A study of the clinical efficacy and safety of the drug Samelix (ademetionine, manufacturer – JSC «Canonfarma production», Russia) in 30 patients with chronic alcoholic liver disease (steatohepatitis mild to moderate currents, cirrhosis of the liver grade a ChildPugh) with the syndrome of cholestasis. Purpose of the study: evaluate the clinical efficacy and safety of Samelix (ademetionine) in 30 patients with chronic alcoholic liver disease. Objectives of the study: evaluate the effect of the drug on biochemical parameters, evaluate the effect of the drug on the clinical manifestation of the disease based on the individual diary data during the course therapy, assess the quality of life through the SF-36 questionnaire before and after treatment; report adverse events. The results of the study showed that therapy with this drug leads to a significant positive dynamics of biochemical parameters, regression of clinical manifestations of the disease, a significant increase in the quality of life. Good and excellent results of therapy were observed in 76.7% of cases. The drug is safe and well tolerated.

Alcohol abuse and the resulting alcoholic liver disease (ALD) are the leading cause of disease and death worldwide. The absence of symptoms of the disease at the early stages explains the diagnosis at the late stages. Better identification of life history and prognostic factors, as well as the search for reliable non-invasive techniques for detecting ALD, are required. The main therapy for patients with ALD, regardless of the stage of the disease, is long-term alcohol abstinence. In compensated patients, the end results are the normalization of abnormal laboratory tests and reduction of liver fibrosis. These end results can be controlled non-invasively, but modern medical interventions are largely limited and ineffective. Maintenance therapy is the basis of treatment, in which it is worth paying special attention to herbal preparations as less toxic.

Alcoholic liver disease (ALD) is a leading cause of liver-related morbidity and mortality. ALD encompasses a spectrum of disorders ranging from asymptomatic steatosis, alcoholic steatohepatitis, fibrosis, cirrhosis and its related complications. Moreover, patients can develop an acute-on-chronic form of liver failure called alcoholic hepatitis (AH). Most patients are diagnosed at advanced stages of the disease with higher rates of complications and mortality. The mainstream of therapy of ALD patients, regardless of the disease stage, is prolonged alcohol abstinence. The current therapeutic regimens for AH (i.e. prednisolone) have limited efficacy and targeted therapies are urgently needed. The development of such therapies requires translational studies in human samples and suitable animal models that reproduce clinical and histological features of AH. In recent years, new animal models that simulate some of the features of human AH have been developed, and translational studies using human samples have identified potential pathogenic factors and histological parameters that predict survival. In this review, we discuss the pathogenesis and management of ALD, focusing on AH, its current therapies and potential treatment targets.
