Preparedness of Primary Healthcare Centers for Critical Emergency Situations in Southwest Turkey

AbstractIntroduction:Primary healthcare centers (PHCCs) frequently are contacted for emergency reasons and are expected to provide basic and advanced life support during emergency situations.Objectives:The aim of this study was to assess the availability of emergency equipment and the knowledge of the staff working in PHCCs.Methods:The survey was conducted in 21 PHCCs located in the rural city of Isparta, Turkey, in 2001. The availability of emergency equipment, emergency drugs, intravenous parenteral solutions, and diagnostic-therapeutic equipment was evaluated. Knowledge of basic life support of the staff (n = 195) was evaluated using a 10-item test.Results:Two (9.5%) PHCs had a complete emergency kit with an airways bag, mask, intravenous parenteral solutions, emergency drugs, and other diagnostic equipment. Emergency equipment was easily accessible in 19 PHCCs (90.5%), while in the remaining centers, the equipment and drugs were stored in locked cabinets.The staff that was evaluated consisted of 43 doctors (22%), 132 nurses and midwifes (67%), and 20 health officers (11%). Doctors scored the highest knowledge score (65.5%), followed by nurses and midwives (58.6% and 56.7%, respectively), and health officers (52.5%).Conclusions:Primary healthcare centers were not prepared to provide advanced life support. Knowledge scores were low and the staff was in need of basic life support training. Further arrangements must be stressed to make PHCC's “emergency-friendly centers” in Isparta, Turkey.

Aim: To assess the knowledge and attitude of students of dental schools in Southern India towards Basic Life Support (BLS), as well as to study the retention of BLS knowledge over time. Subjects and methods: A prevalidated questionnaire was employed, which contains 12 knowledge Questions and five questions designed to assess the students perspective towards BLS. It was hosted on Google Forms, and the URL was distributed to the Students. Results: A total of 267 responses were received, of which 66 were males and 201 were female students. Out of a maximum possible score of 12, the mean knowledge score of the group was 4.54. It was found that those who attended a BLS workshop within the last 2 years scored significantly better than those who either took it more than 2 years ago or had never undergone BLS training. Almost all participants (96%) agreed that there is a need for BLS to be learned by all dental students and hence BLS training should be part of the dental curriculum. 76.8% of respondents expressed reluctance to perform CPR on strangers. Conclusion: The findings show that the dental students who participated in the study had inadequate knowledge in BLS, but had a positive attitude towards the adoption of BLS into the dental curriculum.

Мета роботи – оптимізувати викладання та розподіл алгоритмів виконання практичних навичок лікарями медицини невідкладних станів на догоспітальному та ранньому госпітальному етапах згідно з сучасними стандартами надання невідкладної медичної допомоги. Основна частина. Для реалізації та адекватного інструментального забезпечення демонстрації та наступного відпрацювання практичних навичок на кафедрі наявні спеціалізовані манекени та обладнання, які дозволяють у повному об’ємі виконувати всі маніпуляції, передбачені алгоритмами надання невідкладної медичної допомоги. Важливим аспектом, завдяки якому кафедра наслідує сучасним міжнародним стандартам надання допомоги, є розподіл викладання практичних навичок надання невідкладної допомоги з урахуванням загальноприйнятих в усьому світі понять BLS (basic life support) та ALS (advanced life support). Висновок. Оптимізація методики викладання практичних навичок для надання невідкладної медичної допомоги відповідно до сучасних стандартів лікування невідкладних станів збільшує якість надання допомоги та дозволяє зменшити втрати часу на догоспітальному етапі, що здатне значною мірою підвищити виживання та відсоток позитивного прогнозу для пацієнтів з невідкладною медичною патологією.

Resuscitation on the playing field is at least as important as screening in the prevention of death. Even if a screening strategy is largely effective, individuals will suffer sudden cardiac arrests. Timely recognition of a cardiac arrest with rapid implementation of cardiopulmonary resuscitation (CPR) and deployment and use of automated external defibrillators (AEDs) will save lives. Basic life support, including CPR and AED use, should be a requirement for all those involved in sports, including athletes. An emergency action plan is important in order to render advanced cardiac life support and arrange for transport to medical centres.
