The endoskeletal below knee prostheses was commercially available in Indonesia. More advanced products also available in the market but they come with expensive price and high maintenance cost. This paper presents a design concept of an affordable endoskeletal below knee prostheses for daily living with functional ankle joint. The design was developed through the application of appropriate technology approach from the total ergonomics framework which consists of 7 characteristics; technically feasible, ergonomic, energy saving, economical, socioculturally acceptable, environmental friendly, and trendy. The result is an endoskeletal below knee prostheses with multiaxis joint system and energy store-return components installed on its modular ankle joint.

Existing robotic transtibial prostheses provide only ankle joint actuation and do not restore biarticular function of the gastrocnemius muscle. This paper presents the first powered biarticular transtibial prosthesis, which is a combination of a commercial powered ankle-foot prosthesis and a motorized robotic knee orthosis. The orthosis is controlled to emulate the human gastrocnemius based on neuromuscular models of matched nonamputees. Together with the ankle-foot prosthesis, the devices provide biarticular actuation. We evaluate differences between this biarticular condition and a monoarticular condition with the orthosis behaving as a free-joint. Six participants with transtibial amputation walk with the prosthesis on a treadmill while motion, force, and metabolic data are collected and analyzed for differences between conditions. The biarticular prosthesis reduces affected-side biological knee flexion moment impulse and hip positive work during late-stance knee flexion, compared to the monoarticular condition. The data do not support our hypothesis that metabolism decreases for all participants, but some participants demonstrate large metabolic reductions with the biarticular condition. These preliminary results suggest that a powered artificial gastrocnemius may be capable of providing large metabolic reductions compared to a monoarticular prosthesis, but further study is warranted to determine an appropriate controller for achieving more consistent metabolic benefits.

Aim of study: To investigate the short- and long-term outcome of patients with isolated lateral malleolar fracture type B treated with a single hemicerclage out of metallic wire or PDS cord. Methods: Over an 8-year period 97 patients were treated with a single hemicerclage for lateral malleolar fracture type B and 89 were amenable to a follow-up after mean 39 months, including interview, clinical examination and X-ray controls. Results: The median operation time was 35 minutes (range 15-85 min). X-ray controls within the first two postoperative days revealed an anatomical restoration of the upper ankle joint in all but one patient. The complication rate was 8%: hematoma (2 patients), wound infection (2), Sudeck's dystrophy (2) and deep vein thrombosis (1). Full weight-bearing was tolerated at median 6.0 weeks (range 2-26 weeks). No secondary displacement, delayed union or consecutive arthrosis of the upper ankle joint was observed. All but one patient had restored symmetric joint mobility. Ninety-seven percent of patients were satisfied or very satisfied with the outcome. Following bone healing, hemicerclage removal was necessary in 19% of osteosyntheses with metallic wire and in none with PDS cord. Conclusion: The single hemicerclage is a novel, simple and reliable osteosynthesis technique for isolated lateral type B malleolar fractures and may be considered as an alternative to the osteosynthesis procedures currently in use.

Kata kunci: aseuk hatong, kawih, komunikasi seni, teknologi tani, pertanian ladangABST RACTOur inability to deepen our own original knowledge results in disharmony of technologywith the needs in the field, including the Appropriate Technology (TTG) in agriculture. It istherefore that in any development it is often not harmonious with the needs of society. Theobjective of the research is to know the role of Aseuk Hatong in the agricultural tradition in theTatar Karang Priangan community of Tasikmalaya Regency whicht is religiously Islamic yetstill maintains the tradition. The research method is Schutz phenomenology study. The resultsof research show that (1) the art of communicating (kakawihan) in Aseuk Hatong tradition isa harmonious effort between farmers and the dry nature through mild, contextual humming.(2) Farm technology (Aseuk Hatong)is a tool to cultivate the soil, plowing the fields during thedry season by wrapping with simple typical musical aesthetics of field farmers as the resultsof appropriate technology development, fun, innovative, functional, affordable, cheap, andenvironmentally friendly. As the conclusion, it can be said that Aseuk Hatong Tradition in TatarKarang Priangan as a persuasive media farming during ngahuma (farming) not accompaniedby rainy season is a technology development that takes into account local wisdom occur. Keywords: aseuk hatong, kawih, art communication, farming technology
