AbstractBias in clinical practice, in particular in relation to race and gender, is a persistent cause of healthcare disparities. We investigated the potential of a peer-network approach to reduce bias in medical treatment decisions within an experimental setting. We created “egalitarian” information exchange networks among practicing clinicians who provided recommendations for the clinical management of patient scenarios, presented via standardized patient videos of actors portraying patients with cardiac chest pain. The videos, which were standardized for relevant clinical factors, presented either a white male actor or Black female actor of similar age, wearing the same attire and in the same clinical setting, portraying a patient with clinically significant chest pain symptoms. We found significant disparities in the treatment recommendations given to the white male patient-actor and Black female patient-actor, which when translated into real clinical scenarios would result in the Black female patient being significantly more likely to receive unsafe undertreatment, rather than the guideline-recommended treatment. In the experimental control group, clinicians who were asked to independently reflect on the standardized patient videos did not show any significant reduction in bias. However, clinicians who exchanged real-time information in structured peer networks significantly improved their clinical accuracy and showed no bias in their final recommendations. The findings indicate that clinician network interventions might be used in healthcare settings to reduce significant disparities in patient treatment.

This article presents the case of a female patient suffering from cervical myelopathy due to ossification of the posterior longitudinal ligament (OPLL) which was undiagnosed for a number of years. Besides presenting the clinical case, we describe the pathophysiology, symptoms, diagnostic methods and treatment options related to this condition, which is unusual in the European population.

This article describes a case of Caroli’s disease in a 53-year-old female patient who complained nonspecificabdominal pain without cholestasis or cholangitis. Ultrasound and hepatic magnetic resonanceimaging showed segmental saccular dilations connected to intrahepatic bile ducts without hepaticfibrosis. This clinical case shows the possibility of having oligosymptomatic forms in the diffuse formsof Caroli disease, therefore the interest to consider this diagnosis in case of non-specific abdominalsigns and to request a hepatic ultrasound. Self-monitoring, a competing response and response cost were used in the successful treatment of a 49 year old female patient with a 19 year history of trichotillomania. Self-monitoring and a competing response produced significant decreases in hair-pulling, but still continued as the patient found it pleasurable at times of stress. The addition of response cost procedures overcame this.

Purpose: Demonstrate a clinical case of poorly differentiated chordoma, confirmed using a wide range of research methods.Material and methods: A 63-year-old female patient with poorly differentiated chordoma who underwent immunohistochemical examination, MRI, CT and scintigraphy.Results: An immunohistochemical study confirmed the morphological affiliation of the tumor, supplemented by the data of imaging methods.Conclusion: The poorly differentiated type of chordoma has a specific immunohistochemical picture, however, differential diagnosis based on imaging methods is currently a difficult task.
