Radiologic technologists face various types of patients during multi-detector computed tomography (CT) examinations. In emergency departments, it is common to have patients who cannot follow instructions for the examinations. The asymmetric axial view of the head CT might affect the correctness of the clinician’s diagnosis. This study aimed to assess the impact of head positioning on the image quality of head CT using two phantoms. All scans were performed on a 16-slice CT scanner. In the control group, the tilted angle of the phantoms was 0[Formula: see text], and no multiplanar reconstruction (MPR) was performed. In the experimental groups, the tilted angles of the phantoms were 5[Formula: see text], 10[Formula: see text] and 15[Formula: see text], respectively, and MPR was performed afterwards. The results showed that if the head was tilted during the head CT examinations, image asymmetry and artifacts appeared without MPR. After MPR, one phantom showed that there were significant differences and the other phantom showed no significant differences quantitatively in image symmetry and artifacts between experimental groups and the control group, while both phantoms showed no significant differences qualitatively in image symmetry and artifacts between experimental groups and the control group. Although MPR can correct the image asymmetry and artifacts caused by tilted head positioning to some extent, it consumes time. Therefore, technologists should position the head as exactly as possible when performing head CT examinations.

Objectives: The objective of the following study is to evaluate the effect of reducing tube voltage from 120 to 100 kVp using prospective gating 320 row multi-detector computed tomography angiography on image quality and reduction in radiation dose. Materials and Methods: A total of 78 sequential patients were scanned with prospective electrocardiogram gating. A total of 45 patients (Group 1) with mean body mass index (BMI) 29 ± 2 and heart rate (HR) 57 ± 7 beats per minute (BPM) were scanned at 120 kVp. 33 patients (Group 2) with mean BMI 23 ± 3 and HR 58 ± 6 bpm were scanned at 100 kVp. Effective dose was calculated using dose length product and factor (k = 0.014). Quantitative assessment of image quality was calculated by measuring signal to noise ratio (SNR) and contrast to noise ratio (CNR) in the left ventricle and left main coronary artery. Two experienced cardiac radiologists using a three-point ordinal scale assessed subjectively image quality. Results: In Group 1, the median radiation dose was 5.31 mSv (95% confidence interval [CI]: 4.86-6.09) and for Group 2 (P = 0.009) the mean radiation dose was 3.71 mSv (95% CI: 2.76-4.87), representing 30% decrease in radiation dose. In multivariate analyses, adjusting for age, gender, HR, BMI, tube current and scan length, an absolute median reduction of 2.21 mSv (1.13-3.29 mSv) was noted in patients scanned with 100 kVp (P < 0.0001). The quantitative image quality (SNR and CNR) was not statistically significant between the groups. Subjective image quality was rated as good or excellent in 99% of coronary segments for both groups (P value was considered as non-significant). Conclusion: Our study suggests that radiation dose may be lowered from 120 to 100 kVp with preservation of image quality in patient's whose BMI is ≤27.

OBJECTIVE: This study aims to analyze and compare the diagnostic effectiveness of 320-row multi-detector computed tomography for coronary artery angiography (MDCTA) in subjects with and without sublingual vasodilator (nitroglycerin). MATERIALS AND METHODS: From September 2015 to September 2016, 70 individuals without history of major cardiovascular diseases who underwent MDCTA for health examination were retrospectively categorized into sublingual nitroglycerin (NTG) and non-NTG groups. Medical history, CT dose index (CTDI), and multi-slice CT images were compared between two groups. A diameter of coronary artery (DA, mm) was computed and analyzed. RESULTS: A total of 41 males and 29 females (mean age: 55.43±8.84 years, range: 34– 76) were reviewed. Normal and abnormal MDCTA findings were noted in 54 and 16 participants, respectively, with the detection rate of coronary artery disease being 23%. There was no significant difference in inter-observer variability of coronary CTA image quality and diagnosis between the NTG and non-NTG groups among three experienced radiologists. Although the percentage dilatation of left anterior descending branch (LAD), right coronary artery (RCA) and left circumflex branch (LCX) following in the NTG group were 12.4%, 12.8% and 25.3%, respectively (p < 0.01), there was no significant difference in image quality and diagnosis between the two groups. CONCLUSIONS: Despite the recommendation of routine nitroglycerin use for subjects undergoing computed tomography for coronary artery angiography, our results showed no significant advantage of its use in improving image quality and rate of diagnosis accuracy.

Abstract Funding Acknowledgements Type of funding sources: None. Background The natural history of thoracic aortic aneurysm (TAA) is one of progressive expansion. Asymptomatic patients who do not meet criteria for repair require conservative management including ongoing aneurysm surveillance, mostly carried out by contrast-enhanced computed tomography angiography (CTA). Purpose To prospectively compare image quality and reliability of a prototype non-contrast, self-navigated 3D whole-heart magnetic resonance angiography (MRA) with contrast-enhanced computed tomography angiography (CTA) for sizing of thoracic aortic aneurysm (TAA). Methods Self-navigated 3D whole-heart 1.5 T MRA was performed in 20 patients (aged 67 ± 8.6 years, 75% male) for sizing of TAA; a subgroup of 18 (90%) patients underwent additional contrast-enhanced CTA on the same day. Subjective image quality was scored according to a 4-point Likert scale and ratings between observers were compared by Cohen’s Kappa statistics. Continuous MRA and CTA measurements were analyzed with regression and Bland-Altman analysis. Results Overall subjective image quality as rated by two observers was 1 [interquartile range (IQR) 1-2] for self-navigated MRA and 1.5 [IQR 1-2] for CTA (p = 0.717). For MRA a perfect inter-observer agreement was found for presence of artefacts and subjective image sharpness (κ=1). Subjective signal inhomogeneity correlated highly with objectively quantified inhomogeneity of the blood pool signal (r = 0.78-0.824, all p &lt;0.0001). Maximum diameters of TAA as measured by self-navigated MRA and CTA showed excellent correlation (r = 0.997, p &lt; 0.0001) without significant inter-method bias (bias -0.0278, lower and upper limit of agreement -0.74 and 0.68, p = 0.749). Inter- and intraobserver correlation of aortic aneurysm as measured by MRA was excellent (r = 0.963 and 0.967, respectively) without significant bias (all p ≤ 0.05). Conclusion Self-navigated 3D whole-heart MRA enables reliable contrast- and radiation free aortic dilation surveillance without significant difference to standardized CTA while providing predictable acquisition time and by offering excellent image quality.
