Difference of Procalcitonin Levels in Gram-Positive and Gram-Negative Bacterial Sepsis Patients of Indonesia Army Central Hospital Gatot Soebroto in 2016

Introduction: Procalcitonin is known as a marker of infection and indicator for severity of infections. In sepsis, elevated procalcitonin levels in blood have a significant value that can be used as a sepsis biomarker. The aim of this study was to determine the mean difference of procalcitonin levels in Gram-positive and Gram-negative bacterial sepsis patients.Methods: This study used quantitative method with cross sectional approach. The sample of this study were bacterial sepsis patients of Indonesia Army Central Hospital Gatot Soebroto in 2016 which were divided into two groups: Gram-positive and Gram-negative bacterial sepsis patients with the number of each group was 30 samples. The data were analyzed by using independent t test.Results: This study showed that mean levels of procalcitonin in Gram-positive bacterial sepsis patients was 6.47 ng/ml and Gram-negative was 66.04 ng/ml. There was a significant difference between mean levels of procalcitonin in Gram-positive and Gram-negative bacterial sepsis patients of Indonesia Army Central Hospital Gatot Soebroto in 2016 with p value = 0.000 (p < 0.05).Conclusion: The mean difference of procalcitonin levels in Gram-negative bacterial sepsis patients were higher than Gram-positive bacterial sepsis patients, because Gram-negative bacteria have lipopolysaccharide which is a strong immunostimulator and increases TNF-α production higher than Gram-positive bacteria.

Đánh giá bước đầu ứng dụng tia plasma lạnh trong điều trị hỗ trợ làm lành vết thương phần mềm trên bệnh nhân Covid-19 nặng

TÓM TẮT Đặt vấn đề: Plasma là môi trường chứa các vật chất không còn giữ được cấu trúc phân tử của mình mà bị ion hóa. Thể plasma lạnh được ứng dụng rộng rãi trong Y học với tính an toàn và hiệu quả được khẳng định trong nhiều nghiên cứu. Nghiên cứu này thực hiện tại Trung tâm Hồi sức Tích cực điều trị bệnh nhân COVID-19 của Bệnh viện Trung Ương Huế tại TP.Hồ Chí Minh nhằm mục đích (1) nhận định đặc điểm chung, đặc điểm lâm sàng của bệnh nhân COVID-19 nặng có sang thương phần mềm và (2) đánh giá bước đầu tác dụng tia plasma lạnh trong điều trị hỗ trợ làm lành vết thương phần mềm đối với bệnh nhân COVID-19 nặng Đối tượng, phương pháp: Khảo sát nhanh trong 45 ngày các bệnh nhân bị COVID-19 nặng có sang thương phần mềm đang điều trị tại Trung tâm Hồi sức Tích cực điều trị bệnh nhân COVID-19 trực thuộc Bệnh viện Trung Ương Huế tại TP.Hồ Chí Minh, nghiên cứu mô tả cắt ngang. Kết quả: Trong 915 bệnh nhân bị COVID-19 nặng, có 20 trường hợp bị thương tổn phần mềm được điều trị bằng chiếu tia Plasma lạnh, tỷ lệ nam, nữ 1 : 1, 70% nguyên nhân do loét ép độ I, II, III, 60% tổn thương vùng lưng, mông, cùng cụt. 55,0% thương tổn trước khi chuyển viện đến trung tâm. Sau 14 ngày điều trị 70% vết thương biểu mô hóa hoàn toàn, 90% hết rỉ dịch, sau 3 tuần tất cả vết thương hết đau, hết nổi mẫn hoàn toàn. Kết luận: Tia Plasma lạnh có vai trò tích cực trong hỗ trợ điều trị vết thương phần mềm đối với bệnh nhân COVID-19 mức độ nặng. ABSTRACT COLD PLASMA IRRADIATION AS AN ADJUVANT TREATMENT DIGITAL LESIONS FOR PATIENTS WITH SEVERE COVID-19: INITIAL EVALUATION Background: Plasma is a medium containing substances that no longer retain their molecular structure but are ionized. Cold plasma is widely used in medicine, with safety and effectiveness confirmed in many studies. This study was conducted at the Intensive Care Center for COVID-19 patients of Hue Central Hospital in Ho Chi Minh City to evaluate the general and clinical characteristics in severe COVID-19 patients with digital lesions, to evaluate the effect of cold plasma in the adjuvant treatment of soft tissue wound healing in patients with severe COVID-19 disease initially. Methods: This cross-section descriptive study was conducted on severe COVID-19 patients who undergo digital lesions treated at the COVID-19 Intensive Care Center operated by Hue Central Hospital in Ho Chi Minh, Vietnam, a quick 45 - day survey. Results: In 915 patients with severe COVID-19, 20 cases of digital lesions were experienced with cold plasma irradiation. The male - to - female ratio was 1: 1, 70% of wounds caused by pressure ulcers. 60% of lesions were located on the dorsum, gluteal and sacral region. 55.0% of lesions were discovered before transfer to our center. After 14 days of treatment, 70% of the wounds were completely epithelialized, 90% had no fluid oozing. After three weeks, all the lesions were pain-free, and the redness was completely terminated. Conclusion: Cold plasma Irradiation effectively supports the treatment of digital ulcers in patients with severe COVID-19 disease. Nhận xét về hiệu quả của Tocilizumab trong điều trị hỗ trợ bệnh nhân Covid-19

