The aim of the work was to analyze the long-term results of using a temporary “liquid” implant in the surgery of congenital glaucoma in children. The basis of the developed method of filtrative antiglaucomatous surgery (Patent of Ukraine No. 45099 of 2009) – viscosinusotrabeculotomy – has been set the task of reducing the risk of developing intra- and postoperative complications, reducing the scarring rate and maintaining the newly created ways of the intraocular fluid outflow, which in general will increase the effectiveness of surgical treatment of congenital glaucoma in children. 54 children (91 eyes) with simple congenital glaucoma at the age of 1 to 36 months were operated on average (8.7 ± 8.2) months. The persistent and long-lasting hypotensive effect achieved due to viscosinusotrabeculotomy in children with developed and far-advanced stages of congenital glaucoma stops the processes of stretching of the membranes of the eye and stabilizes their size, which in general allows preserving and visual functions improving, in infancy – creating conditions for their formation.

Aim. To study the effectiveness of antihypertensive operations of trabeculotomy ab externo and sinus trabeculoectomy in our modification in patients with congenital glaucoma and to justify its application. Methods. The following surgeries were performed: trabeculotomy ab externo - on 57 eyes of 51 patients, sinus trabeculoectomy - on 46 eyes of 42 children, classic trabeculoectomy - on 23 eyes of 19 patients, and trabeculoectomy with elements of intrascleral autodrainage in our modification - on 23 eyes of 23 patients. Among them, 22 patients were younger than 1 month, aged from 1 to 6 months - 30, from 6 months to 1 year - 18, from 1 to 3 years - 15 and from 3 to 5 years old - 8 patients. Results. Long-lasting hypotension was achieved in 86.95% of patients after sinus trabeculoectomy, in 80.39% of patients after trabeculotomy ab externo. The best results were obtained in patients younger than 6 months. In the older age groups, antihypertensive effect of trabeculotomy ab externo markedly decreased and was only 50% at the age of 3-5 years. The results after sinus trabeculoectomy were comparable in all age groups. Conclusions. Trabeculotomy ab externo can be recommended as an operation of choice during the first months of life in congenital glaucoma patients, based on the characteristics of the anterior chamber angle.

Introduction. Glaucoma is a chronic, progressive disease of the optic nerve which if left untreated can lead to blindness at end stages. A decrease of intraocular pressure (IOP) has proven to slow down the progression of the disease. IOP decrease can be achieved by medical, laser and surgical treatment. Objective. The aim of this study was to evaluate the response of patients with medically uncontrolled primary open angle glaucoma to selective laser trabeculoplasty (SLT). Methods. The study involved baseline characteristics recorded for each of 35 patients (48 eyes) in whom, despite being under full medication we could not achieve a satisfactory IOP. Patients, who had pressure above 25 mmHg under the maximal medication therapy, were not included into the study and were referred for surgical treatment. IOP was measured on admission, 1 hour, 7 days, 1, 3, 6 and 12 months after SLT. We considered satisfactory surgical result if IOP was decreased more than 20% of the initial value. Also, we investigated the influence of baseline IOP on SLT outcome after 12 months. Patient inclusion criteria were inability to reach target IOP with maximal medical therapy. Exclusion criteria were congenital glaucoma, any type of angle closure glaucoma, advanced-stage glaucoma, eyes with previous laser or surgical glaucoma applications and patients with baseline IOP >25 while fully medicated. Patients who could not be followed for at least 12 months were also excluded. Results. The mean age of our patients was 73?12 years. The mean baseline IOP was 20.48 mmHg (SD=1.91), and the mean change in IOP from baseline of the treated eye after one year was 4.47 mmHg (SD=2.12). In eyes with a higher baseline IOP the reduction of pressure at the end of the study was significantly higher. Satisfactory effect of IOP reduction after one year was achieved in 64.58% of eyes. The IOP reduction did not show to be dependent as regarding age and gender. Conclusion. SLT effectively lowers IOP in patients with primary open-angle glaucoma, and the intervention is not followed by significant complications. Our results confirm that the IOP reduction is more significant if the initial value is higher. Our first reliable results of IOP reduction were confirmed one month after the procedure so that the procedure should not be repeated before one month has elapsed. The study is limited by a small number of eyes, which is insufficient to make a complete case analysis.

