Objective: to evaluate the efficiency of monotherapy and fixed combination therapy of secondary glaucoma (SG) in patients with endocrine ophthalmopathy (EOP).Materials and methods. We examined 145 patients (290 eyes and orbits) with different forms and activity of EOP. Among them, 30 patients (58 eyes and orbits) were diagnosed with ″SG detected for the first time and associated with EOP″ (20 % of the cases).The patients with SG received beta-adrenoblockers (BA) or prostaglandin analogues (PGA) as initial monotherapy. Two weeks later, in the absence of intraocular pressure (IOP) compensation, fixed combinations of beta-blocker/carbonic or anhydrase inhibitor (BB/CAI) or beta-blocker/prostaglandin analogue (BB/ PGA) were prescribed.Results. The PGA monotherapy reduced the IOP level by 29 % compared to the baseline level in 33 % of the cases in increased ophthalmic tonus within Me 27 [26;28] mm Hg. (Wilcoxon test, p = 0.005). In IOP levels over 29 [28;31] mmHg, BB/PGA fixed combination therapy reduced IOP by 33 % from the baseline level.Conclusion. SG monotherapy in patients with EOP is effective in increased IOP within Me 27 [26;28] mmHg. In initial IOP levels higher than 29 [28;31] mmHg, it is expedient to prescribe BB/PGA fixed combination therapy as initial therapy.

Purpose. To evaluate the effect on intraocular pressure (IOP) of switching from latanoprost and travoprost monotherapy to timolol fixed combinations in Japanese patients with normal-tension glaucoma (NTG).Methods. 27 NTG patients (54 eyes) were compared IOP, superficial punctuate keratitis (SPK) scores, and conjunctival injection scores in eyes treated with prostaglandin (PG) or PG analog/beta-blocker (PG/b) fixed-combination 6 months after the change in therapy.Results. The mean baseline intraocular pressure was17.4±1.59 mmHg in eyes receiving PG therapy only and17.4±1.69 mmHg in eyes switched to PG/b. Switching to fixed combination therapy from PG monotherapy, the mean IOP was13.1±1.79 mmHg(P<0.001)  (-24.71% reduction from baseline) at 6 months. The mean conjunctival injection score was 0.69 for eyes on PG monotherapy and 0.56 for eyes on fixed combination therapy(P=0.028). The mean SPK scores were 0.46 and 0.53. This difference was not statistically significant(P=0.463).Conclusions. Switching from PG monotherapy to PG/b fixed combination therapy for NTG resulted in a greater intraocular pressure reduction than PG alone without increasing the number of instillations.

Modern treatment of T2DM requires a shift in paradigm with appropriate intensification of therapy from the very first time of diabetes diagnosis. This is supported by data showing how even a moderate delay in achieving good glycemic control can translate into a later increased risk of developing diabetic complications. The recognition of the complexity of the pathogenesis of T2DM leads to the appreciation of the importance of attacking the disease from different angles, i.e. simultaneous tackling of multiple mechanisms contributing to hyperglycemia. From the turn of century a growing number of new anti-hyperglycemic agents have been made available. As compared to the older ones, these new medicines have a more targeted mechanism of action as they act at the level of the specific pathophysiologic disturbances accounting the development and progression of hyperglycemia. Because of that drugs can be use in combination taking advantage of their complementary mechanisms of action and synergistic. If introduced earlier in the natural history of the disease combination therapy may contribute avoiding undesirable exposure to even mild chronic hyperglycemia and provide early benefits. With respect to that in this review we will discuss advantages, disadvantages and still unanswered questions related to the use of early combination therapy in type 2 diabetes.

Abstract: Vulvovaginalis candidiasis (VVC) is a disease of female reproductive organs with the site of infection at the vulva and vagina mucosa characterized by vaginal discharge and itching due to the uncontrolled growth of the fungus Candida albicans. This study aimed to obtain the profile of vulvovaginalis candidiasis cases at Prof. Dr. R. D. Kandou Hospiptal Manado from January to December 2013. This was a retrospective descriptive study using the medical record. The results showed that of 29 VVC cases (0.70%), the largest age groups were 15-24 and 25-44 years (41.4%). Housewives and students were the most found jobs (20.7%). The discharge accompanied by itching was found in 34.5% of cases. The use of douching and pregnancy were predisposing factors (13.8%). Gram examinations resulted in spores, budding cell, and pseudohyphae were found in 62.1% of cases. Patients who was infected by VVC for the first time were 82.8%. The most frequent type of therapy was combination therapy (48.3%). Keywords: vulvovaginalis candidiasis   Abstrak: Kandidiasis vulvovaginalis (KVV) merupakan suatu penyakit organ reproduksi pada wanita dimana terjadi infeksi pada mukosa vulva dan vagina ditandai dengan adanya keputihan dan gatal dikarenakan pertumbuhan tidak terkendali dari jamur Candida albicans. Penelitian ini bertujuan untuk mengetahui profil pasien kandidiasis vulvovaginalis di RSUP Prof. Dr. R. D. Kandou Manado periode Januari – Desember 2013. Penelitian ini bersifat deskriptif retrospektif dengan mengevaluasi catatan rekam medik pasien. Hasil penelitian menunjukkan terdapat 29 kasus KVV. Kelompok umur terbanyak 15-24 dan 25-44 tahun (41,4%); pekerjaan terbanyak ialah ibu rumah tangga dan pelajar (20,7%); keputihan disertai gatal (34,5%); faktor predisposisi terbanyak yaitu penggunaan douching dan kehamilan (13.8%). Pemeriksaan Gram ditemukan spora, buddingcell dan pseudohifa ditemukan pada 62,1% kasus. Pasien yang baru pertama kali terinfeksi KVV  sebanyak 82,8%. Jenis terapi terbanyak yang diberikan yaitu terapi kombinasi sebanyak 48,3%). Kata kunci: kandidiasis vulvovaginalis

Introduction: We report a case of patient who underwent bilateral eyeball tattoo procedure for cosmetic purpose complicated with intraocular dye injection which led to ocular inflammation, secondary glaucoma, and cataract. Case description: A 21-year-old female presented to emergency department with severe right eye pain and photophobia 21 days after conjunctival tattooing procedure. Slit-lamp examination revealed right-sided eyelid edema and black deposits of tattoo ink under the bulbar conjunctiva. Dye deposits were also present in anterior chamber, covering corneal endothelium, iris and anterior lens’ capsule. Ocular inflammation and secondary glaucoma were diagnosed. The patient underwent surgical treatment to control ocular inflammation. Intraocular pressure remained stable after Nd:YAG iridotomy and on topical drugs. Moreover, during the follow-up, in contralateral eye, we observed “conjunctival lumps”—local hypersensitivity reaction to the dye, which were asymptomatic to the patient. Conclusion: Eyeball tattooing complications are new challenges that ophthalmologist may have to face nowadays. We also reviewed for the first time possible complications of eyeball tattooing described in available literature.

Several fixed combination products are now available in glaucoma management. Considerations when selecting combination therapy include safety, finding the best combination of mechanisms of action. Combigan® is one such product that has been shown to be more effective than either of its constituent agents, brimonidine or timolol, used alone. It has at least a similar intraocular pressure (IOP)-lowering effect as the concomitant use of its two constituents, and it produces a significant additional lowering of IOP in patients who switch from other therapies, including Cosopt®. Combigan has high patient satisfaction ratings.
