ABSTRACT Fusion is a developmental anomaly in tooth morphology, which may be due to either union of two separate tooth buds or partial splitting of a single tooth bud. A thorough understanding of root canal anatomy is an essential prerequisite for a successful endodontic treatment. This case report describes successful endodontic management of a maxillary left lateral incisor fused with a supernumerary tooth using spiral computed tomography (SCT) as a diagnostic aid. A patient reported with a painless swelling in the maxillary anterior region associated with a fused maxillary lateral incisor. Spiral CT was performed to better understand the complex root canal anatomy. Endodontic treatment was done using methyl trioxide aggregate (MTA) as apical plug and obturation by thermoplasticized gutta-percha. The tooth was completely asymptomatic at 1-year follow-up showing signs of healing. The present case report emphasizes on the importance of three-dimensional (3D) imaging techniques, such as SCT in cases of developmental anomalies having complicated root canal morphology for successful endodontic therapy. ABSTRACT Synodontia or fusion is union of two independently developing primary or secondary teeth. In the case reported here, clinical and radiographic examination suggested fusion between maxillary lateral incisor and a supernumerary tooth with the resultant loss of eruption space for permanent canine. Since the teeth exhibited separate pulp chambers without any pulpal involvement, surgical separation followed by odontoplasty was done. Follow-up revealed the tooth to be asymptomatic and sufficient space for eruption of canine was created. The case report highlights the surgical management in case of fusion and timely intervention to prevent periodontal, endodontic and orthodontic complications.

The proper knowledge of the root canal system anatomy is crucial to perform successful endodontic treatments. Maxillary lateral incisors usually have a single root. However, aberrant canal configurations of this tooth have been reported in the literature. The clinician should be aware of the difficulties arising from this more complex root canal anatomy and should be able to manage them. A rare case of a maxillary lateral incisor with three root canals is presented in this article; the challenge of diagnosis and treatment in similar situations is also discussed.

Anterior spacing is a common esthetic problem of patient during dental consultation. The most common etiology include tooth size and arch length discrepancy. Maxillary lateral incisors vary in form more than any other tooth in the mouth except the third molars. Microdontia is a condition where the teeth are smaller than the normal size. Microdontia of maxillary lateral incisor is called as “peg lateral”, that exhibit converging mesial and distal surfaces of crown forming a cone like shape. A carefully documented diagnosis and treatment plan are essential if the clinician is to apply the most effective approach to address the patient’s needs. A patient sometimes requires a multidisciplinary approach to correct the esthetics and to improve the occlusion. This case report describes the management of an adult female patient with a proclined upper anterior teeth, upper anterior spacing, deep bite and peg shaped upper right lateral incisor tooth through orthodontic and restorative treatment approach.
