Currently, there is a global trend towards an increase in the incidence of type 1 diabetes mellitus (DM 1) among children and adolescents. It is characterized by a lifelong progressive course, manifested by endocrine and somatic disorders, as well as neurological and mental complications. The risk of the development of emotional disorders, cognitive dysfunction, adjustment disorder is largely determined by the microsocial environment of a child with DM 1. Objective — to study role of family in the formation of the disorders inmental and psychological health of children and adolescents with severe DM 1. Materials and methods. Examinations involved 285 patients with DM 1(126 children and 159 adolescents). The investigation design included clinical and psychopathological method, pathopsychological method, socio­psychological interviewing of a child and his/her family, test “Family sociogram”, projective picture tests “House­tree­man”, “Me and my disease”, “Kinetic picture of the family”. Results. It has been established that the level of psychological health of family of a child with DM 1 corresponded to the normal indicator in 20 % of cases. The factors have been determined that destabilize psychological health of the family, typology of family relationships with a sick child and their role in the formation of persistent neuropsychiatric complications. The factors of the microsocial environment affecting the formation of socio­psychological maladjustment of a child with DM were determined. The following risk factors of the formation of mental and neurological disorders in DM 1 children have been identified: the age of endocrine disease (DM)onset less than 7 years, the disease duration ≥ 5 years, frequent fluctuations in of glycemialevel, unsatisfactory self-control of the disease, late diagnosis of early and late complications related to the central nervous system, insufficient compliance, pathological types of family sociogram. An alternative method for diagnosing psychological problems in children and adolescents with diabetes ­ the use of projective methods has shown a high information content of target detection for the correction of emotional and behavioral disorders in the conditions of system «Life with diabetes».Conclusions. Socio-psychological patterns of maladaptation of children and adolescents with type 1 diabetes mellitushave been identified, which is important for determining the strategy of therapeutic intervention, socio-psychological support and prevention of social handicap.

The prevalence of type 1 diabetes mellitus (T1DM) among children in Saudi Arabia is increasing with unfavorable outcomes. Therefore, in addition to pharmacotherapy, other measures should be studied regarding psychological aspects mainly among adolescents. The family, which acts as the primary caregiver at this age, may play a major role in disease management. Thus, this study is aimed at assessing the perception of adolescents about the behaviors of their families initially and at investigating the correlation between these behaviors and glycemic control. Up to our knowledge, there was no study in Saudi Arabia that addressed this issue previously. This cross-sectional observational study assessed adolescents aged 10–19 years diagnosed with T1DM on insulin and receiving follow-up care at the King Saud University Medical City in Riyadh, Saudi Arabia. Data were collected via telephone interview and the verified “Modified Diabetes Social Support Questionnaire-Family version”. Glycemic control was then assessed using the most recent hemoglobin A1c (HbA1c) level recorded in their electronic files. Fifty-six adolescents participated in this study with an equal sex distribution (each n=28). Almost all participants were Saudis, and the majority were living in Riyadh (n=41). The mean age was 16.1±2.41 years with a mean of 6.1±4.14 years history of diabetes. The mean HbA1c level was 9.6±2.12%. Participants perceived all behaviors as supportive with “support in critical situations” being the highest (77.3%) and the only factor significantly related to HbA1c (p=0.017). Age was significantly related to all factors (p>0.05). Family plays a major role in the management of diabetes. Their supportive behaviors are perceived by their family members diagnosed with T1DM, but there has been no optimal association with disease control. However, the involvement of the family can aid in decreasing possible complications of the disease by intervening in critical situations.

Mauriac syndrome is a severe complication of type 1 diabetes mellitus, manifested by growth retardation, impaired formation of secondary sexual characteristics, lack of libido, and hepatomegaly. Currently, the presented syndrome is quite rare. This is more related to the emergence of intensive insulin therapy, which keeps the level of daily glycemia within the tolerance of type 1 for diabetes mellitus. The article describes a clinical case of the development of Mauriac syndrome in a 21-year-old patient with decompensated diabetes mellitus and multiple complications. The patient was deprived of proper control over the condition of daily glycemia and full-fledged insulin replacement therapy for a long because of unfavorable social and material conditions in the family, which contributed to the development of chronic insulin insufficiency and complications.

Objetivo: apreender perspectivas e vivências do cuidador familiar sobre os cuidados prestados às crianças e adolescentes com Diabetes Mellitus tipo 1 (DM1) no domicílio. Método: estudo descritivo de abordagem qualitativa, realizada com 11 cuidadores residentes em um município no Noroeste do Paraná. Os dados foram coletados de fevereiro a maio de 2018, mediante entrevistas semiestruturadas, gravadas em áudio, e, então, submetidos à análise de conteúdo, modalidade temática, após aprovação do Comitê Permanente de Ética em Pesquisa com Seres Humanos. Resultados: foram identificadas duas categorias temáticas: “Dos primeiros sintomas à aceitação do diagnóstico: um processo sempre em curso”; e “As dificuldades que permeiam o cuidado no cotidiano”. Conclusão: as vivências relacionadas ao cuidado são permeadas por algumas dificuldades, especialmente em relação à aplicação correta da insulina, às restrições alimentares e à ausência de apoio por parte da escola, no controle alimentar e tratamento medicamentoso.ABSTRACTObjective: to learn family caregivers’ perspectives and experiences regarding home care given to children and adolescents with Type 1 Diabetes Mellitus (DM1). Results: two thematic categories were identified: “From first symptoms to acceptance of the diagnosis – an ongoing process” and “Difficulties permeating everyday care”. Conclusion: certain difficulties permeated the care-related experiences, especially as regards correct application of insulin, dietary restrictions, and lack of support from schools in dietary control and drug treatment.RESUMENObjetivo: conocer las perspectivas y experiencias de los cuidadores familiares con respecto al cuidado domiciliario de niños y adolescentes con Diabetes Mellitus Tipo 1 (DM1). Método: en este estudio cualitativo y descriptivo realizado - previa aprobación del Comité Permanente de Ética en la Investigación con Participantes Humanos - con 11 cuidadores residentes en un municipio del noroeste de Paraná, los datos fueron recolectados entre febrero y mayo de 2018 mediante entrevista grabada semiestructurada , y luego sometido a análisis de contenido temático. Resultados: se identificaron dos categorías temáticas: “Desde los primeros síntomas hasta la aceptación del diagnóstico - un proceso continuo” y “Dificultades que permean la atención diaria”. Conclusión: ciertas dificultades impregnaron las experiencias asistenciales, especialmente en cuanto a la correcta aplicación de la insulina, las restricciones dietéticas y la falta de apoyo de las escuelas en el control dietético y el tratamiento farmacológico.