TÓM TẮT Đặt vấn đề: Sự bùng phát Coronavirus 2019 (COVID-19) đã lây lan nhanh chóng trên toàn thế giới và trở thành một đại dịch toàn cầu, nhiều phương pháp điều trị đang được nghiên cứu, trong đó ức chế interleukin 6 là một phương pháp tỏ ra hiệu quả. Báo cáo này nhằm đánh giá hiệu quả của Tocilizumab trong điều trị hỗ trợ bệnh nhân Covid-19. Báo cáo trường hợp: 4 trường hợp bệnh nhân được xác nhận Covid-19 bằng phương pháp PCR, đang được điều trị Trung tâm Hồi sức tích cực người bệnh Covid-19 thuốc bệnh viện Trung Ương Huế tại thành phố Hồ Chí Minh, viêm phổi nặng cần phải thông khí hỗ trợ. Bệnh nhân được điều trị phác đồ bao gồm tocilizumab cho kết quả tăng interleukin 6 sau 1 tuần điều trị. Kết luận: Trong thử nghiệm lâm sàng liên quan đến bệnh nhân viêm phổi nặng do Covid-19, cho thấy Interleukin - 6 tăng sau 1 tuần điều trị. ABSTRACT COMMENTS ON THE EFFECTIVENESS OF TOCILIZUMAB IN THE SUPPORTIVE TREATMENT OF COVID-19 PATIENTS Introduction: The outbreak of Coronavirus 2019 (COVID-19) has spread rapidly across the globe and has become a global pandemic. Many treatments are being studied, of which interleukin 6 inhibition is an effective method. Case report: Fourcases of patients confirmed covid-19 by PCR method, being treated at COVID-19 Intensive Care Center of Hue Central Hospital in Ho Chi Minh city, severe pneumonia requires ventilation assistance. The patients were treated with a regimen including Tocilizumab that resulted in increased interleukin 6 results after 1 week of treatment. Conclusion: In a clinical trial involving patients with severe pneumonia caused by Covid-19, it was found that Interleukin 6 increased after 1 week of treatment. Bước đầu khảo sát tình hình bệnh nhân nhiễm Covid-19 nặng có tổn thương phần mềm tại Trung tâm Hồi sức Tích cực Người bệnh Covid-19 Bệnh viện Trung ương Huế tại Thành phố Hồ Chí Minh

