Oral precancerous lesions and conditions like oral lichen planus, leukoplakia, or oral submucous fibrosis have the potential to develop into oral cancer. Oral cancer accounts for approximately 3 % of all malignancies. Oral lichen planus is a premalignant condition occurring in the oral cavity due to various factors. Blood investigations are routinely done minimally invasive procedures carried out for various diagnostic purposes. The study aims to identify if the complete blood count (CBC) can be used as a pathologic diagnostic marker in an oral premalignant disorder like oral lichen planus. A total of 64 patients (38 female, 26 male) were taken into the study between the period July 2019 - March 2020. Each patient is exposed to blood investigations, and their results are studied. The results of the study show a minimal variation in the blood results. In conclusion, to determine if it can be used as a pathological diagnostic marker in premalignant disorders like oral lichen planus, further studies must be carried out in larger populations.

ABSTRACT Introduction Oral squamous cell carcinoma (OSCC) is the most common malignant tumor of the oral cavity. It is preceded by various potentially malignant conditions like oral leukoplakia and oral lichen planus. Survivin is an inhibitor of apoptosis whose levels have been found increased in oral cancer as well as various potentially malignant conditions. Thus survivin can act as a biomarker and help in early detection of potentially malignant conditions which can prevent its transformation into malignancy. Aim To assess and compare the level of total human salivary survivin in oral leukoplakia, oral lichen planus, oral cancer, and control group. Materials and methods A total of 96 subjects were included in the study, which were further grouped into 24 in each group. The saliva was analyzed for survivin level among all the four groups. Survivin concentration (pg/mL) was studied in relation to clinical data. The results were analyzed using Mann—Whitney U test to derive the statistical difference. Results The average of levels of survivin in control group was 0.199 pg/mL, in oral leukoplakia group 0.312 pg/mL, in oral lichen planus group 0.380 pg/mL, and in oral cancer group 0.430 pg/mL. A comparison of all these groups revealed statistically significant difference among the groups. Conclusion Survivin may not be considered as an independent predictor of the malignant transformation for premalignant lesions but it can be an indicator for an increased risk of malignant transformation. Context: Molecular alterations in premalignant lesions of oral cavity are not well known, many reports and have found increased HER2 expression to be correlated with poor prognosis in oral cancer. However,literature on expression of HER2 in premalignant lesions is limited and data is conflicting in nature. Overexpression of HER2 in premalignant lesions may denote its positive contribution in malignant transformation of these lesions. Aims: To evaluate the expression of HER2 in premalignant lesions of oral cavity. Settings and Design: In this prospective observational study of 2 months, patients attending OPD at Department of ENT and meeting the inclusion criteria were included. Methods and Material: 23 samples of Leukoplakia and 1 sample of oral lichen planus were stained by routine H&E to confirm clinical diagnosis and assess dysplasia if any, 5 samples of normal mucosa were used as control. Immunohistochemical staining for HER2 was done. ASCO/CAP 2018 guidelines were used for reporting the results. Statistical analysis used: Percentage of lesions expressing cytoplasmic or membranous expression was calculated. Results: 1 sample of Leukoplakia with severe dysplasia expressed focal membranous staining. 20% leukoplakia lesions expressed cytoplasmic staining. Staining was not observed in oral lichen planus and leucoplakia without dysplasia. Conclusions: Membranous expression in Severe dysplasia and higher expression in oral cancer is in concordance with the multistep theory of carcinogenesis. Larger studies are needed if HER2 is to be proposed as a marker for oral premalignant lesions. Significance of cytoplasmic staining in oral premalignant lesions needs to be elucidated.
