The Technology Assessment and Practice Guidelines Forum: A Modified Group Judgment Method

AbstractThis article describes a new group-judgment process, modified from the consensus development approach of the U.S. National Institutes of Health. It is called the “forum” method, a shorthand term for “technology assessment and practice guidelines forum.” This approach is aimed at (a) describing the current state of knowledge of the technology under study; (b) developing practice guidelines for the use of the technology; and (c) providing information for insurers, consumers, policymakers, and others. A pilot study was conducted focusing on total parenteral nutrition. The validity of the process and of the recommendations and their acceptability were evaluated in a survey of participants and a selected group of nonparticipants. Based on the survey results, the forum method appears to be a valuable method for assessing technologies, defining their clinical role, and developing practice guidelines.

OBJECTIVETo increase the number of independent National Institutes of Health (NIH)–funded neurosurgeons and to enhance neurosurgery research, the National Institute of Neurological Disorders and Stroke (NINDS) developed two national comprehensive programs (R25 [established 2009] for residents/fellows and K12 [2013] for early-career neurosurgical faculty) in consultation with neurosurgical leaders and academic departments to support in-training and early-career neurosurgeons. The authors assessed the effectiveness of these NINDS-initiated programs to increase the number of independent NIH-funded neurosurgeon-scientists and grow NIH neurosurgery research funding.METHODSNIH funding data for faculty and clinical department funding were derived from the NIH, academic departments, and Blue Ridge Institute of Medical Research databases from 2006 to 2019.RESULTSBetween 2009 and 2019, the NINDS R25 funded 87 neurosurgical residents. Fifty-three (61%) have completed the award and training, and 39 (74%) are in academic practice. Compared to neurosurgeons who did not receive R25 funding, R25 awardees were twice as successful (64% vs 31%) in obtaining K-series awards and received the K-series award in a significantly shorter period of time after training (25.2 ± 10.1 months vs 53.9 ± 23.0 months; p < 0.004). Between 2013 and 2019, the NINDS K12 has supported 19 neurosurgeons. Thirteen (68%) have finished their K12 support and all (100%) have applied for federal funding. Eleven (85%) have obtained major individual NIH grant support. Since the establishment of these two programs, the number of unique neurosurgeons supported by either individual (R01 or DP-series) or collaborative (U- or P-series) NIH grants increased from 36 to 82 (a 2.3-fold increase). Overall, NIH funding to clinical neurological surgery departments between 2006 and 2019 increased from $66.9 million to $157.3 million (a 2.2-fold increase).CONCLUSIONSTargeted research education and career development programs initiated by the NINDS led to a rapid and dramatic increase in the number of NIH-funded neurosurgeon-scientists and total NIH neurosurgery department funding.

Purpose Assessment activities in academic libraries continue to grow as libraries explore assessment endeavors. Ranging from basic stats gathering and reporting to surveys, focus groups, and usability studies and beyond. Many practitioners are finding it necessary to create new processes and programs, with little guidance. The purpose of this paper is to paint a broad picture of assessment activities in Association of Research Libraries (ARL) university libraries with the goal of creating a resource for libraries developing or improving their assessment programs. Design/methodology/approach A survey was developed that asked questions about assessment personnel, activities, mission, and website. A total of 113 surveys were sent to academic library members of ARL. Survey results were analyzed to compile a list of recommended good practices for assessment and working with assessment committees in academic libraries. Findings The investigators had a response rate of 43 percent. The open-ended nature of the survey questions allowed for the respondents to specifically narrow down the problems and opportunities inherent in library assessment committees. Originality/value This study takes the temperature of the current state of assessment programs in ARL libraries, demonstrating the growth of assessment programs. It begins to document the practices of these libraries, particularly in regards to the sometimes informal and hard to track use of committees and other in-house collaborations, as a first step toward developing best practices for the field. The results illuminate productive areas for further study, including investigating how to measure a culture of assessment and maximizing impact of assessment information presented on assessment websites.

