Without telemedicine adoption as a standard of care, physicians run the risk of losing patients to on-demand telemedicine organizations. Through telemedicine, not only do patients get a more convenient and cost-effective experience, providers have the opportunity to grow their practice and increase patient satisfaction. In this article, Dr. Timothy Porter, a community pediatrician in Chicago, shares his perspective.

Each and every automobile in service or being developed in the industry is benchmarked on the basis of its efficiency in running real conditions. So in our project here we have tried to develop a complete new damper and spring setup which can be used in all sorts of suspension systems and in turn provides a feedback loop of voltage which can then be used charge the batteries and upscale the efficiency of bikes by (5-6)% & and for HUV or Sedans by (2-4)% (can even go higher) depending on the terrain. In this setup we harness the mechanical energy into electrical where earlier it was left as heat and vibrational losses. This setup is as cost effective as the earlier dampers where as providing an efficient output in minimal cost increase due to its novelty. The other Features include Electronic height adjustment & on demand suspension softness or stiffness. Wireless Mesh Networks (WMNs) is becoming an emerging paradigm, due to its simple and cost effective deployment, and increased growth in popularity for the next generation wireless Internet. Unlike the single channel, it is challenging to provision a robust multicasting by means of multi-channel and multi-interface WMNs. Therefore, in order to tackle the multicast issues, a high-quality path selection and channel assignment are essential. The proposed Quality of Service – On Demand Multichannel Multicast Routing Protocol (QoS- ODMMRP) includes merged path selection and top-down channel tuning mechanism to support multi-channel and multi-interface WMNs. Thus, the proposed work reduces the number of transmissions and increases the communication throughput efficiently.

Background: Gastroesophageal reflux disease (GERD) is a common gastrointestinal disorder that results from regurgitation of acid from the stomach into the esophagus. Treatment available for GERD includes lifestyle changes, antacids, histamine-2 receptor antagonists (H2RAs), proton pump inhibitors (PPIs), and anti-reflux surgery. Aim: The aim of this review is to assess the cost-effectiveness of the use of PPIs in the long-term management of patients with GERD. Method: We searched in PubMed to identify related original articles with close consideration based on inclusion and exclusion criteria to choose the best studies for this narrative review. The first section compares the cost-effectiveness of PPIs with H2RAs in long-term heartburn management. The other sections shall only discuss the cost-effectiveness of PPIs in 5 different strategies, namely, continuous (step-up, step-down, and maintenance), on-demand, and intermittent therapies. Results: Of 55 articles published, 10 studies published from 2000 to 2015 were included. Overall, PPIs are more effective in relieving heartburn in comparison with ranitidine. The use of PPIs in managing heartburn in long-term consumption of nonsteroidal anti-inflammatory drug (NSAID) has higher cost compared with H2RA. However, if the decision-maker is willing to pay more than US$174 788.60 per extra quality-adjusted life year (QALY), then the optimal strategy is traditional NSAID (tNSAID) and PPIs. The probability of being cost-effective was also highest for NSAID and PPI co-therapy users. On-demand PPI treatment strategy showed dominant with an incremental cost-effectiveness ratio of US$2197 per QALY gained and was most effective and cost saving compared with all the other treatments. The average cost-effectiveness ratio was lower for rabeprazole therapy than for ranitidine therapy. Conclusion: Our review revealed that long-term treatment with PPIs is effective but costly. To achieve long-term cost-effective approach, we recommend on-demand approach to treat heartburn symptoms, but if the symptoms persist, treatment with continuous step-down therapy should be applied.

Remote clouds are gradually unable to achieve ultra-low latency to meet the requirements of mobile users because of the intolerable long distance between remote clouds and mobile users and the network congestion caused by the tremendous number of users. Mobile edge computing, a new paradigm, has been proposed to mitigate aforementioned effects. Existing studies mostly assume the edge servers have been deployed properly and they just pay attention to how to minimize the delay between edge servers and mobile users. In this paper, considering the practical environment, we investigate how to deploy edge servers effectively and economically in wireless metropolitan area networks. Thus, we address the problem of minimizing the number of edge servers while ensuring some QoS requirements. Aiming at more consistence with a generalized condition, we extend the definition of the dominating set, and transform the addressed problem into the minimum dominating set problem in graph theory. In addition, two conditions are considered for the capacities of edge servers: one is that the capacities of edge servers can be configured on demand, and the other is that all the edge servers have the same capacities. For the on-demand condition, a greedy based algorithm is proposed to find the solution, and the key idea is to iteratively choose nodes that can connect as many other nodes as possible under the delay, degree and cluster size constraints. Furthermore, a simulated annealing based approach is given for global optimization. For the second condition, a greedy based algorithm is also proposed to satisfy the capacity constraint of edge servers and minimize the number of edge servers simultaneously. The simulation results show that the proposed algorithms are feasible.
