Keselamatan dan keamanan pelayaran merupakan suatu kajian yang harus mendapatkan perhatian yang lebih serius pada decade terakhir  ini, mengingat semakin banyaknya kasus kecelakaan kapal yang diakibatkan oleh adanya human-error. Perhitungan stabilitas kapal dengan basic jumlah dan berat dari tiap party barang yang dimuat diatas kapal adalah salahsatu factor penentu dalam menyusun stowage barang diatas kapal terutama untuk kapal-kapal cellular yang memuat barang dalam kemasan container. Karena sebagaimana diketahui bersama bahwa pada lazimnya shipper hanya memberitahukan perkiraan berat barangnya saja tanpa mencantumkan berat containernya, sedangkan dalam kenyataannya barang tersebut dimuat diatas kapal bersamaan dengan containernya, sehingga bagi shipping line data berat barang yang disampaikan oleh shipper tersebut belum akurat. Olehkarena itu melalui organisasi maritime dunia (IMO) diterbitkanlah suatu amandemen yang bersifat mandatory kepada semua shipper diwajibkan untuk menyampaikan verifikasi hasil timbang barang dengan benar termasuk berat containernya secara tertulis kepada shipping-line sebelum container tersebut dikapalkan. Bila tidak ada surat pernyataan (verifikasi) tentang berat barang dan containernya, maka shipping line dilarang untuk memuat container tersebut. Pernyataan atau verifikasi ini dikenal dengan istilah VGM (verified Gross Mass)

The potential risks to vessels and crew in merchant shipping stem from human error, vulnerability to environmental influences, failure of shipboard systems and equipment. In case of transportation of heavy and oversized cargo, the preceding list can be expanded to include potential hazards directly associated with the cargo. This study identified a system of potential negative events in the transportation of oversized and heavy cargo under the influence of multiple factors. Negative events are divided into two categories: those occurring during loading/unloading and those occurring during transportation. The consequences of these negative events for cargo and operational condition of the ship are also identified. Six basic options for the operational condition of the vessel in the transportation process of oversized and heavy cargo have been determined. The conceptual model of the combination of factors affecting the operational condition of the vessel during the transportation of oversized and heavy cargo is formulated, and the chain of formation of the probability of negative events during the transportation of oversized and heavy cargo is identified and mathematically described. A basic pattern of relationship between various negative events occurring during transportation of oversized and heavy cargoes and ship's operational conditions is established. The obtained results allow the probability of possible negative events and change of ship's condition due to such influences during the transportation of the cargo category concerned to be estimated in practice, taking into account the specifics of a particular port, vessel and shipping line.

Abstract. Twin-engine propeller aircraft accidents occur due to mechanical reasons as well as human error, such as misidentifying a failed engine. This paper proposes a visual indicator as an alternative method to the dead leg–dead engine procedure to identify a failed engine. In total, 50 pilots without a multi-engine rating were randomly assigned to a traditional (dead leg–dead engine) or an alternative (visual indicator) group. Participants performed three takeoffs in a flight simulator with a simulated engine failure after rotation. Participants in the alternative group identified the failed engine faster than the traditional group. A visual indicator may improve pilot accuracy and performance during engine-out emergencies and is recommended as a possible alternative for twin-engine propeller aircraft.

In an effort to improve electricity services to customers, there needs to be a balance between the supply of electricity and revenue. Payment methods made by the Perusahaan Listrik Negara (PLN) use Postpaid and Prepaid methods. Because services with postpaid methods better meet customer satisfaction, this method is chosen. Sales Accounting Information System is an information technology-based accounting application to support financial transactions in a company to run faster, more precisely and accurately at a reasonable cost. In its operation, it takes competent human resources to not be a source of problems (human error) and adequate infrastructure. To overcome the obstacles that will arise, whether triggered by the application or by the human resources themselves, it is necessary to control both aspects. Based on this research it shows that the application  of  Sales Accounting Information Systems and existing human resources is sufficiently supportive  and  reliable  in  supporting the creation of good control as it is proven that the level of acceptance of sales targets is realized effectively and efficiently.

Due to the different characteristics of the piece goods (e.g. size and weight), they are transported in general cargo warehouses by manually-operated industrial trucks such as forklifts and pallet trucks. Since manual activities are susceptible to possible human error, errors occur in logistical processes in general cargo warehouses. This leads to incorrect loading, stacking and damage to storage equipment and general cargo. It would be possible to reduce costs arising from errors in logistical processes if these errors could be remedied in advance. This paper presents a monitoring procedure for logistical processes in manually-operated general cargo warehouses. This is where predictive analysis is applied. Seven steps are introduced with a view to integrating predictive analysis into the IT infrastructure of general cargo warehouses. These steps are described in detail. The CRISP4BigData model, the SVM data mining algorithm, the data mining tool R, the programming language C++ for the scoring in general cargo warehouses represent the results of this paper. After having created the system and installed it in general cargo warehouses, initial results obtained with this method over a certain time span will be compared with results obtained without this method through manual recording over the same period.

Prinsip kehati-hatian merupakan sebuah prinsip yang mendasarkan pada pemikiran tidak adanya temuan atau pembuktian ilmiah yang konkrit, konklusif dan pasti tidak dapat dijadikan alasan untuk menunda upaya-upaya mencegah kerusakan lingkungan. Prinsip ini merupakan jawaban atas kebijakan pengelolaan lingkungan yang didasarkan di awal kegiatan pelaku usaha. Pengaman prinsip kehati-hatian dilakukan melalui kebijakan negara berupa, tindakan pengawasan dan administratif yaitu ijin berlayar kapal tanker di terirorial Indonesia. Akan sangat merugikan apabila bendera kapal yang terindikasi flag of convinience dengan persyaratan mudah dan dibawah standar diijinkan masuk teritori Indonesia. Pembuktian terdahulu pada kapal MT. King Fisher dan MT. Lucky Lady berbendera Malta seharusnya sudah menjadi sebuah pembuktian ilmiah akan dampak dan risiko yang nyata. Pembuktian ilmiah dimaksud dalam unsur kehati-hatian yang tidak dapat dielakkan dalam kasus pencemaran minyak karena kecelakaan kapal tanker dibuktikan melalui kerugian yang berhasil dibuktikan melalui proses penggantian kerugian dari kedua kapal berbendera Malta. Kasus kedua kapal berbendera Malta yang dicarter oleh Pertamina Cilacap keduanya mengalami kecelakaan berkaitan dengan human error.

Abstract The medical imaging field has grown significantly in recent years and demands high accuracy since it deals with human life. The idea is to reduce human error as much as possible by assisting physicians and radiologists with some automatic techniques. The use of artificial intelligent techniques has shown great potential in this field. Hence, in this paper the neuro fuzzy classifier is applied for the automated characterization of atheromatous plaque to identify the fibrotic, lipidic and calcified tissues in Intravascular Ultrasound images (IVUS) which is designed using sixteen inputs, corresponds to sixteen pixels of instantaneous scanning matrix, one output that tells whether the pixel under consideration is Fibrotic, Lipidic, Calcified or Normal pixel. The classification performance was evaluated in terms of sensitivity, specificity and accuracy and the results confirmed that the proposed system has potential in detecting the respective plaque with the average accuracy of 98.9%.
