BACKGROUND This paper presents a systematic literature review of existing remote health monitoring systems with special reference to neonatal intensive care (NICU). Articles on NICU clinical decision support systems (CDSSs) which used cloud computing and big data analytics were surveyed. OBJECTIVE The aim of this study is to review technologies used to provide NICU CDSS. The literature review highlights the gaps within frameworks providing HAaaS paradigm for big data analytics METHODS Literature searches were performed in Google Scholar, IEEE Digital Library, JMIR Medical Informatics, JMIR Human Factors and JMIR mHealth and only English articles published on and after 2015 were included. The overall search strategy was to retrieve articles that included terms that were related to “health analytics” and “as a service” or “internet of things” / ”IoT” and “neonatal intensive care unit” / ”NICU”. Title and abstracts were reviewed to assess relevance. RESULTS In total, 17 full papers met all criteria and were selected for full review. Results showed that in most cases bedside medical devices like pulse oximeters have been used as the sensor device. Results revealed a great diversity in data acquisition techniques used however in most cases the same physiological data (heart rate, respiratory rate, blood pressure, blood oxygen saturation) was acquired. Results obtained have shown that in most cases data analytics involved data mining classification techniques, fuzzy logic-NICU decision support systems (DSS) etc where as big data analytics involving Artemis cloud data analysis have used CRISP-TDM and STDM temporal data mining technique to support clinical research studies. In most scenarios both real-time and retrospective analytics have been performed. Results reveal that most of the research study has been performed within small and medium sized urban hospitals so there is wide scope for research within rural and remote hospitals with NICU set ups. Results have shown creating a HAaaS approach where data acquisition and data analytics are not tightly coupled remains an open research area. Reviewed articles have described architecture and base technologies for neonatal health monitoring with an IoT approach. CONCLUSIONS The current work supports implementation of the expanded Artemis cloud as a commercial offering to healthcare facilities in Canada and worldwide to provide cloud computing services to critical care. However, no work till date has been completed for low resource setting environment within healthcare facilities in India which results in scope for research. It is observed that all the big data analytics frameworks which have been reviewed in this study have tight coupling of components within the framework, so there is a need for a framework with functional decoupling of components.

IntroductionIn the general population, female children have been reported to have a survival advantage. For children admitted to paediatric intensive care units (PICUs), mortality has been reported to be lower in males despite the higher admission rates for males into intensive care. This apparent sex reversal in PICU mortality is not well studied. To address this, we propose to conduct a systematic literature review to summarise the available evidence. Our review will study the reported differences in mortality between males and females aged 0–17, who died in a PICU, to examine if there is a difference between the two sexes in PICU mortality, and if so, to describe the magnitude and direction of this difference.Methods and analysisStudies that directly or indirectly addressed the association between sex and mortality in children admitted to intensive care will be eligible for inclusion. Studies that directly address the association will be eligible for data extraction. The search strings were based on terms related to the population (children in intensive care), the exposure (sex) and the outcome (mortality). We used the databases MEDLINE (1946–2020), Embase (1980–2020) and Web of Science (1985–2020) as these cover relevant clinical publications. We will assess the reliability of included studies using the risk of bias in observational studies of exposures tool. We will consider a pooled effect if we have at least three studies with similar periods of follow up and adjustment variables.Ethics and disseminationEthical approval is not required for this review as it will synthesise data from existing studies. This manuscript is a part of a larger data linkage study, for which Ethical approval was granted. Dissemination will be via peer-reviewed journals and via public and patient groups.PROSPERO registration numberCRD42020203009.

Critically ill patients in the intensive care unit (ICU) have a high risk of developing malnutrition, and this is associated with poorer clinical outcomes. In clinical practice, nutrition, including enteral nutrition (EN), is often not prioritized. Resulting from this, risks and safety issues for patients and healthcare professionals can emerge. The aim of this literature review, inspired by the Rapid Review Guidebook by Dobbins, 2017, was to identify risks and safety issues for patient safety in the management of EN in critically ill patients in the ICU. Three databases were used to identify studies between 2009 and 2020. We assessed 3495 studies for eligibility and included 62 in our narrative synthesis. Several risks and problems were identified: No use of clinical assessment or screening nutrition assessment, inadequate tube management, missing energy target, missing a nutritionist, bad hygiene and handling, wrong time management and speed, nutritional interruptions, wrong body position, gastrointestinal complication and infections, missing or not using guidelines, understaffing, and lack of education. Raising awareness of these risks is a central aspect in patient safety in ICU. Clinical experts can use a checklist with 12 identified top risks and the recommendations drawn up to carry out their own risk analysis in clinical practice.

Objetivo: Analisar as publicações científicas acerca de alarmes de monitorização e do fenômeno Fadiga de alarmes em terapia intensiva. Métodos: Revisão integrativa de literatura realizada nas bases de dados Lilacs, PubMed e SciELO. Resultados: Após análise das produções elucidaram-se os conceitos e definições do fenômeno; fatores de predisposição e estratégias para sua minimização; a relação com a segurança do paciente, alcançando recorte temporal de 1993 a junho 2010. A fadiga de alarmes ocorre quando um grande número de alarmes encobre aqueles clinicamente significativos, possibilitando que alarmes relevantes sejam desabilitados, silenciados ou ignorados pela equipe. O número excessivo de alarmes torna a equipe indiferente, reduzindo seu estado de alerta, levando à desconfiança do sentido de urgência dos alarmes, resultando em falta de resposta a alarmes relevantes. Conclusão: O aparato tecnológico exige atenção dos profissionais para garantir a segurança do doente grave.