Abstract Background Second medical opinions (SOs) can assist patients in making informed treatment decisions and improve the understanding of their diagnosis. In Germany, there are different approaches to obtain a structured SO procedure: SO programs by health insurers and SOs according to the SO Directive. Through a direct survey of the population, we aimed to assess how structured SOs should be provided to fulfil patients’ needs. Methods A stratified sample of 9990 adults (≥18 years) living in the federal states of Berlin and Brandenburg (Germany) were initially contacted by post in April and sent a reminder in May 2020. The survey results were analyzed descriptively. Results Among 1349 participants (response rate 14%), 56% were female and the median age was 58 years (interquartile range (IQR) 44–69). Participants wanted to be informed directly and personally about the possibility of obtaining an SO (89%; 1201/1349). They preferred to be informed by their physician (93%; 1249/1349). A majority of participants would consider it important to obtain an SO for oncological indications (78%; 1049/1349). Only a subset of the participants would seek an SO via their health insurer or via an online portal (43%; 577/1349 and 16%; 221/1349). A personally delivered SO was the preferred route of SO delivery, as 97% (1305/1349) would (tend to) consider this way of obtaining an SO. Participants were asked to imagine having moderate knee pain for years, resulting in a treatment recommendation for knee joint replacement. They were requested to rate potential qualification criteria for a physician providing the SO. The criteria rated to be most important were experience with the recommended diagnosis/treatment (criterion (very) important for 93%; 1257/1349) and knowledge of the current state of research (criterion (very) important for 86%; 1158/1349). Participants were willing to travel 60 min (median; IQR 60–120) and wait 4 weeks (median; IQR 2–4) for their SO in the hypothetical case of knee pain. Conclusion In general, SOs were viewed positively. We found that participants have clear preferences regarding SOs. We propose that these preferences should be taken into account in the future design and development of SO programs.

“Prevention is better than cure” is a proverb commonly used to highlight how it is better to avoid a problem from the start rather than fixing the damage afterwards. When considering this concept to our approach to the environment, we encounter two schools of thought; Ecocentrism and Technocentrism. Ecocentrism places the ecosystem at the center and stress that we need to limit our natural resource exploitation, so that we can conserve the environment and prevent its destruction. Technocentrism, on the other hand, focuses on technology and science as a way to repair any damage done to the environment rather than changing ethical perspectives on environmental issues. This paper seeks to explore the two ideologies by comparing between Finland and Bhutan; two countries that seem to resemble the two approaches. Finland is considered one of the highly technologically advanced countries, in addition to its reputation for environmental protection. Bhutan is a Himalayan Kingdom that is known for its strong environment conservation and their intriguing approach to sustainable development. The paper compares between the two countries sustainable development approach, environment, and health. It was concluded that, in terms of the environment; Bhutan is doing much better than Finland. Finland’s carbon emissions reduction was mainly because they produce goods in other countries which increase the other countries emissions not Finland. Additionally, high consumption levels in Finland are a major threat to the environment. As for health, there is no extreme difference; however, Bhutan’s health care system is still improving. Yet, the two countries are facing serious issues regarding mental health. Ultimately, the ecocentric approach appears to be much better; especially, in this day and age as many downsides of technology keep unfolding.

In assembling a CASE environment from a collection of commercial off-theshelf (COTS) tools, tool users must find ways to connect the tools such that they provide adequate support for their particular software development approach. This task takes place in the context of limited knowledge of the tools, limited access to the source or internal structures of the tools, limited resources with which to perform and maintain the tool connections, and evolving understanding of the needs of the tool users. This places severe restrictions on what can be attempted in terms of tool interconnection. Environment framework technologies (e.g., ECMA PCTE, ATIS, BMS, ToolTalk, or CORBA) claim to provide a set of common integration services that aid in the tool integration process. There have been numerous discussions about the value, maturity, and complexity of these framework technologies. Such discussions are characterized by three points: general agreement that framework technology per se is a valuable goal to pursue; moderate disagreement as to whether the current level of framework technology is a sufficient basis for production quality CASE environments; and considerable disagreement about which of the current technologies are the most likely to mature and succeed. Notable about these discussions, however, is that there has not been extensive use of the technologies in question. This stems from several sources: their expense, their unfamiliarity and complexity to current tool users, and a widespread concern about their immaturity. This lack of use is perhaps understandable, but it has had the additional effect that partisans of one or another technology have made assertions based on little factual information about the relative merits of the technology in question. To expand our own expertise in tool integration and framework technologies, and to answer the question, “What tool integrations are possible for third-party tool users given the current state of COTS tools and integration technology?” we performed a set of experiments involving the integration of a collection of common COTS tools with environment framework technologies in support of a typical development scenario. Our selection of these tools and technologies was based on common availability and application to the development scenario(s) of interest.

Objectives: To obtain further information from members of the International Network of Agencies for Health Technology Assessment (INAHTA) on the involvement of consumers in their programs.Methods: A questionnaire for a survey was developed and sent to member agencies in November 2010. Survey responses were compared with those from an earlier survey conducted in 2005.Results: Of the thirty-three agencies that provided responses, 67 percent involve consumers in some aspects of their health technology assessment (HTA) programs, compared with 57 percent in 2005. As in the earlier survey, most agencies reporting involvement have contact with consumer or patient organizations and a large minority also involve individual consumers. Summaries of HTA reports that are intended to be easily understood by consumers are prepared by 84 percent of the agencies, and 42 percent involve consumers in dissemination of HTA material. In both areas, there was some increase from the levels previously reported.Conclusions: The survey results suggest that there is a trend to increased involvement of consumers by the INAHTA agencies in their programs but that the level of involvement remains relatively limited. The manner of consumer participation varies between agencies.
