ABSTRACT Cervical pregnancy is a pregnancy where implantation occurs in the cervix of uterus rather than occurring in the uterine cavity. It is a rare life-threatening condition so early diagnosis and treatment can save life. AbstractBackground:We sought to characterize a cohort of participants with swallow-induced syncope (SIS) with clinical and electrophysiologic evaluations.Methods:Using electrocardiographic monitoring and neurophysiologic methods of swallowing, we evaluated a cohort of 5 patients with SIS, 4 of whom had longitudinal follow-up.Results:We determined electrophysiologically that the duration between the onset of swallow and a bradyarrhythmia or asystole is extremely short (2–3 seconds) in SIS. Most participants with SIS do not have a neurologic or esophageal disorder. SIS can occur with different food types, in sitting or standing position, and has varying frequency in different participants. Permanent pacemaker placement is a curative measure in SIS.Conclusions:Our findings suggest that SIS is elicited by reflex afferent pathways originating in the oropharynx, rather than an esophageal origin, as previously proposed. Our longitudinally followed cohort with detailed clinical and electrophysiologic characterization should aid the clinician in the diagnosis and treatment of this potentially life-threatening condition.

Ureteroarterial fistula (UAF) is a relatively rare condition with about 150 cases reported in the literature. Since it is a potentially life-threatening condition, a prompt diagnosis and treatment are crucial. We present here a rare, challenging case of UAF diagnosed after left nephrectomy, thus involving the ureteral stump. The difficult diagnosis and treatment by contemporary use of endovascular stent placement and ureteral occlusion by mean of metallic coils and Onyx injection are discussed.

Acute corticosteroid-induced rhabdomyolysis is a rare, but potentially life-threatening, condition that deserves the attention of medical professionals and sport scientists. Early diagnosis is vital in minimising the secondary damage caused by rhabdomyolysis. This case of rhabdomyolysis highlights the severity of symptoms and the importance of decisive treatment. Clinicians should be familiar with the most common symptoms of acute corticosteroid-induced rhabdomyolysis to enable early diagnosis and efficient management of this condition.

Hirschsprung-associated enterocolitis (HAEC) is a serious and life-threatening condition, and atypical tuberculosis (TB) associated with HAEC is even more serious. A male newborn aged 4 days was diagnosed with Hirschsprung disease and transanal Soave pull-through was performed at 4 months old. Six months later, he suffered from enterocolitis. Although he was treated with multiple broad-spectrum antibiotics for 2 weeks, he developed a fever without any other symptoms for TB infection. We found numerous, bilateral, uniformly distributed, small pulmonary nodules in the lower lobes in an abdominal radiograph by chance. He was then discharged with complete resolution of all symptoms after anti-TB therapy. Early diagnosis and treatment of TB can effectively improve the prognosis of children with HAEC.

Traumatic diaphragmatic injury (TDI) is a fairly uncommon with incidence of 0.8 and 1.6 %, commonest fallowing blunt trauma abdomen. Right-side have higher mortality rate compared to left side and penetrating injuries. It present as occult to obvious. Laparoscopy offers diagnostic and therapeutic tool of care. Lack of awareness of the condition may delay in diagnosis, results life threatening complications. Missed hernia is a known complication of blunt trauma as acute diagnosis can be difficult to ascertain. An early diagnosis and treatment lead to better outcome. In present study CECT revealed acute diaphragmatic hernia, bilateral pleural effusion and hair line fracture of right tibia on skiagram limb. Because of uncertainty in diagnosis of acute diaphragmatic hernia or rupture surgeons faces challenges for the management, high suspicion index required to diagnose the cases. An early diagnosis and treatment lead to better outcome

Pyometra is a condition characterized by the accumulation of pus within the uterine cavity that usually develops in elderly women . Spontaneous rupture of pyometra causing diffuse peritonitis is very rare.Unless recognized in time it can be a life threatening condition. The aim of this article is to report a case of spontaneous rupture of pyometra who was admitted in our hospital as surgical emergency and was successfully treated by surgery. Cervical ectopic pregnancy is the implantation of a pregnancy in the endocervix1. Such pregnancy typically aborts within the first trimester, if it is implanted closer to the uterine cavity called cervico isthmic pregnancy it may continue longer2. Cervical pregnancy accounts for less than 1% of all ectopic pregnancies, with an estimated incidence of one in 2500 to one in 180003-5. Though the pregnancy in this area is uncommon but possibly life threatening condition due to risk of severe hemorrhage and may need hysterectomy. Early detection and conservative approach of treatment limit the morbidity and preserve fertility. A 26 years lady diagnosed as a case of cervical ectopic pregnancy and managed conservatively successfully with adjunctive techniques like cervical artery ligation and cervical temponade to control haemorrhage. RESUMO A mola hidatiforme é uma forma benigna da doença trofoblástica gestacional pouco frequente na gravidez e com potencial para evoluir para formas que necessitam de tratamento sistêmico e podem ameaçar a vida. As apresentações com molas de grande volume, com eliminação de vesículas e anemia, são cada vez menos frequentes devido ao uso sistemático de ultrassonografia gestacional no primeiro trimestre de gestação que permite o diagnóstico precoce. Ainda assim, raramente podem ocorrer certas complicações, como hemorragia, hipertireoidismo, pré-eclâmpsia, insuficiência respiratória e cistos ovarianos, que exigem conduta imediata e encaminhamento das pacientes para centro de referências que realizem aspiração uterina, preferencialmente, pela técnica de vácuo-aspiração, a fim de se evitar possíveis intercorrências, dentre as quais a perfuração uterina. Deve ser realizado seguimento rigoroso, sistemático e pontual para diagnóstico precoce de NTG e preservação da fertilidade das mulheres acometidas. Enfatizar os métodos de diagnóstico e tratamento das principais comorbidades associadas a mola hidatiforme é o objetivo deste estudo, que foi realizado através de uma revisão de literatura , utilizando artigos dos últimos 15 anos, nacionais e internacionais arquivados nos bancos de dados MEDLINE, BIREME, PUBMED, SciELO e Livros periódicos analisados nas biblioteca do Instituto Presidente Antônio Carlos.   Palavras-chave: Comorbidades associadas a gravidez molar. ABSTRACT A hydatidiform mole is a benign form of infrequent gestational trophoblastic disease in pregnancy and it has the potential to evolve into forms that require systemic treatment and can be life-threatening. Presentations with large amounts of moles, with elimination of vesicles and anemia, are becoming less frequent due to the systematic use of gestational ultrasound in the first trimester of pregnancy that allows early diagnosis. Still, it could rarely occur certain complications, such as bleeding, hyperthyroidism, preeclampsia, respiratory failure and ovarian cysts, requiring immediate management and referral to the center of reference to perform a uterine aspiration, preferably by vacuum aspiration technique, in order to avoid possible complications, among them uterine perforation. It should be rigorous systematic and timely followed, for early diagnosis of NTG and preservation of affected women’s fertility. This paper aims to emphasize the methods of diagnosis and treatment of major comorbidities associated with hydatidiform mole. This course conclusion work was done through a literature review, using articles from the last 15 years, national and international, using the databases MEDLINE, BIREME, PUBMED, SciELO and periodicals Books analyzed in the library of the Instituto Presidente Antonio Carlos.
