AbstractBackground:This study aimed to compare the view into the maxillary sinus using the posterior translacrimal approach compared with grade 3 antrostomy.Methods:Grade 3 antrostomy followed by a posterior translacrimal approach was performed on four cadavers. The maximum intramaxillary view was documented endoscopically guided by electromagnetic navigation. Representative screenshots were evaluated in a blinded manner by three independent sinus surgeons. In addition, a prospective investigation of specific complications in the post-operative course of consecutive patients was performed.Results:In the cadaver study, the posterior translacrimal approach provided a significantly better view into the maxillary sinus compared with grade 3 antrostomy. In the clinical study, only 1 out of 20 patients reported on a minor problem with lacrimal drainage at 6 months.Conclusion:The posterior translacrimal approach to visualising the maxillary sinus should be considered a strong alternative to more radical techniques.

Introduction.Posterior stabilization of the spine is a standard procedure in spinal surgery. In addition to the standard techniques, several new techniques have been developed. The objective of this cadaveric study was to examine the accuracy of a new electromagnetic navigation system for instrumentation of pedicle screws in the spine.Material and Method.Forty-eight pedicle screws were inserted in the thoracic spine of human cadavers using EMF navigation and instruments developed especially for electromagnetic navigation. The screw position was assessed postoperatively by a CT scan.Results.The screws were classified into 3 groups: grade 1 = ideal position; grade 2 = cortical penetration <2 mm; grade 3 = cortical penetration ≥2 mm. The initial evaluation of the system showed satisfied positioning for the thoracic spine; 37 of 48 screws (77.1%, 95% confidence interval [62.7%, 88%]) were classified as group 1 or 2.Discussion.The screw placement was satisfactory. The initial results show that there is room for improvement with some changes needed. The ease of use and short setup times should be pointed out. Instrumentation is achieved without restricting the operator’s mobility during navigation.Conclusion.The results indicate a good placement technique for pedicle screws. Big advantages are the easy handling of the system.

Guangxi Medical and Health Appropriate Technology Development and Application Project (No. S2019039), Guangxi, China. Background: Based on the results of recent studies, the PD-1 monoclonal antibodies have been approved to treat the patients with advanced hepatocellular carcinoma (HCC) by the FDA. Radiation therapy (RT) can enhance responsiveness to PD-1 monoclonal antibody by potential mechanisms. A phase Ⅱa study was conducted to assess the safety and the efficacy of combining RT with anti-PD-1 for patients with advanced hepatocellular carcinoma. Methods: Patients with advanced HCC were eligible. Stereotactic body radiation therapy (SBRT) were adopted, and the dose of radiation were Dt-PGTV 30-50 Gy/10fractions. Camrelizumab (200mg) were given intravenously every 3 weeks since the first day of RT until disease progression, or intolerable toxicity. Adverse events (AEs) and objective response rate (ORR) were summarized to assess the safety and efficacy. Results: From April 2020 to November 2020, 17 patients were enrolled (median age 54, range 32-69). 15 (88%) patients were male. 14 (82%) had ECOG performance score of 0. All the patients had Child-Pugh score A. 16 patients staged as Barcelona Clinic Liver Cancer staging C or China Liver Cancer staging Ⅲ. Extrahepatic metastases were identified in 11 (65%) patients. 13 (77%) patients were Hepatitis B virus infected. 15 (88%) patients had previously 2 lines or more chemotherapy. 9 (53%) patients had Alpha-fetoprotein level≥400 ng/ml. The ORR was 47%. The best response assessed by RECIST 1.1 was partial response (8 patients). Four patients had grade 3 immune-related adverse events (irAEs), including increased aspartate aminotransferase and alanine transaminase (n =1),decreased hemoglobin (n =1),decreased platelet count (n =1),decreased neutrophil count (n =1). All grade 3 irAEs were mitigated with proper treatment. None treatment-related deaths occurred. Conclusions: In this study, RT combined with anti-PD-1 had an acceptable safety profile and indicated an effective treatment option in patients with unresectable HCC. Chapter 3 aims to gauge both the reality of, as well as public opinion on, the central issue of corruption. It investigates public opinion on corruption among elected officials, source of corruption, effectiveness of laws and regulations in mitigating corruption, support for campaign finance reforms, etc. The data strongly suggest that people think corruption is rampant despite limited evidence that quid pro quo corruption is anything more than a minor problem. This fundamental attitude has not changed much in the wake of the Citizens United decision. Furthermore, they believe the problem is mostly intractable and that most of the commonly proposed reforms of the campaign finance system will not work. Nevertheless, they still support these reforms. Moving from simple descriptive data to more associational analyses, this chapter also explores the effect of campaign finance laws on campaign spending and then the effect of both on corruption attitudes. The results are not what the Court would have expected.

E. H. Gombrich's Adoption of the Formula form Follows Function: A Case of Mistaken Identity?This article is a longer note on what is a minor problem in the oeuvre of a great art historian. Its theme is E. H. Gombrich's use of the formula form follows function as the summary of his commonsense approach to the problem of style change. Although I am not sure how interesting this inquiry is in an art historical context, from the perspective of my own field of design history and of modernist design theory, Gombrich's adoption of the formula constitutes an intriguing problem.

Abstract A modified method and key for the identification of canned salmon species have been subjected to an additional collaborative study. Of 54 samples examined, 11 collaborators accurately identified 10 of 10 samples (100%) of sockeye, 11 of 11 samples (100%) of chum, 11 of 11 samples (100%) of pink, 11 of 11 samples (100%) of coho, and 11 of 11 samples (100%) of chinook. Nine of 11 collaborators were able to find in all samples usable scales with intact clear areas for mounting and identification. Two of the 11 had a minor problem. The improved, modified method and key have been adopted as official first action for the identification of the sockeye, chum, pink, coho, and chinook salmon species to replace method 18.089.
