Information Value and Quality for the Health Sector

The approaches of information value and quality can be discussed in any area of activity. The health sector is one of them, and the scientific literature has always been seen as a valuable tool in patient care. A number of studies highlight the impact of scientific literature on clinical decision-making and patient care costs. An important part of the literature in question is a result of online literature searching. The PubMed database contains over 22 million citations for biomedical literature and is widely used. The author, therefore, present its main features and review the leading studies developed to evaluate the retrieved information and to find optimal search strategies. Finally, she presents a study in the specific area of critical care, describing the strategies used by a group of doctors and the retrieval results achieved on two different occasions.

A review of: Marshall, Joanne Gard. “The Impact of the Hospital Library on Clinical Decision Making: the Rochester Study.” Bulletin of the Medical Library Association 80.2 (1992): 169-78. Objective – To determine the impact of hospital library services on clinical decision making. Design – A descriptive survey. Setting – Fifteen hospitals in the Rochester area of New York, United States of America. Seven hospitals were in the city of Rochester, and eight were in surrounding rural communities. Subjects – Active physicians and residents affiliated with the Rochester hospitals. Methods – This study built upon the methodology used in an earlier study by D. N. King of the contribution of hospital libraries to clinical care in Chicago. Lists were compiled of all the active physicians and residents who were affiliated with the Rochester hospitals. In order to ensure that there was a reasonable number of participants from each hospital, and that librarians in hospitals with larger numbers of staff were not overburdened with requests, predetermined percentages were set for the sample: 10% of active physicians from hospitals with more than 25 medical staff members, 30% from hospitals with less staff, and 30% of residents and rural physicians. This resulted in a desirable sample size of 448. A systematic sample with a random start was then drawn from each hospital’s list, and physicians and residents were recruited until the sample size was achieved. Participants were asked to request information related to a clinical case from their hospital library, and to evaluate its impact on patient care, by responding to a two-page questionnaire. Main results – Based on usable questionnaires, there was an overall response rate of 46.4% (208 of 448). Eighty percent of the respondents stated that they probably (48%) or definitely (32.4%) handled a clinical situation differently due to the information received from the library. In terms of the specific aspects of care for which changes were made, 71.6% reported a change in advice given to the patient, 59.6% cited a change in treatment, 50.5% a change in diagnostic tests, 45.2% a change in drugs, and 38.5% a change in post-hospital care or treatment. Physicians credited the information provided by the library as contributing to their ability to avoid additional tests and procedures (49%), additional outpatient visits (26.4%), surgery (21.2%), patient mortality (19.2%), hospital admission (11.5%), and hospital-acquired infections (8.2%). In response to a question about the importance of several sources of information, the library received the highest rating amidst other sources including lab tests, diagnostic imaging, and discussions with colleagues. Conclusion – This study validates earlier research findings that physicians view the information provided by hospital libraries as having a significant impact on clinical decision making. Library supplied information influences changes to specific aspects of care as well as the avoidance of adverse events for patients. The significance of this influence is underscored by the finding that relative to other sources, information obtained from the hospital library was rated more highly.

BACKGROUND Many clinical questions arise during patient encounters that clinicians are unable to answer. An evidence-based medicine approach expects that clinicians will seek and apply the best available evidence to answer clinical questions. One commonly used source of such evidence is scientific literature, such as that available through MEDLINE and PubMed. Clinicians report that 2 key reasons why they do not use search systems to answer questions is that it takes too much time and that they do not expect to find a definitive answer. So, the question remains about how effectively scientific literature search systems support time-pressured clinicians in making better clinical decisions. The results of this study are important because they can help clinicians and health care organizations to better assess their needs with respect to clinical decision support (CDS) systems and evidence sources. The results and data captured will contribute a significant data collection to inform the design of future CDS systems to better meet the needs of time-pressured, practicing clinicians. OBJECTIVE The purpose of this study is to understand the impact of using a scientific medical literature search system on clinical decision making. Furthermore, to understand the impact of realistic time pressures on clinicians, we vary the search time available to find clinical answers. Finally, we assess the impact of improvements in search system effectiveness on the same clinical decisions. METHODS In this study, 96 practicing clinicians and final year medical students are presented with 16 clinical questions which they must answer without access to any external resource. The same questions are then represented to the clinicians; however, in this part of the study, the clinicians can use a scientific literature search engine to find evidence to support their answers. The time pressures of practicing clinicians are simulated by limiting answer time to one of 3, 6, or 9 min per question. The correct answer rate is reported both before and after search to assess the impact of the search system and the time constraint. In addition, 2 search systems that use the same user interface, but which vary widely in their search effectiveness, are employed so that the impact of changes in search system effectiveness on clinical decision making can also be assessed. RESULTS Recruiting began for the study in June 2018. As of the April 4, 2019, there were 69 participants enrolled. The study is expected to close by May 30, 2019, with results to be published in July. Evidence-based practice requires astute clinicians to blend our best clinical judgment with the best available external evidence and the patient's own values and expectations. Sometimes, we value one more than another during clinical decision-making, though it is never wise to do so, and sometimes other factors that we are unaware of produce unanticipated clinical outcomes. Sometimes, we feel very strongly about one clinical method or another, and hopefully that belief is founded in evidence. Some beliefs, however, are not founded in evidence. The sound use of evidence is the best way to navigate the debates within our field of practice.

AbstractThe uptake of the QbTest in clinical practice is increasing and has recently been supported by research evidence proposing its effectiveness in relation to clinical decision-making. However, the exact underlying process leading to this clinical benefit is currently not well established and requires further clarification. For the clinician, certain challenges arise when adding the QbTest as a novel method to standard clinical practice, such as having the skills required to interpret neuropsychological test information and assess for diagnostically relevant neurocognitive domains that are related to attention-deficit hyperactivity disorder (ADHD), or how neurocognitive domains express themselves within the behavioral classifications of ADHD and how the quantitative measurement of activity in a laboratory setting compares with real-life (ecological validity) situations as well as the impact of comorbidity on test results. This article aims to address these clinical conundrums in aid of developing a consistent approach and future guidelines in clinical practice.
