Primary cutaneous CD4+ small/medium-sized pleomorphic T-cell lymphoma in the practice of a dermatologist: A case report

The article demonstrates a case of diagnosis of a rare variant of primary skin lymphoma, namely of primary cutaneous CD4+ small/medium-sized pleomorphic T-cell lymphoma with an indolent course. The diagnosis of SMPTCL is based on the combination of the clinical picture, histological and immunomorphological data. A 70-year-old patient presented at the clinic with complaints about tumor-like formations on the skin of the face and back accompanied by moderate itching. The diagnosis was verified by pathomorphological and immunohistochemical studies of the biopsy of the affected skin. The presented clinical case emphasizes the importance of doctors’ clinical oncological alertness and the need for clinical and laboratory examination of the patient using modern histological and immunohistochemical methods for studying the skin biopsy.

Диагностика первичной кожной CD4+ плеоморфной Т-клеточной лимфомы кожи из клеток малого и среднего размера сложна ввиду редкой встречаемости данной нозологии. Заболевание необходимо дифференцировать от первичной В-клеточной лимфомы кожи, анапластической крупноклеточной CD30+ лимфомы кожи, опухолевой стадии грибовидного микоза, псевдолимфомы. Показаны важность клинической онконастороженности врачей и необходимость проведения клинико-лабораторного обследования больного с использованием современных гистологических и иммуногистохимических методов исследования. В статье приведен случай диагностики редкого варианта первичной лимфомы кожи – первичной кожной CD4+ плеоморфной Т-клеточной лимфомы кожи из клеток малого и среднего размера у пациента 70 лет, обратившегося на консультацию к дерматовенерологу. Диагноз верифицирован патоморфологическими и иммуногистохимическими исследованиями биоптата пораженной кожи. The diagnosis of primary cutaneous CD4+ pleomorphic T-cell lymphoma of the skin from small and medium-sized cells is difficult due to the rare occurrence of this nosology. The disease must be differentiated from primary B-cell lymphoma of the skin, anaplastic large cell CD30+ skin lymphoma, tumor stage of fungal mycosis, pseudolymphoma. The importance of clinical oncological alertness of doctors and the need for clinical and laboratory examination of a patient using modern histological and immunohistochemical research methods are shown. The article presents a case of diagnostics of a rare variant of primary skin lymphoma of cellular skin lymphoma – primary cutaneous CD4+ small/medium sized pleomorphic T-cell lymphoma in a 70-year-old patient. The diagnosis was verified by pathomorphological and immunohistochemical studies of the biopsy of the affected skin.
