Objectives. To assess the extent to which new primary healthcare (PHC) models implemented in two regions of Quebec have improved patient experience of care, unmet needs, and use of services for individuals with and without chronic diseases, compared with other forms of PHC practices.Methods. In 2005 and 2010, we carried out population and organization surveys. We divided PHC organizations into new model practices and other practices and followed the evolution over time of patient experience of care.Results. Patients with chronic diseases had better accessibility but worse continuity of care in the new model practices than in the other practices at both time periods. Through the reform, accessibility decreased evenly in both groups, but continuity and perceived outcomes improved more in the other practices. Use of primary care services decreased more in the new model practices. Among patients without chronic disease, accessibility decreased much less in the new models and responsiveness increased more. There was no significant change in ER attendance and hospitalization.Conclusion. The evolution of patient experience of care has been more favorable for patients without chronic diseases. These findings raise concerns about equity since the aim of the PHC reform was targeting in priority individuals with the greatest needs.

Obstructive sleep apnea (OSA) and Alzheimer’s disease (AD) are two common chronic diseases with a well-documented association. Whether the association is causal has been highlighted by recent evidence reporting a neurobiological link between these disorders. This narrative review discusses the brain regions and networks involved in OSA as potential vulnerable areas for the development of AD neuropathology with a particular focus on gender-related implications. Using a neuroimaging perspective supported by neuropathological investigations, we provide a new model of neurodegeneration common to OSA and AD, that we have called OSA-AD neurodegeneration in order to decode the causal links between these two chronic conditions.

Stochastic dependence among cognitive processes can be modeled in different ways, and the family of multinomial processing tree models provides a flexible framework for analyzing stochastic dependence among discrete cognitive states. This article presents a multinomial model of multidimensional source recognition that specifies stochastic dependence by a parameter for the joint retrieval of multiple source attributes together with parameters for stochastically independent retrieval. The new model is equivalent to a previous multinomial model of multidimensional source memory for a subset of the parameter space. An empirical application illustrates the advantages of the new multinomial model of joint source recognition. The new model allows for a direct comparison of joint source retrieval across conditions, it avoids statistical problems due to inflated confidence intervals and does not imply a conceptual imbalance between source dimensions. Model selection criteria that take model complexity into account corroborate the new model of joint source recognition.
