Foot ulcers, infections, and deformity are some of the major sources of mortality and morbidity among the diabetic population. The relationship between glycated hemoglobin (HbA1c) and diabetic peripheral neuropathy (DPN) has been well established. There is a dearth of literature on the relationship between vibration perception threshold (VPT) and HbA1c values. So, the objective of the study was to determine the strength of linear relationship between HbA1c levels and vibration perception threshold in DPN. This cross-sectional study was conducted at Kasturba Hospital, Manipal, and diabetic foot screening camps held at various parts of Udupi district. Ethical approval was obtained from the Institutional Ethics Committee, Kasturba Hospital, Manipal (IEC:281/2017). A total of 534 participants ranging from 30 to 70 years of age and were diagnosed with type 2 diabetes mellitus on medications were included in the study. Neuropathy assessment consisting of monofilament and vibration perception threshold was done using Neurotouch beta version (Yostra Labs, Bengaluru, India). HbA1c measurement was done using turbidimetric inhibition immunoassay technique (Roche Diagnostics, Mannheim, Germany). Pearson correlation coefficient showed a moderate to good correlation between HbA1c and VPT ( r = .0.753, P < .001). Linear regression result has shown a significant relationship of VPT with HbA1c (4.033 [95% confidence interval = 3.67-4.39]). The present study has concluded that there is strong relationship between HbA1c values and VPT and could be a predictor for complications in the foot following DPN.

Ο σακχαρώδης διαβήτης είναι διαταραχή μεταβολισμού της γλυκόζης με μικροαγγειοπαθητικές και μακροαγγειοπαθητικές επιπλοκές. Η περιφερική νευροπάθεια και οι επιπλοκές από τα κάτω άκρα συνεισφέρουν στην αύξηση της νοσηρότητας και τη μείωση της ποιότητας ζωής των διαβητικών ασθενών. Η ξηροδερμία των πελμάτων μπορεί να αποτελεί μια πρώιμη εκδήλωση της διαβητικής νευροπάθειας και έχει συσχετισθεί με την εμφάνιση ελκών στα κάτω άκρα. Ωστόσο,δεν υπάρχει καμιά αξιολόγηση της επίδοσης των διαφόρων μεθόδων που χρησιμοποιούνται για την εκτίμηση της ξηροδερμίας των πελμάτων για τη διάγνωση της περιφερικής διαβητικής νευροπάθειας.Σκοπός της διατριβής ήταν να καταγραφούν οι δυνατοί τρόποι αξιολόγησης της ξηροδερμίας των πελμάτων στα άτομα με σακχαρώδη διαβήτη και η συσχέτισή τους με την περιφερική διαβητική νευροπάθεια.Υλικό και μέθοδοι: Συνολικά 98 διαβητικοί ασθενείς (38 γυναίκες, μέση ηλικία 58.8 ±11.0 έτη) συμμετείχαν στη μελέτη. Η ξηροδερμία εκτιμήθηκε με τη δερματική δοκιμασία NeuropadTM και με πίνακες που διαμορφώθηκαν από την EEMCO[European Group on Efficacy Measurement of Cosmetics: Overall dry skin score(ODS), και άθροισμα συμπτωμάτων όσον αφορά τα συμπτώματα αποφολίδωσης,τραχύτητας, ερυθρότητας και σπασιμάτων (SRRC)].Η ύπαρξη περιφερικής νευροπάθειας εκτιμήθηκε με την αξιολόγηση του δείκτη συμπτωμάτων νευροπάθειας(neuropathy symptom score, NSS), του δείκτη νευρολογικής ανικανότητας(neuropathy disability score, NDS), τον προσδιορισμό του οδού αντίληψης των δονήσεων (vibration perception threshold, VPT) και την αντίληψης πίεσης 10 g μεμονοινίδιο. Αποτελέσματα: Ποσοστό 10.9 % των ασθενών ήταν σε μονοθεραπεία μεμετφορμίνη, ενώ η πλειοψηφία των ασθενών ήταν ινσουλινοθεραπευόμενοι (60.9%). Σε συνδυασμό δύο αντιδιαβητικών φαρμάκων (πλην ινσουλίνης) βρίσκονταν 17.4 % των ασθενών, ενώ 9.8 % των ασθενών βρίσκονταν υπό θεραπεία με 3 αντιδιαβητικά φάρμακα (πλην ινσουλίνης). Στατιστικά σημαντικές θετικές συσχετίσεις παρατηρήθηκαν μεταξύ ODS και ηλικίας, ΔΜΣ και των δεικτών NSS, NDS και VPT, ενώ στατιστικά σημαντική αρνητική συσχέτιση βρέθηκε με τη δοκιμασία monofilament. Επιπλέον, τo σκορ SRRC συσχετιζόταν στατιστικά σημαντικά και αρνητικά με τον κνημονραχιόνιο δείκτη. Η καλύτερη ευαισθησία και ειδικότητα του NeuropadTM παρατηρήθηκε στα 15 λεπτά εφαρμογής-έναντι της εφαρμογής για 10 λεπτά-του τεστ (δεξιά: ευαισθησία 76%, ειδικότητα 48%, αριστερά: ευαισθησία 79%, ειδικότητα 50%). Τιμές ODS ≥2 και SRRC≥ 4 είχαν ευαισθησία 48% και 55%, αντίστοιχα ενώ η ειδικότητα ήταν 60% και 80%, αντίστοιχα για τη διάγνωση της περιφερικής διαβητικής νευροπάθειας.Συμπέρασμα: Από τις μεθόδους εκτίμησης της ξηροδερμίας των πελμάτων σε άτομα με σακχαρώδη διαβήτη η δερματική δοκιμασία NeuropadTM για 15 λεπτά υπερέχει ως προς τη διάγνωση της περιφερικής διαβητικής νευροπάθειας σε σύγκριση με τη χρήση των ODS και SRRC λόγω της απλότητας, της ευρείας διαθεσιμότητας και της καλύτερης επίδοσής της για τη διάγνωση της περιφερικής διαβητικής νευροπάθειας. Επιπλέον, υπάρχει σημαντική συσχέτιση μεταξύ της ξηροδερμίας του δέρματος στα πέλματα με τους δείκτες της περιφερικής διαβητικής νευροπάθειας.

Background: Diabetes Mellitus (DM) is a chronic disease and it is characterized by reduced insulin sensitivity and/or impaired insulin production. It affects approximately 415 million people worldwide and involves a variety of complications. DM has a number of complications, including diabetic neuropathy. All of these complications can have effects on body composition, vibration perception threshold (VPT), foot health and health-related quality of life (HRQoL). Objective: The aim of this study is to determine the correlation between the Timed Up and Go (TUG), VPT, Foot Health Status Questionnaire and 15-D Questionnaire in type 2 diabetes mellitus (T2DM) patients. Methodology: A total of 90 T2DM patients (56 men and 34 women) were evaluated on their body composition, VPT, the foot health status through the FHSQ, the HRQoL was evaluated through the 15-D Questionnaire and the TUG test was performed. Results: Statistically significant associations were found between TUG and lean and fat mass, VPT, the sections “General Foot Health” and “Physical Activity” in the FHSQ questionnaire, and the 15D total score and its sections “Mobility” and “Depression”. Conclusions: There is a moderate direct correlation between the Timed Up and Go and the fat mass percentage and the vibration perception threshold. Moreover, there is a moderate inverse correlation between Timed Up and Go and fat-free mass, foot health and health-related quality of life in T2DM patients. Therefore, Timed Up and Go can be a tool to assist clinicians in monitoring and managing T2DM patients.
