I’ve been a medical writer and author’s editor for 45 years. I have read the instructions for authors in dozens of medical journals. I know what authors (and author’s editors) think of these instructions, at least among those who know that journals actually have instructions for authors. For almost as long, I’ve been a member of four professional societies concerned with scientific publishing, and I know a lot of editors-in-chief of medical journals. I appreciate their desire to have authors follow the instructions when preparing manuscripts, at least among those editors who remember that their journals have such instructions and insist, at least occasionally, that they be followed.

As the medical profession becomes more and more of a commercial enterprise, commentators are subjecting conflicts of interest in medicine to increasing scrutiny. However, one critical area of conflict has largely escaped discussion—the conflicts of interest raised by the advertising policies of medical journals. Moreover, when these conflicts are discussed, they are examined almost exclusively in terms of the concerns that they pose for journal editors. Yet, there is a second critical concern with journal advertising policies. The policies also create serious conflicts of interest for the professional societies that own medical journals.In this article, we will discuss the conflicts of interest that are raised for journal editors and professional societies by journal advertising policies, and we will conclude that the policies are exactly backward. Currently, medical journals rely on advertisements from pharmaceutical companies and other health-related businesses and avoid—indeed exclude—advertisements from consumer-oriented companies, like producers of automobiles, golf equipment, or jewelry. We submit that the medical journals, the medical profession, and the public would be better served if consumeroriented advertisement were preferred over health-related advertising.

Peer reviewed scientific publishing is critical for communicating important findings, interpretations and theories in any branch of science. While the value of peer review is rarely doubted, much concern is being raised about the possible biases in the process. I argue here that most of the biases originate in the evolved innate tendency of every player to optimize one’s own cost benefits. Different players in the scientific publishing game have different cost-benefit optima. There are multiple conflicts between individual optima and collective goals. An analysis of the cost-benefit optima of every player in the scientific publishing game shows how and why biases originate. In the current system of publishing, by optimization considerations, the probability of publishing a ‘bad’ manuscript is relatively small but the probability of rejecting a ‘good’ manuscript is very high. By continuing with the current publishing structure, the global distribution of the scientific community would be increasingly clustered. Publication biases by gender, ethnicity, reputation, conformation and conformity will be increasingly common and revolutionary concepts increasingly difficult to publish. Ultimately, I explore the possibility of designing a peer review publishing system in which the conflicts between individual optimization and collective goal can be minimized. In such a system, if everyone behaves with maximum selfishness, biases would be minimized and the progress towards the collective goal would be faster and smoother. Changing towards such a system might prove difficult unless a critical mass of authors take an active role to revolutionize scientific publishing.

The role of networks is swiftly increasing in the production and communication of scientific knowledge. Network aspects have, therefore, an ever growing importance in the analysis of the scientific enterprise, as well. The present paper demonstrates some techniques of studying the network of scientific journals on the subject of seeking the position of Orvosi Hetilap (Hungarian Medical Journal) in the international journal network.
