The best intramedullary nail for the treatment of unstable AO/OTA 31.A2.1-3 fractures remains uncertain. A total of 237 patients (45 male, 192 female) were eligible for the assessment with an average age of 81.9 ± 10.5 years and a minimum follow-up of six months. We assessed the cut-out rate, the TAD and calTAD (Tip Apex distance) of three different implants. An overall cut-out rate of 2.5% (n = 6) was observed. The cut-out rate was 1.2% (n = 1) in the Proximal Femur Nail Antirotation (PFNA) group, 3.7% (n = 3) in the Gamma Nail group and 2.9% (n = 2) in the Gamma3® with a U-Blade (RC) lag-screw group. The TAD and calTAD differed between the cut-out and non-cut group—20.0 mm vs. 18.5 mm and 13.1 mm vs. 15.3 mm, respectively. A significantly higher TAD of 32.5 mm could be seen in the cut-out after PFNA (p < 0.0001). The only significant change in follow-up using Parker’s ratio was observed in the PFNA group (p < 0.0001). The rate of patients requiring surgery after complications was 8.4% (n = 20) without any significant difference between the three groups. The PFNA blade showed significant migration within the femoral head, however the cut-out rate remained the smallest compared to Gamma3 with or without additional U-Blade (RC) lag screw.

The analysis of results of surgical treatment of 157 patients with proximal femur fractures was presented. In all cases the osteosynthesis using fixator PFN was performed in clinic of traumatology amd orthopedics of Moscow Municipal Hospital N 71 from 2005 till 2009. The authors concluded: proximal femur nail is mini-invasive and mini-traumatic fixator that is essential at treatment of elderly and old patients.

Pertrochanteric fractures usually occur in patients over 65 years of age, with greater loss of skeletal mass (osteoporosis). Nonsurgical methods of treatment are accompanied by relatively high lethality rate. Moreover, they do not produce satisfactory anatomical and functional results. Surgical treatment by using dynamic implants represents a method of choice in the fixation of pertrochanteric fractures. This paper presents the treatment results of 110 patients, 61 with pertrochanteric fractures, who were surgically treated by the dynamic method of internal fixation, and 49 patients who were treated by the method of external fixation. Dynamical implants enabled both dynamization and compression of the fracture in the axis of the neck, as well as the diaphysis of the femur, which lowered the risk of mechanical complications, and, at the same time, provided effective healing of the fracture, early activation, and mobilization of the patients on whom the surgery was performed. In patients infected by various diseases, for whom surgical trauma represents a life threat, the external fixation is recommended as a method of choice.

Resumo Objetivo Avaliar e comparar o tempo total do procedimento cirúrgico e a exposição ao raio X no intraoperatório em diferentes técnicas de fixação das fraturas transtrocanterianas do fêmur em pacientes idosos, utilizando técnicas extramedulares e intramedulares baseadas em parafuso de tração cefálico. Métodos Foram avaliados no serviço de ortopedia e traumatologia do nosso hospital 107 pacientes com fraturas transtrocanterianas, sendo 34 do sexo masculino e 73 do sexo feminino, com idade mínima de 61 anos e máxima de 101 anos. As fraturas fixadas, utilizando a técnica com dynamic hip system (DHS, na sigla em inglês) somaram 21 pacientes; em 55 pacientes, foi utilizado o proximal femur nail (PFN, na sigla em inglês) standard; e em 31 idosos, foi optado pelo uso do gama nail standard. Todos os procedimentos foram realizados pelo mesmo cirurgião acompanhado de sua equipe de auxiliares de enfermagem, assim como pelo mesmo técnico de radiologia manuseando o mesmo intensificador de imagens. Foram avaliados o tempo total da cirurgia (em minutos) e a emissão de raios X medida em centigrays. Resultados A fixação das fraturas transtrocanterianas com PFN proporciona um menor tempo cirúrgico com uma diferença estatística significativa (p = 0,013), quando comparada com as demais técnicas utilizadas entre os grupos envolvidos. Foi observada, também, uma menor exposição intraoperatória aos raios X (p = 0,015), a qual foi estatisticamente relevante quando utilizado o DHS como método de fixação comparado com o gama nail e o PFN. Conclusão Apesar do PFN ter o menor tempo de cirurgia, a técnica do DHS se mostrou com menores níveis de exposição dentro da amostra estudada.
