Recently one patient died and another came near death because of severe metabollic acidosis following the administration of Furadantoin® Intravenous Solution. (The brochure accompanying the medication does not indicate that metabolic disturbances might result from its use.) Both patients were adults with severe and extensive paralysis due to poliomyelitis and marked respiratory paralysis as well. Both had renal calculi and urinary tract infections with bacteria which were resistant to all but the more toxic antibacterial agents.

The most prevalent microorganisms isolated from urine specimens of dogs (385) and cats (132) with clinical signs of urinary tract infections were Escherichia coli, Proteus spp., and Staphylococcus aureus. The results of quantitative urine-culturing methods showed 48.6% of the canine and 12.1% of the feline specimens had more than 10(5) organisms per ml of urine. The bacteria isolated appear to have a greater resistance to antibacterial agents than previously reported.

Seeds extract of three Artocarpus species: Their in-vitro antibacterial activities against multidrug-resistant (MDR) Escherichia coli isolates from urinary tract infections (UTIs). Biodiversitas 22: 4362-4368. Multidrug-resistant (MDR)-E. coli is a major cause and has become a very serious problem in urinary tract infections (UTIs). As a result, it requires an antibacterial agent derived from biological materials. It has been reported that the seeds of three species of Artocarpus (A. heterophyllous, A. champeden, and A. camansi) have antibacterial properties against Methicillin-Resistant Staphylococcus aureus (MRSA). However, there are three other Artocarpus species in Indonesia, i.e., keledang (A. lanceipolius), tarra (A. elasticus), and terap (A. Odoratissimus) whose antibacterial property has not been investigated. To minimize the research gap, this study aims to determine the antibacterial activity of seed extracts of A. lanceipolius, A. elasticus, and A. odoratissimus against MDR-E. coli isolates of UTIs. Antibacterial activity was evaluated using the agar well diffusion assay. The microdilution method was used to determine the minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) values. The results revealed that the seed extracts of A. lanceipolius, A. elasticus, and A. odoratissimus have the potential as antibacterial agents against MDR-E. coli isolate of UTIs. A. elasticus seed extract shows the widest zone of inhibition in the range of 7.0-13.3 mm and the smallest MIC and MBC values ??of 6.25-12.5 mg/mL and 12.5-25 mg/mL, respectively. In conclusion, A. lanceipolius, A. elasticus, and A. odoratissimus seed extracts have the potential to be developed as antibacterial agents against UTI-causing MDR-E. coli. Further in vivo research and determining the mode of action of antibacterial activity are needed.

Abstract World-wide, urinary tract infections (UTIs) are an important clinical problem. In such, the most frequently isolated uropathogen is Escherichia coli. In the treatment of uncomplicated UTIs, e.g. cystitis, the widely used antibiotics are nitrofurantoin, trimethoprim/sulfamethoxazole, fosfomycin trometamol or ciprofloxacin, while the treatment of pyelonephritis requires the usage of antibiotics with a broader spectrum of activity, such as cephalosporins of the 3rd and 4th generation, aminoglycosides or even carbapenems. The aim of this study was to assess the susceptibility to aminoglycosides (such as amikacin, gentamicin, netilmicin and tobramycin) of E. coli isolated from UTIs in adult community patients living in Lubelszczyzna. We found that all of the 86 strains of E. coli encountered were susceptible to amikacin. Moreover, the prevalence of susceptibility to tobramycin, gentamicin or netilmicin among the tested strains was found to be 89,5%, 90,7% or 94,2%, respectively. The data obtained in the present study shows the high susceptibility to aminoglycosides of E. coli isolated from the community-acquired UTIS in adults. These data, together with that derived from current literature, indicate that aminoglycosides, when employed in combination therapy with other antibiotics, may still be very useful group of antibacterial agents in the treatment of UTI’s in Poland.