A cegueira é definida como a incapacidade de captação de informações do mundo através do sentido da visão. Em recém-nascidos e crianças, o glaucoma congênito é uma das principais patologias desencadeantes da cegueira, sendo o diagnóstico precoce fundamental para orientar o tratamento e alterar o prognóstico da patologia. A literatura, no geral, traz o tratamento cirúrgico como primeira linha adotada na abordagem do Glaucoma Congênito, sendo as principais técnicas utilizadas a goniotomia e a trabeculotomia. Tendo em vista a escassez de relatos na literatura local a esse respeito, o objetivo deste artigo consiste em verificar a conduta mais prevalente em uma série de casos de Glaucoma Congênito na Rede Pública do Mato Grosso do Sul. Este estudo é do tipo descritivo, quali-quantitativo, observacional, retrospectivo, baseado em análise dos registros nos prontuários dos pacientes atendidos na rede pública de saúde de Campo Grande – MS. Dos oito casos analisados, um sofreu evisceração com colocação de prótese, dois não tiveram o nome da técnica cirúrgica relatada e os outros cinco realizaram trabeculectomia. Apesar da maioria dos casos encontrados ter sido abordada com o tratamento cirúrgico, conforme o preconizado pela literatura, o procedimento eleito na maioria dos casos, a trabeculectomia, difere daquele citado na literatura como o de primeira escolha. Palavras-chave: Glaucoma. In newborns and children, congenital glaucoma is one of the main causes of blindness, and early diagnosis is fundamental to guide treatment and change the prognosis of the condition. The literature adopts surgical treatment as the first line for the approach of Congenital Glaucoma, the main techniques being used for goniotomy and trabeculotomy. The objective of this article is to verify the most prevalent behavior in the series of cases of Congenital Glaucoma in the Public Network of MS, considering the scarcity in the literature, in order to evaluate if the techniques adopted in clinical practice follow what the medicine based in evidences presents as more appropriate. This is a descriptive, qualitative, observational, retrospective study, based on the records analysis in the medical patients records attended  in the public health network in Campo Grande - MS. Of the eight analyzed cases, one had evisceration with prosthesis placement, two had no name of the surgical technique reported and the other five underwent trabeculectomy. Although most of the cases are treated with the surgical treatment as recommended by the literature, the procedure of first choice is the goniotomy technique. Differently from what was found in the ducts of the analyzed cases, the majority were treated with trabeculotomy. It is believed that the significant use of this technique relates to its greater ease of execution, transmitting more operational safety to the surgeons. Keywords: Glaucoma. Treatment Outcome. Purpose: To identify psychosocial indicators and evaluate the filtering surgeries impacts on quality of life (QOL) of children with primary congenital glaucoma (PCG) and their family. Methods: Parents of children with bilateral PCG who underwent filtering surgery were included. Data were collected through: (a) psychological inquiry to determine psychosocial indicators; (b) Children’s Visual Function Questionnaire (CVFQ). The associations between the CVFQ scores and visual acuity, intraocular pressure, optic disk cupping, horizontal corneal diameter, axial length, number of surgeries, and hypotensive eye drops were investigated. The statistical significance level was considered as p ⩽ 0.05. Results: The mean age of the nine mothers interviewed and their children (six boys and three girls) was 29 ± 5 years and 35 ± 18 months, respectively. The psychosocial indicators determined were: emotional impact of the diagnosis, disease knowledge, mother and family’s feelings on facing the surgical treatment, surgical result comprehension, treatment adherence, child's emotional and behavioral reactions, social support, and future expectations. In CVFQ analysis, QOL score was strongly associated with visual acuity ( r = −0.79; p = 0.01). Besides the treatment score was correlated to intraocular pressure ( r = −0.68; p < 0.05), optic disk cupping ( r = −0.85; p = 0.03), and corneal diameter ( r = −0.69; p = 0.02). Correlations were not found for number of surgeries and eye drops. Conclusions: This study confirmed the PCG surgical treatment impact on QOL and determined psychosocial indicators which may favor the suitable actions in psychological treatment and follow-up of the children and families.

This article presents an analysis of the results of the algorithm of management of children with refractory glaucoma which included: preparation and carrying out surgical treatment, analysis of postoperative period of children with refractory glaucoma. Thetotal number of children diagnosed with glaucoma was 478. Of them 82.1%, 410 children underwent surgical treatment. 30.2% (125 children) needed reoperation. The results of the analysis of the clinical and functional state of the visual organ in children for the period from 2016 to 2020 are given.
