Gastro – intestinal duplications are usually detected in children before 2 years of age due symptoms/complications associated with the condition or during surgery in the child for some unrelated condition. In adults colonic duplication is of rare occurrence and often diagnosed during surgery. However, it may be diagnosed pre operatively due to symptoms of obstruction, volvulus or rarely due to perforation. We present an adult having tubular duplication of ascending colon, that presented with perforation. The duplication had a blind end and did not have distal communication. In addition, it had its own blood supply. The management of the case is discussed. Aetiogenesis of the anomaly is enumerated and literature reviewed. For ascending colon duplication similar meaning word accessory ascending colon has also been used in the manuscript.

Duplication of the gastrointestinal tract occur any part of the alimentary tract from the tongue to the anus. Male have slightly more predominance than females. Gastrointestinal duplication will have a varied presentation. Ileum and the oesophagus are most commonly involved. Colonic duplication is rare and can present with diagnostic difficulties. Gastrointestinal duplication has presence of well developed coat of smooth muscle, intimately attached to the gastrointestinal tract in the mesentric region and show a common blood supply with the native bowel. Sometime it will have an epithelial lining representing some portion of the alimentary tract.

Enteric duplications are rare congenital anomalies found anywhere from mouth to anus. Colonic duplications constitute about 13% of all enteric duplications. In this report a 6-year-old boy with chronic abdominal pain for a duration of last 2 years requiring intermittent hospital admissions was diagnosed as colonic duplication mimicking intestinal volvulus. Clinical findings are nonspecific and definitive diagnosis can only be made during surgical intervention and surgical treatment is advocated for all duplications. The topic is discussed under the light of relevant literature with a brief a brief literature review.

Intestinal duplications are rare congenital anomaly found in pediatric age group. Although, ileum is the most common site, there are cases of colonic duplications even in the adult. We report a case of 43 years woman presented with chronic constipation, intermittent colicky abdomen pain and a cystic lump in left abdomen diagnosed as tubular duplication cyst of descending colon. Colonoscopy is a good investigation tool for diagnosis of colonic duplications; however, it may not be true in all cases. She was managed with left hemicolectomy and excision of cyst with uneventful post-operative days. This case has been reported because of rarity of this condition.
