Abstract Background: A year ago, a new type of coronavirus emerged. Once treated for severe and critical COVID-19 infections, patients are discharged from the hospital for further treatment and rehabilitation. The aim of this study was to evaluate the efficacy and safety of a newly developed comprehensive rehabilitation program based on traditional Chinese medicine (TCM) in the rehabilitation of patients with severe and critical COVID-19.Methods: We recruited a total of 72 patients who had suffered from severe and critical COVID-19 infections and were undergoing rehabilitation in Chongqing, China. A comprehensive rehabilitation program was formulated according to the TCM syndromes of these patients. Specific treatments included oral TCM, Baduanjin, Moxibustion, Acupoint application, and foot baths. Prior to the initiation of treatment, and four weeks after the initiation of treatment, we carried out a range of assessments, including the TCM Syndrome curative effect score, the modified Medical Research Council (mMRC) dyspnea score, the St. George's Respiratory Questionnaire, the Short Form (SF)-36 Quality of Life Scale, and the 6-minute walking test. We also carried out CT scans, serology tests. Statistical analysis was also conducted to evaluate the efficacy and safety of TCM on severe and critical COVID-19 patients.Results: Analysis showed that there were significant differences (P < 0.05) when compared before and after four weeks of TCM treatment, in terms of the TCM syndrome curative effect score, mMRC dyspnea score, St. George's Respiratory Questionnaire score, SF-36 Quality of Life Scale score, and the 6-minute walking test. We also identified significant differences (P < 0.05) between these two timepoints, with regards to the neutrophil ratio, lymphoid cell ratio, lymphocytes, platelets, red blood cells, and hemoglobin. There were no significant differences when compared between the two timepoints with regards to white blood cells and neutrophils (P > 0.05). The efficacy of chest CT scans was 83.9%. Logistic regression showed that the CT scans of patients who did not take the TCM decoction did not improve significantly. The higher a patient’s score on the 6-minute walking test, the higher the probability of no significant improvement on the CT scan.Conclusions: A comprehensive rehabilitation program based on TCM improved a number of clinical parameters in patients suffering from severe and critical COVID-19 infections, including quality of life, clinical symptoms, exercise endurance, and respiratory function. TCM also enhanced lymphocytes, lymphocyte ratio, platelet, red blood cell (RBC) count, and hemoglobin content. TCM also appeared to contribute to the absorption of lung lesions.

Background. The aim of this study was to analyse the relationship between the clinical manifestations, disease severity based on radiography images, functional activity level, and quality of life in patients with knee osteoarthritis in a rural population living in Serbian enclaves in Kosovo, as well as to determine the correlation between the WOMAC and the EQ-5D questionnaire in this population. Method. The cross-sectional study was conducted at the Internal Medicine Clinic, Clinical Hospital Center Pristina-Gracanica, located in Laplje Selo from February to December 2013. One hundred patients with confirmed (American College of Rheumatology criteria) knee osteoarthritis completed the EQ-5D and Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) questionnaires, rated pain on a visual analogue scale (VAS), and underwent knee radiographic examinations. Result. Most patients were obese with moderate radiographic changes according to the Kellgeren–Lawrence scale and suffered from very severe pain according to the VAS scale. The duration of disease significantly correlated with the WOMAC scores, VAS score, and all of the scores on the EQ-5D, except for mobility. The age of participants showed a similar correlation with the same variables. The patients with higher Kellgren–Lawrence scores (3-4) were significantly older, with a significantly higher body mass index (BMI) and longer duration of disease than patients with lower scores (1-2). Significantly higher VAS, pain/discomfort EQ-5D, and WOMAC pain and function scores were also recorded among patients with more significant radiological changes. The correlations between WOMAC and EQ-5D were satisfactory. Conclusion. The severity of clinical manifestations and radiographic area changes may affect functional ability and the quality of life in knee OA patients living in rural areas, which requires adequate treatment and physical therapy.

