ABSTRACT Introduction CA 19-9 is used as a tumor marker in colon, pancreas, biliary, and gastric cancers. Laryngeal cancer is the most common malignant epithelial tumor among head and neck cancers and has no specific tumor marker. Case report A 66-year-old male patient had severe reflux symptoms during 5 years and had an isolated CA 19-9 elevation. Follow-up analysis revealed that he had larynx cancer and after laryngectomy, CA 19-9 levels decreased to normal range. Discussion Currently, CA 19-9 is not a marker for malignancy. Laryngeal carcinoma has no specific tumor marker, but laryngeal squamous cell carcinoma may be manifested by elevated CA 19-9 levels. Primary tumours of mesentery are relatively rare, and its biological behaviour is unpredictable. We herein describing a case of incidentally detected mesenteric leiomyoma along with review of literature in a 19-year-old gentleman, who had a mass in the mesentery of terminal ileum, which was detected during surgery. Mesentric masses can be both solid as well as cystic. Published reports have consisted of small numbers of cases, which makes it difficult to determine the incidence of specific tumor types. Reasonable estimates of incidence ranges from 1 case per 200,000 population and in 30-40% cases they are cystic and more in females. Diagnosis is delayed unless they become symptomatic with haemorrhage, obstruction or mass per abdomen. As primary mesenteric tumors are rare, particularly in young patients, it is considered important that this type of unusual tumor be included in the differential diagnosis for mesenteric tumors. Since the biologic behaviour of mesenteric smooth muscle tumours seems to be unpredictable long term follow up is warranted.

ABSTRACT Spindle epithelial tumor with thymus-like different ion (SETTLE) is an extremely rare tumor arising from thyroid gland which was first time recognized and reported by Chan and Rosai. It is believed to be arising from the branchial pouch or the remnant of thymus within the thyroid gland as it reveals primitive thymic elements. It has been reported more than 20 times as an individual case reports in the searched English literature. It mostly occurs in the children and adolescent but cases of SETTLE are reported in adults and even in elderly patients. Age of patient is not diagnostic feature of the SETTLE tumor. According to the published data, SETTLE tumor has an indolent course but delayed blood-borne distant metastases is seen in the cases with long-term follow-up. The present case is 13-year-old boy who presented with solitary nodule of left lobe of the thyroid without any other symptoms. The patient was subjected to ipsilateral hemithyroidectomy. The histopathological report revealed the diagnosis of SETTLE tumor which was confirmed with the immunohistochemistry. The patient is followed-up regularly and there is no evidence of metastasis with more than 4 years follow-up. There are no existing guidelines regarding the optimum treatment and the duration of follow-up due to paucity of data about management of SETTLE tumor. We report our experience of long-term follow-up of all patients treated during a ten-year period with primary radiotherapy irrespective of the disease stage of their laryngeal carcinoma. This policy was adopted in order to allow all patients the opportunity to save their larynx and to preserve their voice. In the present literature, surgery is favoured for treating advanced disease.

AbstractNeurobiology-based interventions for mental diseases and searches for useful biomarkers of treatment response have largely failed. Clinical trials should assess interventions related to environmental and social stressors, with long-term follow-up; social rather than biological endpoints; personalized outcomes; and suitable cluster, adaptive, and n-of-1 designs. Labor, education, financial, and other social/political decisions should be evaluated for their impacts on mental disease.