TÓM TẮT Đặt vấn đề: Tổn thương phần mềm là loại tổn thương da, tổ chức dưới da, cân, cơ... do nhiều nguyên nhân khác nhau như loét tỳ đè, bỏng, viêm da cơ địa, viêm da tiếp xúc, Zona, viêm da bọng nước,… xảy ra trên bệnh nhân nhiễm Covid-19 nặng, các tổn thương này có thể có từ trước hoặc sau khi bị nhiễm COVID-19. Chúng tôi thực hiện đề tài này nằm mục tiêu (1) nhận định đặc điểm chung của bệnh nhân nhiễm COVID-19 nặng có tổn thương phần mềm, (2) bước đầu nhận định tình trạng có tổn thương phần mềm xảy ra ở bệnh nhân nhiễm Covid-19 nặng và (3) tìm hiểu mối tương quan giữa quá trình chăm sóc vết thương và thời gian lành vết thương của bệnh nhân. Đối tượng và phương pháp: 25 bệnh nhân có tổn thương phần mềm trên tổng số 1.094 bệnh nhân nhiễm COVID-19 nặng, trung tâm hồi sức tích cực người bệnh COVID-19 bệnh viện Trung ương Huế tại thành phố Hồ Chí Minh từ 15.09 - 01.11 năm 2021 (khảo sát nhanh trong 45 ngày). Kết quả: Khảo sát nhanh trong 45 ngày trên 1.094 bệnh nhân bị COVID-19 nặng đang điều trị tại trung tâm hồi sức tích cực Người bệnh COVID-19 bệnh viện Trung ương Huế tại thành phố Hồ Chí Minh có 25 trường hợp bị tổn thương phần mềm: Hầu hết những bênh nhân này đều có bệnh lý nền nặng kèm theo, nam gặp ít hơn nữ (44%/56%), nam ở lứa tuổi 30 - < 50 tuổi chiếm cao nhất, nữ từ 50 tuổi trở lên là chủ yếu chiếm 92,86%. Đa số bệnh mắc phải lớn tuổi, trên 50 tuổi. Loét tỳ đè chiếm đa số 76% (19/25 trường hợp) so với các thương tổn khác, trong khi đó bệnh nhân bị tổn thương trong quá trình điều trị tại trung tâm chiếm 36,8% (7/19 trường hợp) còn phát hiện trước lúc vào viện là 63,2% (12/19 trường hợp). Tỷ lệ thương tổn phần mềm trên tổng số bệnh nhân điều trị tại thời điểm khảo sát chiếm 2,29% (25/1.094 bệnh nhân), do LDTĐ chiếm 0,64% bị tại trung tâm (7/1.094 bệnh nhân). Điều trị nội khoa chiếm đa số 76% nhiều hơn so với phẫu thuật, tỷ lệ phẫu thuật chiếm 24% (6/25 trường hợp). Thời gian điều trị liền vết thương trước 14 ngày chiếm đa số 56%. Kết luận: Quá trình chăm sóc và điều trị bệnh có mối tương quan với quá trình lành vết thương và dự phòng các thương tổn phần mềm. Chiếu tia Plasma lạnh góp phần trong quá trình liền vết thương. Chăm sóc và điều trị các tổn thương phần mềm có hiệu quả, tái tạo biểu bì mô nhanh chóng, tỷ lệ thương tổn thấp, góp phần giúp bệnh nhân sớm hồi phục, giảm tỷ lệ tử vong. ABSTRACT SURVEY OF COVID-19 PATIENTS HAVINGSOFT TISSUE DAMAGE AT THE COVID-19 INTENSIVE CARE CENTER OF HUE CENTRAL HOSPITAL IN HO CHI MINH CITY Backgrounds: Soft tissue damage is damage to the skin, subcutaneous tissue, scales, muscles caused by various causes such as pressure ulcers, burns, atopic dermatitis, contact dermatitis, shingles, bullous dermatitis occurring in patients with severe COVID-19 infection. These lesions may be present before or after a COVID-19 infection. This study aims to (1) identify the general characteristics of severe COVID-19 patients with soft tissue damage, (2) assess the status of soft tissue damage in patients with severe COVID-19 infection, and (3) find out the correlation between the wound care process and the healing time of the wound. Methods: Twenty - five patients have soft tissue lesions in a total of 1.094 patients with severe COVID-19 infection at COVID-19 Intensive Care Center operated by Hue Central Hospital in Ho Chi Minh city from September 15 to November 1, 2021 (quick survey in 45 days). Results: A quick 45 - day survey of 1.094 severe COVID-19 patients showed that there were 25 cases with soft tissue damage: most of these patients have severe underlying medical conditions, men less than women (44%/56%, men aged 30 - < 50 years old account for the highest number, women aged 50 years and older accounted for the majority of cases ( 92.86%) Most of the patients are over 50 years old. Pressure ulcers (PU) accounted for the majority of cases at 76% (19/25 cases) compared with other lesions, in which patients injured during treatment at the center accounted for 36.8% (7/19 cases). Meanwhile, PU detected before hospital admission was 63.2% (12/19 cases). The rate of soft tissue lesions on the total number of patients treated at the survey time accounted for 2.29% (25/1.094 patients), due to pressure ulcers accounting for 0.64% at the center (7/1.094 patients). Internal Medicine treatment accounted for most cases (76%), more than surgery treatment, 24% (6/25 cases). The time of wound healing before 14 days accounted for most cases at 56%. Conclusion: The care and treatment process correlates to the healing process and the prevention of soft tissue injuries. Adequate care and treatment of soft tissue injuries help to quickly heal epidermal tissue, reduce injury rates, lead to early recovery and low mortality. Đánh giá sự hài lòng của người bệnh nội trú tại Trung tâm Hồi sức Tích cực Người bệnh nhiễm Covid-19 trực thuộc Bệnh viện Trung ương Huế tại Thành phố Hồ Chí Minh

