We present a modified bronchoplasty technique involving rotation of the bronchial structures. Our goal was to reconstruct the bronchus without using any foreign material while fully preserving the parenchyma. We used a biportal VATS approach. The centrally located bronchial tumor at the juncture between the right main bronchus, the right upper lobe bronchus, and the bronchus intermedius was first resected. The right upper lobe bronchus was rotated caudally, toward the bronchus intermedius, together with a slight clockwise rotation posteriorly to facilitate the approximation and tension-free closure of the bronchial defect. This video tutorial demonstrates the operative steps and explains how the rotational aspect is achieved.

Bronchial chondromas are very rare pulmonary benign tumors. Despite their rarity, clinicians should pay attention to the possibility of endobronchial tumor in patients with unexplained respiratory symptoms. Treatment modalities for complete excision of the tumor should be initiated to prevent complications and malignant transformation based on the individual tumor situation,

Primary bronchial tumors are extremely rare. However, symptoms, such as coughing and wheezing, are not specific to this disease, and primary bronchial tumors are often misdiagnosed as bronchial asthma. This report describes the case of a pregnant patient with a bronchial tumor that mimicked deteriorating bronchial asthma. A 37-year-old female patient suffered from repeated episodes of pneumonia since 26 weeks of gestation. Despite treatment, she suffered from another episode of pneumonia at 28 weeks of gestation. This was considered as deteriorating asthma. Bronchoscopy performed at 34 weeks of gestation showed a tumor in the left main lung bronchus, obstructing nearly 100% of the trachea. After cesarean delivery at 34 weeks, she underwent endoscopic bronchial tumor resection. Because of recurrent bronchial obstruction and the possibility of malignant disease, subsequent left main lung bronchial resection and bronchoplasty were performed. The pathological diagnosis was low-grade mucoepidermoid carcinoma. In conclusion, if pneumonia develops repeatedly during pregnancy, the possibility of bronchial tumor should be considered.

Abstract Background: Metastasis to the endobronchial from primary ovarian carcinoma (OC) is very rare. To enhance our understanding of this disease, we report a case of endobronchial metastasis from primary OC and review the literature. Case presentation: This is a case report and retrospective analysis of bronchial tumor patient secondary form primary OC. The patient was a 51-year-old woman who presented intermittent cough and expectoration for 3 month, and suffocating pneumonia for 3 weeks. Multiple nodular masses at the right upper lobe and thickening soft tissue with surrounding invasion of bronchi at the left upper lobe were found. The chest X ray scan demonstrated that enlarged right and left upper hilar, spreading mediastinum, elevated right septum were observed. Bronchoscopy showed that stenosis of the opening of right main bronchus and apical, middle and posterior segmental bronchi at right lobe was identified and the opening of left main bronchus was almost completely blocked by some visible neoplasm. The pathological result indicated that the disease was bronchial tumor derived from primary OC. Until now, only 12 cases were reported. The latent period ranged from 2 months to 21 years and the overall survival time ranged from 1.5 months to 5.5 years. Conclusions: The endobronchial is one of metastasis site of OC. The latent period of the disease is long and after active treatment, good prognosis can be acquired. This is the first study on endobronchial metastasis from primary OC, in China.

A case of a primary lung carcinoma with histologic and immunohistochemical features of a mammary carcinoma is presented. The patient is a 72-year-old man who presented with symptoms of cough and dyspnea. Diagnostic imaging showed a bronchial tumor in the left lower lobe that was surgically resected by a left lower lobectomy. The tumor was characterized by a homogenous cellular proliferation composed of small to medium-sized cells with round nuclei and inconspicuous nucleoli. Multiple immunohistochemical stains were performed, and the tumor was notably positive for estrogen receptor, progesterone receptor, GATA3, and pan-keratin, while molecular analysis showed somatic mutation in ARID1A. Clinical follow-up showed that the patient is alive and well 18 months post-surgical resection without evidence of recurrence or metastatic disease. Based on the overall features of this neoplasm, we consider that the tumor herein presented represents an unusual type of lung carcinoma that we refer to as primary mammary-like carcinoma of the lung.
