The occurrence of Alzheimer’s disease (AD) is growing, with 68% of cases occurring in women. Declines in global function exacerbated by reversal of day–night patterns, disturbed sleep–wake rhythms, and excessive daytime sleepiness make managing AD difficult. In this study, the authors examined the effect and duration of effect of therapeutic light on sleep, rest–activity, and global function in women with AD using mixed methods in a two-group experimental design with repeated measures on one factor. Twenty women with AD were randomized to experimental or control conditions. Blue-green or dim red light was delivered via cap visor in the morning. Results of the qualitative analysis of serial interviews with family and facility caregivers regarding perceived effect of light on global function are presented. Themes emerged in both groups with respect to cognition and psychosocial function. Future studies with larger samples using quantitative measures of global function are warranted to verify findings.

Aim: The clock drawing test (CDT) is widely used as a visual spatial ability test and screening test for dementia patients. The appearance frequency of qualitative errors obtained through the qualitative analysis of CDT may be related to the participant’s falls. The aim of this study was to clarify the difference in the number of people who presented with qualitative errors in the CDT between a fall and non-fall group of patients with Alzheimer’s disease (AD). Methods: The CDT was implemented for 47 patients with AD. A quantitative analysis was conducted, and a qualitative analysis was performed for errors. The patients were divided into two groups based on their history of falls over the past year. The results of the CDT quantitative analysis were tested using the Mann-Whitney U test, and Fisher’s exact test was employed to determine the difference in the number of people who presented with error types between the two groups (fall group, non-fall group) in the CDT qualitative analysis. Results: In the quantitative analysis, a significant difference was found for the total scores, with the total CDT score of the fall group (n = 22) significantly lower than that of the non-fall group (n = 25) (p = 0.006, effect size: φ = 0.40). In the qualitative analysis, a significantly higher number of patients in the fall group than in the non-fall group presented with a conceptual deficit (p =0.001, φ = 0.51). No differences were found in the number of patients in the two groups who presented with the other five error types. Conclusions: These results showed that a lower score in the CDT quantitative analysis might suggest an increased risk of falls. It was also clarified that a larger number of patients in the fall group than in the non-fall group presented with a conceptual deficit of the qualitative error types in the CDT. Therefore, these results suggest that the appearance of a conceptual deficit may be an index for the selection of patients with AD prone to falling when implementing fall prevention measures.

Purpose To date, verb fluency tasks have been mainly analyzed quantitatively for individuals with dementia. Qualitative analysis, however, such as examining the semantic and psycholinguistic content of the responses might further inform researchers and clinicians about patients' cognitive and linguistic status. Therefore, the current study examined psycholinguistic and lexical characteristics of verb fluency responses in individuals with probable Alzheimer's disease (pAD) and healthy older adults to delineate qualitative and quantitative differences between the two groups. Method The verb fluency responses from participants with pAD (amnestic type) were compared to those from age- and education-matched healthy older adults. The responses were analyzed with respect to the number and proportion of correct responses, word frequency, age of acquisition, phoneme and syllable length, and neighborhood density. The verb responses were also categorized into mental state verbs and action verbs. Additionally, a battery of cognitive–linguistic tests was administered, and for each group, relationships between correct verb fluency responses and other cognitive–linguistic skills were investigated using correlation and regression analyses. Results Similar to previous findings regarding noun retrieval in dementia, the results revealed that individuals with pAD not only produced fewer correct verb fluency responses but also generated little to no mental state verbs compared to the control group. The group with pAD also produced verbs with shorter phoneme and syllable lengths, higher word frequency, and earlier age of acquisition ratings relative to the healthy older adults. The number of correct verb fluency responses was mainly predicted by a reading comprehension score in the pAD group and a nonverbal fluency test score in the healthy group. Conclusion The current quantitative and qualitative findings add support to the contention that lexical–semantic impairments underlie word retrieval problems in pAD and such difficulties present in generative naming paradigms and also across grammatical categories.