TÓM TẮT Mục tiêu: Đánh giá chỉ số hài lòng của người bệnh đang điều trị tại trung tâm hồi sức tích cực người bệnh nhiễm Covid-19 trực thuộc bệnh viện Trung Ương Huế tại thành phố Hồ Chí Minh. Phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang trên 325 người bệnh nhiễm Covid-19 đang điều trị tại trung tâm hồi sức tích cực người bệnh nhiễm Covid-19 từ tháng 9/2021 đến tháng 11/2021. Kết quả: Có 176 bệnh nhân nữ (54,15%) và 149 nam (45,85%). Độ tuổi trung bình 50,28 tuổi, trình độ học vấn cao đẳng chiếm tỉ lệ cao nhất 115 người bệnh chiếm 35,38%, không có đối tượng có trình độ học vấn cấp 1, đối tượng nghiên cứu đã tiêm 1 và 2 mũi Vaccine Covid-19 chiếm tỉ lệ cao lần lượt là 130 chiếm 40,00% và 107 chiếm 32,92%, đối tượng nghiên cứu già mất sức lao động chiếm tỉ lệ cao nhất 102 người bệnh chiếm 31,38%. Tỉ lệ hài lòng toàn diện của người bệnh về khả năng tiếp cận các thông tin trong khu điều trị mức 4 là: 9,14%, mức 5 là 89,41%, về sự minh bạch thông tin và thủ tục khám bệnh, điều trị mức 4 là 12,72%, mức 5 là 82,86%, về cơ sở vật chất và phương tiện phục vụ người bệnh mức 4 là 14,10%, mức 5 là 83,75%, về thái độ ứng xử, năng lực chuyên môn của nhân viên y tế ở mức 4 là 12,20%, mức 5 là 86,58%, về kết quả cung cấp dịch vụ ở mức 4 là 15,23%, mức 5 là 84,77%. Kết luận: Hầu hết bệnh nhân rất hài lòng với chất lượng và thái độ điều trị tại trung tâm hồi sức tích cực người bệnh nhiễm Covid-19 trực thuộc bệnh viện Trung Ương Huế tại thành phố Hồ Chí Minh ABSTRACT ASSESSMENT SATISFACTION OF IN - PATIENTS AT THE COVID-19 INTENSIVE CARE CENTER OF HUE CENTRAL HOSPITAL IN HO CHI MINH CITY Objective: To survey the satisfaction index of in - patients at the Covid-19 intensive care center of Hue Central Hospital in Ho Chi Minh city. Methods: A cross - sectional descriptive study was carried on 325 Covid-19 patients treated at the Intensive Care Center from September 2021 to November 2021. Results: There were 176 female patients (54.15%) and 149 male patients (45.85%). The average age was 50.28 years old; the higher education was in 115 patients (35.38%). Patients who receiveddose 1 and 2 of Covid-19 vaccine were in 130 patients, accounted for 40,00%, 107 patients accounted for 32.92% respectively. The elderly patients who lost their ability to work were in 102 patients accounted for 31.38%. The overall satisfaction rate of patients about the ability to access information in level 4 was 9.14%, level 5 was 89.41%, the transparency of information and medical examination procedures in level 4 was 12.72%, level 5 was 82.86%, the material facilities to serve patients at level 4 was 14.10%, level 5 was 83.75%, the behavior attitude in the treatment, professional qualificationof medical staff’s at level 4 was 12.20%, level 5 was 86.58%, the service responseresults at level 4 was 15.23%, level 5 was 84.77 %. Conclusion: All patients at the Covid-19 intensive care centerof Hue Central Hospital in Ho Chi Minh city highly satisfied with the treatment and taking care of the medical staff as well as the facilities of the center. Effect of Intrauterine Administration of Seminal Plasma for Patients with Recurrent Implantation Failure Before FET: Study Protocol for a Prospective Randomised Controlled Trial

