ABSTRACT Macrodontia is a rare dental anomaly which consists on the presence of any tooth or teeth larger than the average. In the present report the case of a child with an uncommon true isolated macrodontia of a maxillary central incisor is presented. The maxillary left central incisor was considered a macrodontic tooth since its mesiodistal crown dimension was 12.41 mm while the mesiodistal crown dimension of the maxillary right central incisor was 10.06 mm. The radiographic examination confirmed an enlarged maxillary left central incisor and a normal developing dentition. The patient did not report esthetic complaints, however, due the presence of anterior open bite and crowding, he was referred to orthodontic clinic. Thus, it must be point out the importance of the radiographic diagnosis to support the clinical findings, helping to plan and provide better treatment for the patient. The present report follows the case of a young boy with solitary median maxillary central incisor (SMMCI) syndrome between the ages of 4 and 7 years. This condition is characterized by the presence of one single maxillary central incisor in the midline instead of two central incisors. No other developmental abnormalities involving growth or brain function were noted at, or subsequent, to birth. This report includes a discussion of the aetiology of SMMCI syndrome and its association with birth defects such as holoprosencephaly (HPE), CHARGE and VACTERL, as well as a discussion of the long-term prognosis and associated dental and medical issues for this particular patient

ABSTRACT Amelogenesis imperfecta is a rare hereditary disorder that affects dental enamel and is often associated with an anterior open bite. Orthodontic treatment of a 16-year-old female patient with hypocalcified amelogenesis imperfecta and a 9-mm anterior open bite was presented. Radiographic examination revealed a steep mandibular plane angle, an increased lower face height, a Class II skeletal pattern, and a convex profile. Additionally, the patient had stainless steel crowns on all upper and lower posterior teeth and composite veneers on the upper anterior teeth. The patient was treated nonsurgically using a multiloop edgewise archwire (MEAW). MEAW mechanics allowed for successful correction of the anterior open bite, with significant reduction in the mandibular plane angle and improvement in the patient's profile. No fixed retainers were used, results remained stable 78 months after removal of orthodontic appliances. MEAW mechanics should be considered for patients with large anterior open bites, although this technique requires excellent patient compliance.

Impaction of maxillary central incisors is frequent in dental practice. However its management is challenging because of its importance in aesthetics. Supernumerary teeth are the most common dental anomaly that can cause impaction of adjacent teeth, crowding, diastema, rotation and displacement of teeth. We report a case of 13 year old male with an impacted supernumerary tooth in the maxillary anterior region, which was interfering with the eruption of the permanent, left central incisor. The impacted supernumerary tooth was surgically removed. With the application of an orthodontic traction, impacted left maxillary central incisor was brought down to its proper position in the dental arch.

Inflammatory external root resorption is one of the major complications after traumatic dental injury. In this case report, we describe treatment of a maxillary central incisor affected by severe, perforating external root resorption. An 18-year-old patient presented with a previously traumatized, root-filled maxillary central incisor associated with pain and sinus tract. Radiographic examination revealed periradicular lesion involving pathologic resorption of the apical region of the root and lateral root surface both mesially and distally. After removal of the root canal filling, the tooth was disinfected with intracanal triple antibiotic paste for 2 weeks. The antibiotic dressing was then removed, and the entire root canal was filled with mineral trioxide aggregate. The endodontic access cavity was restored with composite resin. After 18 months, significant osseous healing of the periradicular region and lateral periodontium had occurred with arrest of external root resorption, and no clinical symptoms were apparent.

This is the first reported case of siblings with Solitary Median Maxillary Central Incisor (SMMCI) syndrome presenting with an erupted single symmetrical central maxillary midline incisor between normal central incisors. Usually only one central incisor is present. This case is also interesting as the tooth described could have been mistaken for a mesiodens, however, a mesiodens is not symmetrical and usually erupts between the maxillary central incisors and sits slightly to the right or left of the midline. CPD/Clinical Relevance: This case highlights the fact that SMMCI should not be considered as a simple dental anomaly because it may be associated with more complex craniofacial malformations.

When dental implants are malpositioned in relation to the adjacent teeth and alveolar bone or in an excessive buccal or lingual position, the final prosthesis rehabilitation impairs the peri-implant health of the gingival tissues and the aesthetics of the patient. Thus, the purpose of this case was to report and discuss a multidisciplinary protocol for the treatment of a compromised maxillary tooth in a patient with an abscess in his right central incisor due to an excessive buccal implant position. The patient presented with an implant-supported provisional restoration on his right maxillary central incisor and a traumatic injury in his left central incisor. The treatment protocol consisted in (i) abutment substitution to compensate the incorrect angulation of the implant, (ii) clinical crown lengthening, (iii) atraumatic extraction of the left central incisor, and (iv) immediate implant placement. Finally, (v) a custom abutment was fabricated to obtain a harmonious gingival contour around the prosthetic crown. In conclusion, when implants are incorrectly positioned in relation to the adjacent teeth, associated with soft-tissue defects, the challenge to create a harmonious mucogingival contours may be achieved with an interdisciplinary approach and with the placement of an appropriate custom abutment.
