ABSTRACTTalon cusps and gemination are rare dental anomalies that can cause significant aesthetic and clinical problems. Bilateral talon cusps on geminated teeth have not been reported so far. The case of gemination with talon cusps on both maxillary central incisors was presented in this report. The main complaints of the patient were tongue irritation and aesthetic problems. The talon cusps were gradually reduced at 2 consecutive sittings and the exposed surface was treated with a fluoride gel. The aesthetical appearance was improved using a composite resin. Fixed orthodontic treatment was initiated for repositioning the left maxillary lateral incisor. A long term, multidisciplinary approach is necessary for the treatment of gemination with talon cusps. ABSTRACT Aim The purpose of this case report is to present the orthodontic, surgical and restorative treatments in the case of an operated cleft lip and palate and severe maxillary deficiency in a 14-year-old female patient. Background Only orthodontic treatment could be inefficient for cleft lip and palate patients characterized with maxillary hypoplasia. Orthodontic and surgical treatment shows sufficient results, especially with severe skeletal deficiency. Case report A cleft lip and palate patient required complex multidisciplinary treatment to preserve health and restore esthetics. Dental leveling and alignment of the maxillary and mandibular teeth were provided before the surgery. Maxillary advancement and clockwise rotation of the maxillary-mandibular complex was applied by a Le Fort 1 osteotomy with two internal distraction devices. After the active treatment including orthodontic treatment and orthognathic surgery, upper full mouth ceramic restoration was applied. Conclusion This report shows the efficiency of internal distraction devices in cleft lip palate patients and exemplifies the multidisciplinary care required for such difficult cases. Dentists have several treatment options when restoring patients with congenitally missing maxillary lateral incisors. Endosseous implant therapy combined with orthodontics can achieve predictable outcomes while meeting patient expectations. This case report exhibits an interdisciplinary approach involving orthodontics, implant surgery, and prosthodontics to resolve this common dental concern.

He also had severe crowding in the maxillary arch and a congenitally missing lower right first permanent premolar. This case report describes the orthodontic treatment of a woman, aged 15 years, with permanent dentition, brachyfacial typology, with congenitally missing maxillary lateral incisors. Multibracket straightwire fixed appliance was used to open the space for dental implant placement, and treat the impaired occlusion. The missing lateral incisors were substituted with oral implants.

Background: Patients presenting with BIMC are relatively rare and the aim of the Orthodontist is to create a dentition that is functionally and aesthetically acceptable. The BIMC may result in altered facial appearance with malocclusion and psychological distress in some individuals. Case presentation: The present case report describes a 17-year-old female patient with nonsyndromic, BIMC. Clinical and CBCT examinations revealed BIMC. Following open surgical exposure treatment planning, the patient was treated with orthodontic fixed mechanotherapy to correct BIMC and other problems. Slow force mechanics with passive self-ligating MBT 0.022 orthodontic braces were used. Results: Open surgical exposure and active orthodontic treatment was completed in 18 months. The management of BIMC requires multidisciplinary approach to achieve better aesthetics and improved occlusal function. Conclusion: A combined Open Surgical Exposure-Orthodontic Traction-Orthodontic treatment would help achieve desired aesthetics and functioning occlusion. Fixed prosthetic restoration has become a primary treatment option for the replacement of congenitally missing lateral incisors where adequate Implant restorations have limited use due to many factors. The central incisor and canine often erupt less than optimal positions adjacent to the edentulous lateral incisor space, and therefore preprosthetic orthodontic treatment is frequently required. Derotation of the central incisor and canine, space opening and correction of root proximities may be required to create appropriate space, in which fixed prosthesis may achieve an esthetic restoration. This paper presents two case reports with congenital missing lateral incisors and one case report with missing central incisors due to trauma. The paper also discusses the aspects of preprosthetic orthodontic diagnosis and the treatment that needs to be considered with conservative and fixed prosthetic replacement .