Background: To assess the effect of intrauterine administration of seminal plasma for patients with recurrent implantation failure before frozen-thawed embryo transfer. Methods: Trial design: This is a parallel group, randomized (1:1 allocation ratio) controlled trial.Participants: All patients will be recruited from Chengdu Women’s and Children’s Central Hospital. Inclusion criteria: 1. Women after IVF/ICSI treatment in Chengdu Women’s and Children’s Central Hospital. 2.Infertile women with a history of recurrent implantation failure. 3.Infertile couples (both male and female) aged between 20 and 39 years;4. Couples who have at least 1 good quality embryos for transfer. 5. Males had negative in infectious disease test. 6. The males have semen. 7. Women who intend to undergo FET after IVF or ICSI or pre-implantation genetic testing for aneuploidy (PGT-A). 8. Competent and able to give informed consent. Intervention and comparator: Treatment group receiving intrauterine administration of seminal plasma before frozen-thawed embryo transfer. Main outcomes: Clinical pregnancy after frozen-thawed embryo transfer. Randomisation: Patients will be randomly allocated to either treatment or control group at 1:1 ratio. Random numbers will be generated by using software SPSS 25.0 performed by an independent statistician from Chengdu Women’s and Children’s Central Hospital. Blinding (masking): Only the data analyst will be blinded to group assignment. Numbers to be randomised (sample size): To account for a 10% dropout rate, we will recruit 174 patients (87 in each group). Trial status: The date of approval is 31rd May 2021, version 2.0. Recruitment started on 1rd June and is expected to end in July 2022. Discussion: Intrauterine administration of seminal plasma before frozen-thawed embryo transfer of patients with recurrent implantation failure may improve clinical pregnancy rate, it has great Page 2 of 14 significance for assisted reproduction. Trial registration: The study protocol has been approved by the ethics committees at Chengdu Women’s and Children’s Central Hospital. The trial was registered at the Chinese Clinical Trial Registry ChiCTR2100046803. Registered on 28 May 2021.

Vulvar Diseases that Required a Biopsy: A Retrospective Study

Introduction: The vulvar area may be affected by many noninfectious conditions with similar clinical appearance, requiring a cutaneous biopsy. Our goal was to characterize the noninfectious vulvar diseases that required a biopsy in a southwestern Europe Central Hospital during a 10-year period. Methods: A retrospective study of all the noninfectious vulvar diseases with histological confirmation diagnosed in our institution was conducted between January 1, 2008 and December 31, 2017. Results: The sample included a total of 323 biopsies from 317 patients, aged between 11 and 98 years (mean age of 54.2 years). A total of 36 vulvar diseases was identified. Neoplastic conditions were the most frequently found, particularly melanotic macules (22.3%). The most frequent malignant tumor was vulvar intraepithelial neoplasia (6.2%) and squamous cell carcinoma (5.6%). The most common dermatosis was lichen sclerosus (12.7%). Conclusion: Neoplasms were the most frequently diagnosed conditions affecting the vulvar area that required a biopsy. Ruling out malignancy was also the main reason to perform a biopsy. This study highlights the variety of noninfectious diseases that may affect the vulva and require a biopsy. Since vulvar diseases may be serious and carry high levels of patient distress a correct understanding of these conditions is crucial.
