Motor Vehicle Crashes (MVCs) are a public health problem in the United States. In 2009, 33,808 Americans were killed in a MVC and 2.22 million more were injured.4 Pulmonary contusion (PC) is a common injury following MVC with over 38% of the Abbreviated Injury Scale (AIS) 3+ thoracic injuries identified as some form of PC in a recent National Automotive Sampling System (NASS) study.5 Miller et al. correlated the percent injured lung to the possibility of developing Acute Respiratory Distress Syndrome (ARDS). The results indicated that if 20% of the lung was injured, the incidence of ARDS sharply increased with seventy-eight percent of those patients developing ARDS.2 The significance of these findings is that the volumetric measurement of PC can predict possible clinical outcomes.

Abnormalities of serum glucose in pediatric patients are commonly encountered in the emergency department and represent an acute threat to life and neurologic function. Rapidly identifying and aggressively treating hyperglycemia with diabetic ketoacidosis and hypoglycemia are critical to ensure the best possible outcome. This review will guide the emergency provider in the identification, resuscitation, workup, and disposition of these critically ill patients. Hypoxemia and obstructive sleep apnea (OSA) have been recognized as a threat to life. Nonetheless, information regarding the association between pre-dialytic pulse oximeter saturation (SpO2) level, OSA and mortality risks remains mysterious in patients with maintenance hemodialysis (MHD). Bioclinical characteristics and laboratory features were recorded at baseline. Pre-dialytic SpO2 was detected using a novel microchip LED oximetry, and the Epworth Sleepiness Scale (ESS) score greater than 10 indicated OSA. Non-adjusted and adjusted hazard ratios (aHRs) of all-cause and cardiovascular (CV) mortality were analyzed for pre-dialytic SpO2, OSA and potential risk factors. During 2152.8 patient-months of follow-up, SpO2 was associated with incremental risks of all-cause and CV death (HR: 0.90 (95% CI: 0.82–0.98) and 0.88 (95% CI: 0.80–0.98), respectively). The association between OSA and CV mortality was significant (HR: 3.19 (95% CI: 1.19–9.38). In the multivariate regression analysis, pre-dialytic SpO2 still had an increase in all-cause and CV death risk (HR: 0.88 (95% CI: 0.79–0.98), 0.82 (95% CI: 0.71–0.96), respectively). Considering the high prevalence of silent hypoxia in the post COVID-19 era, a lower pre-dialytic SpO2 level and severe OSA warn clinicians to assess potential CV risks. In light of clinical accessibility, the microchip LED oximetry could be developed as a wearable device within smartphone technologies and used as a routine screen tool for patient safety in the medical system.

Background Understanding COVID-19 risk perceptions and their impact on behaviour can improve the effectiveness of public health strategies in the future. Prior evidence suggests that, when people perceive uncontrollable risks to their health, they are less likely to make efforts to protect their health in those ways which they can control (e.g. through diet, exercise, and limiting alcohol intake). It is therefore important to understand the extent to which the threat of COVID-19 is perceived to be an uncontrollable risk, and to assess whether this perceived risk is associated with differences in health behaviour. MethodsWe surveyed a nationally representative sample of 496 participants, shortly after the peak of the pandemic in the UK. We collected data to assess people’s perceptions of COVID-19-related risk, and how these perceptions were associated with behaviours. We examined self-reported adherence to behaviours recommended by the UK Government and National Health Service to prevent the spread of the virus, as well as more general health behaviours. We predicted that increased perceived extrinsic mortality risk (the portion of a person’s mortality risk which they perceive to be uncontrollable) would disincentivise healthy behaviour. ResultsPerceived threat to life was found to be the most consistent predictor of reported adherence to measures designed to prevent the spread of infection. Perceived extrinsic mortality risk was found to have increased due to the pandemic, and was also associated with lower reported adherence to Government advice on diet and physical activity, as well as smoking. ConclusionsOur findings suggest that promoting a message that highlights threat to life may be effective in raising levels of adherence to measures of infection control, but may also have unintended consequences, leading to a reduction in health-promoting behaviours. We suggest that messages that highlight threat to life should be accompanied by statements of efficacy, and that messages evoking feelings of concern for others may also be effective in promoting compliance with anti-infection measures.
