Chronic congestive heart failure (CHF) is a complex disorder characterized by inability of the heart to keep up the demands on it, followed by the progressive pump failure and fluid accumulation. Although the loop diuretics are widely used in heart failure (HF) patients, both pharmacodynamic and pharmacokinetic alterations are thought to be responsible for diuretic resistance in these patients. Strategies to overcome diuretic resistance include sodium intake restriction, changes in diuretic dose and route of administration and sequential nephron diuretic therapy. In this review, we discuss the definition, prevalence, mechanism of development and management strategies of diuretic resistance in HF patients.

Fungal diseases have their greatest impact on citrus in Florida by reducing tree vigor, fruit yield, and quality. Given the complex etiology of these diseases, this software was developed to facilitate diagnosis of symptoms and to explain the dynamics of Alternaria brown spot of mandarins, greasy spot, melanose, Phytophthora brown rot, post-bloom fruit drop, and sour orange scab. CITPATH includes a diagnostic key to identify symptoms of the major fungal diseases of citrus foliage and fruit in Florida and a hypertext program containing a description and graphic display of symptoms, maps of geographic occurrence, diagrams of disease development, and management strategies. Users can also consult a list of citrus cultivars susceptible to specific diseases and a reciprocal list of diseases affecting specific cultivars. Chemical control methods are discussed briefly with reference to the current Florida Citrus Spray Guide, a hardcopy of which is included with the software purchase. Developed for commercial growers, county extension programs, citrus horticulture classes, and master gardeners, this software is available on CD-ROM disks containing other citrus databases and as a separate disk for MS-DOS-based computers.

PurposeInfrastructure development and management form the central part of the government's commitment and responsibility to deliver essential services to the communities. However, much focus has been placed on the development aspect, with very little focus on the management and maintenance aspects, causing service delivery problems. This study explores the causes of poor management of immovable municipal infrastructure in South Africa.Design/methodology/approachA qualitative approach was adopted; thus, a sample of 15 participants consisting of senior municipal workers, local organizations and forums in Ngqushwa Municipality in Eastern Cape were identified and interviewed. The interview data were analyzed using thematic content analysis to determine the common themes and the frequencies.FindingsThe study's findings indicate numerous causes of poor municipal infrastructure management, including lack of funding, lack of capacity, poor planning and oversight, lack of By-laws, grant dependency and corruption. The municipality's failure to address these issues has resulted in increased service protests, destruction of public property, interruption of services and loss of confidence in municipal administrations.Research limitations/implicationsAlthough the study concentrated on one municipality, the findings may be applicable to other South African municipalities.Practical implicationsTo provide efficient services for the inhabitants to curtail aggravated service delivery protests, there is an urgent need for the municipalities to institute effective measures to manage and maintain the infrastructure that serves the communities.Originality/valueThe study has identified the factors underpinning ineffective management of the municipalities' facilities and the resultant's effects. Thus, the findings will guide the government and the authorities on the infrastructural management strategies for effective service delivery.

Producers of low volume, long life products must utilize the latest commercial, off the shelf (COTS) components in order to meet cutting edge technological needs. These COTS components often have a primary use in the high volume commercial markets (e.g. smart phones) which are characterized by short product lifecycles to satisfy consumer needs and remain competitive. Consequently, the two to three year lifecycles of these products tend to heavily influence the lifecycle of the components inside. Most tactical military products (as an example of low volume, long life products) have a very long design, production and support period that can often exceed ten years. Given the lifecycle mismatch between the products and components, an obsolescence management process is essential in order for a producer of low volume, long life products to effectively manage obsolescence in their product line. In this paper, the obsolescence management strategies are reviewed to identify best practices and recommendations that can improve a company’s ability to deal with obsolescence, sometimes called DMSMS (Diminishing manufacturing sources and material shortages), though the terms are not strictly interchangeable. This is achieved not only through a comprehensive literature review but also through a series of case studies from different companies. These validate findings from literature and provide a realistic perspective on the challenge to manage obsolescence, during the product development lifecycle.
