Topical corticosteroids are currently recommended only for short-term management of flares of lichen sclerosus, with efficacy in halting disease progression. Given the chronic nature of this condition, there is a lack of literature surrounding the chronic effects of topical corticosteroids on the male genitalia with many dermatologists avoiding prescribing long term. This case report aims to provide anecdotal observation for the long-term use of topical corticosteroids and details the long-term follow-up of an individual who used potent and superpotent topical corticosteroids for over 25 years without significant demonstrable side effects. A short review on relevant literature is provided.

AbstractPseudohypoaldosteronism type 1 (PHA1) is a life-threatening disorder for two reasons, first because it causes severe hyperkalemia and second because the rarity of the disorder means that diagnosis is often delayed due to the lack of clinician familiarity with this condition. In this case report, we have described how even to an unsuspecting mind and eye, a systematic approach can lead to reversal of the severe hyperkalemia and arrive at the diagnosis of PHA1 as a cause of hyperkalemia. Long-term management can be successful only with dedicated care, and the prognosis is unfortunately worsened by the lack of availability of sodium-K resin in many countries, including India.

This case report speculates that the prolonged vibrations from enduro off-road sports are deleterious to the spine. The results of this case report may also aid sports physicians in better understanding this complex and relatively unknown phenomenon. No published data are present in the current literature that demonstrate the correlation between early spine osteoarthritis from enduro motorcycle overuse and the long-term management effects of a non-invasive kinesiological approach to reduce pain and inflammation and improve spine mobility and muscle strength.

Actinomycosis is a subacute or chronic suppurative bacterial infection which caused because of filamentous gram-positive, anaerobic to microaerophilic nonacid fast bacilli primarily of the genus Actinomyces that normally colonize the mouth, colon, and vagina. Primary cutaneous actinomycosis is a rare entity and is generally associated with trauma. A 61- year-old Yemeni male firstly presented to the surgical department in King Fahd Central Hospital with multiple swelling, petted lesions and boring of sensation in the right foot. Local examination showed multiple erythematous nodules and plaques with discharge in the right leg, and active sinus was not determined.

A 7-year-old spayed female Pomeranian dog was evaluated for a 6-month history of intractable coughing and dyspnoea. The cough was unresponsive to antibiotics and a bronchodilator medication prescribed by the previous animal hospital. No abnormalities were identified on the blood work, echocardiography and radiography. However, computed tomography revealed mild bronchiectasia. Based on the history, physical examination and non-specific diagnostic imaging findings, the dog was diagnosed with chronic bronchitis. The cough failed to resolve, despite an attempt to manage it with prednisolone, theophylline, codeine and N-acetylcysteine. With the owner’s consent, we stopped all previous medications and finally tried the leukotriene receptor antagonist zafirlukast. The cough progressively improved and had mostly resolved four weeks after starting the administration of zafirlukast. The dog remained in complete remission without recurrence for seven months. This case report is the first to describe the successful long-term management of chronic cough with zafirlukast in a dog.
