Tuberculosis is one of the most common diseases in Bangladesh and has variable clinical manifestations. Isolated Hepatic tuberculosis is not a common disease; in fact the presentation of hepatic tuberculosis may be without having history of any active Pulmonary TB or military TB. Patient usually present without having any typical symptoms; so it is difficult for a physician to diagnose the disease quickly. Patient presenting with space occupying lesion in liver is confused with abscess, hepatoma or metastases. Here we are presenting a case report of isolated hepatic tuberculosis.Bangladesh J Medicine Jan 2015; 26 (1) : 43-45

Pseudo-tumor hepatic tuberculosis is a rare entity. Tuberculosis involving the liver in the absence of active pulmonary tuberculosis is very uncommon. It is characterized by non-specific symptoms and radiological polymorphism. This case report illustrates the difficulty in reaching the correct diagnosis in case of hepatic masses, which are most often confused with carcinoma of the liver, primary or metastatic and hence, in the past referred to as pseudo-tumoral hepatic tuberculosis. We report a case of an inflammatory pseudo-tumor of the liver due to tuberculosis in a 6 years child. Computer tomography showed evidence of a large heterogenously enhancing predominantly cystic multiseptated mass in left lobe of liver with peripheral nodular enhancement and persistent enhancement of internal septations and nodular solid components. Hence malignant tumor was initially considered in view of the radiological findings. Alpha fetoprotein was within normal limits. The patient was treated surgically by debridement of left lobe of liver.

Hepatic tuberculosis (TB) is rare, but recently it is more frequently detected. On imaging, it presents with different appearances and can mimic a variety of other conditions. A high degree of suspicion combined with appropriate diagnostic modalities and image-guided tissue sampling examination greatly aid in the timely diagnosis of the disease. The sights of this case report are to illustrate and discuss the different presentation of hepatic TB on computed tomography. This entity is frequently related to caseous necrosis, which is the hallmark of this disease. However, our patient didn’t have any liver lesion in spite of deranged liver function tests.

Tuberculosis is a mycobacterial infection, but it can mimic many diseases including diseases of the musculoskeletal system. The need to consider it as a differential diagnosis of musculoskeletal tumours and tumour-like conditions cannot be overemphasized. The opinions of some, that tuberculosis is a disease of the undeveloped world, can no longer be sustained especially in the presence of immunodeficiency disorders, including HIV/AIDS, which have a worldwide prevalence. Traditionally, literature evidence shows that tuberculosis can mimic quite a number of diseases, including cancers. This has been found to be true, even in some technologically advanced countries. We present a case of hepatocellular cancer patient incidentally found to have concomitant hepatic tuberculosis who complained of bone lesion, with clinico-radiologic features of metastasis, but which turned out to be a tuberculous infection of the bone.
