three-term type CD conjugate gradient algorithm is proposed. Some good features are presented as follows: (i) A modified three-term type CD conjugate gradient formula is presented. (ii) The given algorithm possesses sufficient descent property and trust region property. (iii) The algorithm has global convergence with the modified weak Wolfe–Powell (MWWP) line search technique and projection technique for general function. The new algorithm has made great progress in numerical experiments. It shows that the modified three-term type CD conjugate gradient method is more competitive than the classical CD conjugate gradient method.

This paper describes a modified three-term Hestenes–Stiefel (HS) method. The original HS method is the earliest conjugate gradient method. Although the HS method achieves global convergence using an exact line search, this is not guaranteed in the case of an inexact line search. In addition, the HS method does not usually satisfy the descent property. Our modified three-term conjugate gradient method possesses a sufficient descent property regardless of the type of line search and guarantees global convergence using the inexact Wolfe–Powell line search. The numerical efficiency of the modified three-term HS method is checked using 75 standard test functions. It is known that three-term conjugate gradient methods are numerically more efficient than two-term conjugate gradient methods. Importantly, this paper quantifies how much better the three-term performance is compared with two-term methods. Thus, in the numerical results, we compare our new modification with an efficient two-term conjugate gradient method. We also compare our modification with a state-of-the-art three-term HS method. Finally, we conclude that our proposed modification is globally convergent and numerically efficient.

The safety of the reproductive organs has a great positive impact on the human psyche, so early and accurate diagnosis of scrotal disorders by ultrasound was one of the necessities that puts the individual in the correct path of treatment. This study was conducted to evaluate the significance of ultrasound imaging technology in the diagnosis of varicocele and its relationship with sperm characteristics through semen analysis. We retrospectively reviewed a total of 281 male patients who underwent scrotum ultrasound at King Abdul-Aziz University Hospital-Radiology Department between 2017 and 2019 with age range of (11-76) years. Ultrasound result showed 119 (42.3%) had varicocele. Semen analysis was performed for 143 patients and the results showed a significant correlation between sperm characteristics and varicocele disorder (p < 0.001). Imaging of the scrotum by ultrasound is of great importance in diagnosing various andrology diseases, including varicocele, which have clear effects on sperm count and motility.