<p class="abstract"><strong>Background:</strong> Knee osteoarthritis is a progressive degenerative joint disease and remains a leading cause of pain, physical impairment and decline in health-related quality of life in adults.   Despite its incidence being amongst the highest in chronic diseases, effective biomarkers are not available to assist in its management. The main goal of this study is to identify mediators that serve as biomarkers and investigate if the levels of these biomarkers will be correlated to the efficacy of a rehabilitation program.</p><p class="abstract"><strong>Methods:</strong> This is a prospective cohort study with 65 participants. Patients with mild-to-moderate symptomatic knee osteoarthritis will be recruited. The Rehabilitation Program will consist of three session/week during eight weeks. Assessment about functional evaluation will be performed before and after treatment, using the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) and EuroQOL-5D (Euro quality of life - five dimension) scales, Visual Analog Scale (VAS), and physical function tests (time up and go, isometric strength testing and kinematic gait analysis). Serum levels of classical pro-inflammatory cytokines, hyaluronan and high mobility group box 1 protein (HMGB-1) will be evaluated. The primary outcome is the change in WOMAC scale from baseline to end. Statistical analyses will be used to determine correlation of physical improvement and serum biomarkers. Adverse events will be monitored throughout the study.</p><p class="abstract"><strong>Conclusions: </strong>This trial expect to study the correlation between the anti-inflammatory effects of rehabilitation program derived factors that may be involved in suppressing cytokine induction via suppressing HMGB-1.</p><p class="abstract"><strong>Trial registration:</strong> Clinicaltrials.gov - NCT02964624.</p>

Abstract Due to the exacerbation of the disease, the rehabilitation of patients with rheumatoid arthritis is often limited. The aim of the study was to analyze the impact of a comprehensive rehabilitation on a subjective perception of pain and quality of life in patients with rheumatoid arthritis depending on the level of disease activity. The study involved 58 women with rheumatoid arthritis aged 18–60, who underwent a 4-weeks comprehensive rehabilitation program. The assessment included the disease activity level on the DAS28 scale, pain intensity on the 10-point pain scale (VAS) and the value of the CRP protein. The HAQ-DI and KALU questionnaire were used to assess the quality of life. In both groups (group A—DAS28 < 4.2, group B—DAS28 ≥ 4.2) the statistically significant effects in decreasing the level of pain and improvement of quality of life were observed. This indicates the need and effectiveness of rehabilitation regardless of the level of activity of rheumatoid arthritis according to the DAS28 scale. There were no significant changes in the CRP protein level. In conclusion, the rehabilitation of patients with moderate to high disease activity is effective and the success of comprehensive rehabilitation is demonstrated by the decrease of the DAS28 score and the pain level reported by patients, as well as improving self-assessment of health and quality of life.

Abstract Background: Comprehensive rehabilitation therapy based on Traditional Chinese Medicine (TCM) has been widely applied in various cancer treatments in China. Thus far, Chinese herbal medicine (CHM) has been shown to be effective in reducing the adverse effects of chemotherapy and improving the quality of life (QoL) during chemotherapy. In the present study, a more rational and effective therapeutic scheme is to be applied to assess the effects of comprehensive rehabilitation program based on CHM and Liuzijue (LZJ) exercise combined with adjuvant chemotherapy on patients with postoperative non-small cell lung cancer (NSCLC). Methods: A multi-center, randomized clinical trial is to be performed 354 Ib-IIIa NSCLC patients in 5 centers in China. Patients satisfying the inclusion criteria are to be randomly divided into 3 group according to the ratio of 1:1:1, namely intervention group A (IGA), intervention group B (IGB) and controlled group (CG). Each group will receive adjuvant platinum-based doublet chemotherapy for 4 cycles. IGA participants receive chemotherapy combined with CHM and LZJ exercise, IGB participants receive chemotherapy combined with CHM and rehabilitation education, and CG participants receive chemotherapy combined with placebo and rehabilitation education. The primary outcome is QoL (The European Organization for Research and Treatment of Cancer: EORTC QLQ LC43), the secondary outcomes include: (1)2 years disease-free survival rate and disease-free survival (DFS); (2) TCM symptoms; (3) tumor markers; (4) safety and adverse events (NCI CTC version 4.0). Discussion: Our previous study reported that TCM in combination with chemotherapy could lower the overall incidence of adverse events but increase the digestive and gastrointestinal (GI) side effects than chemotherapy alone in postoperative NSCLC. The study is to lay a basis for the effectiveness of chemotherapy with or without comprehensive rehabilitation program in QoL for patients with postoperative NSCLC.
