Background: Post-endoscopic retrograde cholangiopancreatography pancreatitis (PEP) is a serious disease. The role of nonsteroidal anti-inflammatory drugs (NSAIDs) in the prevention of PEP is conflicting. Objectives: This review aimed to assess the preventive role of NSAIDs in PEP with special emphasis on the dose and route of administration. Methods: We searched PubMed and Google Scholar for relevant observational studies published in English during the period from January 2010 to January 2020. The terms post-ERCP pancreatitis, diclofenac sodium, indomethacin, NSAIDs, dose, route of administration were used. Results: Of the 179 identified, 19 full texts were screened and included in the review. Ten studies were from Europe, seven from Asia and two were published in the USA, the studies showed that NSAIDs were effective in preventing PEP when used rectally or intramuscularly, higher doses are more efficacious and the combination with stents was not superior, careful patients selection is needed in particular regarding the body mass index. Conclusion: NSAIDs were effective in PEP prevention; however, the evidence is weak due to the observational nature and the different methods used in the included studies. Randomized controlled studies are needed to solve the issue.

Worldwide immunization can save millions of peoples to lives year by using the vaccines. The subunit of antigen components is manufactured which can stimulate the immune system by providing specific immunity against specific diseases. Subunit vaccines have many advantages like as high safety profile but having limited ability to provide immunogenicity. These traditional subunit vaccines activate only innate immunity, encourage cell-mediated transport of antigen to lymphoid tissues. Newly nano-adjuvants based vaccines carrier systems like liposomes, virosome, micelles, polymeric particles, protein, and peptides are developed by using various substances like viral proteins, polymer and polystyrene having immanent adjuvanticity and also provide exalted capability in manufacturing subunit vaccines. It has chromospheres substances that have various properties such as targeted, anti-damaging and caliber to lead immune reactions towards Th1 and Th2 route, which is an important feature for humoral as well as cellular immunity. The whole thing based on the carrier system, the role of nano-adjuvants, its pharmacokinetics and distribution in the body system. It has the ability to provide antigen-specific immunity to both systemic as well as mucosal by different vaccination passage. Also, the nano-adjuvants based vaccine suggested that direct targeting of antigen to improve the vaccine potency without sacrificing safety.

Background: Of all the postsurgical pain conditions, thoracotomy pain poses a particular therapeutic challenge in terms of its prevalence, severity, and ensuing postoperative morbidity. Multiple pain generators contribute to the severity of post-thoracotomy pain, and therefore a multimodal analgesic therapy is considered to be a necessary strategy. Along with opioids, thoracic epidural analgesia, and paravertebral blocks, N-Methyl-D-Aspartate (NMDA) receptor antagonists such as ketamine have been used as adjuvants to improve analgesia. Objective: We reviewed the evidence for the efficacy of intravenous and epidural administration of ketamine in acute post-thoracotomy pain management, and its effectiveness in reducing chronic postthoracotomy pain. Study Design: Systematic literature review and an analytic study of a data subset were performed. Methods: We searched PubMed, Embase, and Cochrane reviews using the key terms “ketamine,” “neuropathic pain,” “postoperative,” and “post-thoracotomy pain syndrome.” The search was limited to human trials and included all studies published before January 2015. Data from animal studies, abstracts, and letters were excluded. All studies not available in the English language were excluded. The manuscript bibliographies were reviewed for additional related articles. We included randomized controlled trials and retrospective studies, while excluding individual case reports. Results: This systematic literature search yielded 15 randomized control trials evaluating the efficacy of ketamine in the treatment of acute post-thoracotomy pain; fewer studies assessed its effect on attenuating chronic post-thoracotomy pain. The majority of reviewed studies demonstrated that ketamine has efficacy in reduction of acute pain, but the evidence is limited on the long-term benefits of ketamine to prevent post-thoracotomy pain syndrome, regardless of the route of administration. A nested analytical study found there is a statistically significant reduction in acute post-thoracotomy pain with IV or epidural ketamine. However currently, the evidence for a role of ketamine as a preventative agent for chronic postthoracotomy pain is insufficient due to the heterogeneity of the studies reviewed with regard to the route of administration, dosage, and outcome measures. Limitations: The evidence for a role of ketamine as a preventative agent for chronic post-thoracotomy pain is insufficient due to the heterogeneity of the studies reviewed. Conclusion: The majority of randomized controlled trials reviewed show no role for ketamine in attenuating or preventing post-thoracotomy pain syndrome at variable follow-up lengths. Therefore, additional research is warranted with consideration of risk factors and long-term follow-up for chronic post-thoracotomy pain though the evidence for benefit appears clear for acute post-thoracotomy pain. Preventive vaccination is currently the most affordable and economical way to reduce morbidity and mortality from many infections, improve quality and human life expectancy with an almost ideal balance of benefits and risks among all medical procedures. The article deals with the reasons for variability of the immune response caused by vaccines, between individuals and between populations, which is of fundamental importance for human health. The authors have presented data indicating a key role of the gut microbiota in the control of the immune response to vaccination. Particular attention is paid to the microbial diversity in different loci of the body. The role of microorganisms in the proper functioning of the body and the formation of a number of pathological conditions is described. Most modern vaccines are live-attenuated, killed / inactivated or subunit (recombinant) vaccines, and they are designed for the parenteral route of administration. Most of these vaccines elicit a weak immune response, especially in the mucous membranes, due to the route of administration and are associated with weak cell-mediated immunity. Therefore, mechanisms that can enhance virus-specific vaccine immunity in infants and children are required, such as the use of more potent or selective immunity-enhancing adjuvants. Some probiotic strains may be considered as promising vaccine adjuvants. This article evaluates the recent clinical studies of probiotics used to enhance vaccine-specific immunity in adults and infants. The present-day knowledge on the role of the probiotic strain Lactobacillus rhamnosus GG with the aim of activating immunity after vaccination are presented.
