Distributor companies in carrying out activities require better employees. In determining the good employees, often meet obstacles, usually the assessment is still objective, especially in terms of contract extension. In accordance with these needs the writer will create a system that can determine the extension of the contract to be more objective and accurate. In planning the system, he development method used is the waterfall method and the development tools used are flowcharts, data flow diagrams and the method used for prediction is to use the fuzzy method, the writer chose fuzzy mamdani to help solve predictions in this system. The fuzzy mamdani method uses several criteria or variables including Absence, Achievement, Long Work, Cooperation, Graduates, Age. For the application of the application system supporting the decision of a contract extension is Microsoft Visual Basic 6.0. The final results of this study are employees whose work contracts are extended by a value of 1.7. Imron Rusdi 1.5. Arif Hamdani 1.5. Suanda 4. Risman 2. Wardoyo 2..

The information system for purchases and cash disbursements is one of the entire information systems. In retail sales companies need control of purchases and cash disbursements that are precise and fast. In an effort to minimize sudden out-of-stock inventory, it is necessary to secure an inventory to cope with a sudden out of stock due to delays in purchasing goods that occur at CV. New Space. This is what encourages the author to conduct research on CV. Angkasa Baru and choose the title "Purchasing Information System and Cash Distribution at CV. New Space. In writing this research systemin formation, the author uses the Microsoft Visual Basic 6.0 application, the research writing method uses the Research and Development (R&D) model, while the stages of this research model go through the stages of observation or interviews with parties related to the data required in writing this thesis, stages of designing the application system, designing specifications, validation test stages, revision of validation until the system passes. The development method used by the author is the prototype method, where the system created will be tailored to the needs of the user, if there is a change in the system it will be repaired until the user is satisfied with the system.

Abstract Information-Integration is vital for keeping manufacturing operations competitive. A case study approach has been adopted to better understand the role of information in integrated manufacturing. Information is now considered a corporate asset. Creation, processing, movement, and security of information is therefore as important as that of the products/services of an enterprise. The case studies have helped in identifying the issues involved in developing an information system and supporting software framework for a manufacturing enterprise. The case studies have helped in refining an integration model, and identifying the characteristics desirable in modeling methodologies and tools. This paper describes a case study dealing with integrated manufacture of optical fiber products. A phased development and implementation approach was adopted where a small, manageable slice of the system is considered for the case study followed by functional modeling (IDEF0) and data flow modeling (Data Flow Diagrams). This identifies the pieces of information of interest. The information relationships are modeled using Extended Entity Relationship (EER) diagrams which are then mapped on to a relational model. The relational tables thus obtained were implemented on a commercial Database Management System. The functional constraints and application interfaces were then built using SQL and commercial application interface tools. The sections in the paper describe the functional models, data flow diagrams, EER diagrams, relational database design, and user/application interfaces developed for the system. Implementation experiences and observations are discussed followed by plans for the next phase of the system.

Koperasi Kredit Karya Bhakti Kotabumi adalah salah satu Koperasi yang ada di kotabumi. Selama ini Koperasi Kredit Karya Bhakti Kotabumi khususnya dalam menentukan kelayakan peminjaman belum memanfaatkan terknologi komputerisasi dalam mengambil keputusan karna masih menggunakan proses yang manual. Metode yang digunakan dalam penelitian tugas akhir ini adalah metode Waterfall,   alat perancangan sistem yang digunakan adalah mapping chat, data flow diagram, struktur basis database, desain input, desain output, pengkodean, flowchat program, dan struktur menu aplikasi. Hasil dari penelitian ini adalah sebuah program SPK dalam bentuk Microsof Visual Basic 6.0 yang dapat membantu team surpey dalam pengolahan data pemohoan yang akan mengajukan peminjaman kredit. Basis data yang di buat adalah DB-PEMINJAM . Dengan demikian disarankan kepada bagian Team SurpeyKoperasi Kredit Karya Bhakti Kotabumi supaya menggunakan program aplikasi ini agar dalam pengambilan keputusan kelayakan peminjaman kredit dapat lebih efektif dan efisien
