Over a 4-year period, 218 mallet fractures in 211 adult patients were treated using a custom-made thermoplastic splint. Clinical results were collected prospectively, including the visual analogue score for pain, the range of motion and extensor lag, and the Patient Evaluation Measure (PEM). The joints were congruent in 168 and subluxed in 50. There were no differences in range of movement, extensor lag or PEM associated with articular subluxation or the size of the articular fragment. Pre-existing joint degeneration did not influence outcome. Non-surgical treatment demonstrates predictably good outcomes regardless of fragment size or subluxation in most patients and should be considered when discussing treatment for patients with bony mallet fractures. Experts from different domains have resorted to machine learning techniques to produce explainable models that support decision-making. Among existing techniques, decision trees have been useful in many application domains for classification. Decision trees can make decisions in a language that is closer to that of the experts. Many researchers have attempted to create better decision tree models by improving the components of the induction algorithm. One of the main components that have been studied and improved is the evaluation measure for candidate splits. In this article, we introduce a tutorial that explains decision tree induction. Then, we present an experimental framework to assess the performance of 21 evaluation measures that produce different C4.5 variants considering 110 databases, two performance measures, and 10× 10-fold cross-validation. Furthermore, we compare and rank the evaluation measures by using a Bayesian statistical analysis. From our experimental results, we present the first two performance rankings in the literature of C4.5 variants. Moreover, we organize the evaluation measures into two groups according to their performance. Finally, we introduce meta-models that automatically determine the group of evaluation measures to produce a C4.5 variant for a new database and some further opportunities for decision tree models.

Purpose – The purpose of this paper is to investigate a lean manufacturing metric called Takt time as a benchmark evaluation measure to evaluate a public hospital’s service quality. Lean manufacturing is an established managerial philosophy with a proven track record in industry. A lean metric called Takt time is applied as a measure to compare the relative efficiency between two emergency departments (EDs) belonging to the same public hospital. Outcomes guide managers to improve patient services and increase hospital performances. Design/methodology/approach – The patient treatment lead time within the hospital’s two EDs (one department serves male and the other female patients) are the study’s focus. A lean metric called Takt time is used to find the service’s relative efficiency. Findings – Findings show that the lean manufacturing metric called Takt time can be used as an effective way to measure service efficiency by analyzing relative efficiency and identifies bottlenecks in different departments providing the same services. Originality/value – The paper presents a new procedure to compare relative efficiency between two EDs. It can be applied to any healthcare facility.

Background Implant arthroplasties for trapeziometacarpal osteoarthritis are often associated with high complication and revision surgery rates. There are no previous studies reporting revision outcomes of failed interposition implant arthroplasty. Methods A patient register search was done for all implant arthroplasties performed for trapeziometacarpal osteoarthritis during a 10-year period in a single hand surgical unit. Altogether, 32 patients had primary interposition implant arthroplasty (Artelon 22, Pyrosphere 6, Ortosphere 2, and Pyrodisk 2), and 19 of these patients had revision surgery with 23 revision procedures performed. In all, 15 of the revised 19 patients were reexamined clinically (Connolly-Rath score, Quick Disabilities of the Arm Shoulder and Hand, patient evaluation measure, the visual analog score for pain, thumb range of motion and strength measurements) and radiographically. Results The indication for revision surgery was pain alone or implant dislocation accompanied by pain in all cases. Thirteen of the revised 15 patients reported functional deficit and pain after revision. There was no statistically significant difference in the revision outcomes between patients operated on primarily with the Artelon implant versus pyrocarbon/ceramic implants. Compared to previous studies on revision surgery and primary trapeziometacarpal arthroplasty, our results showed slightly higher pain and poorer functional scores. Conclusions Interposition implant arthroplasty may yield high revision rates. The results after revision surgery may be worse than previously described, and there may also be a tendency for worse results than those of primary arthroplasty. Interposition implant arthroplasty should always be thoroughly contemplated.

Plane deicing is mandatory to insure safe plane take-off. Previous human factors studies have shown that open-basket deicing activity can be improved. The objective of the paper is to compare heart rate assessment models within a field study with numerous influencing variables and small sample size as well as to deepen our understanding of the most demanding openbasket tasks using cardiac output. A field study in a Canadian centered plane deicing facility was conducted in 2016-2017. 12 participants contributed to a thorough description and analysis of open-basket deicing activities. Respiratory and cardiac output of these participants was collected using Hexoskin vests. Working heart rate, heart rate reserves as well as calculations of absolute cardiac cost were done. Working heart rate (WHR), Heart Rate Reserve (HRR) and Absolute Cardiac Cost (ACC) do not behave uniformly for the majority of participants. In field studies with a large number of influencing variables on the heart rate, it is usually not sufficient to consider one single evaluation measure like WHR. In the interest of protecting employees, it seems to make sense to use the more cautious measures HRR or ACC as parameters instead of WHR. Superimposed activities (e.g. forced postures and dynamic use of upper body) have a significant effect on heart rate increases. In 8 out of 11 cases we have fatigue-related increases in heart rate over the observation period. Similar studies need to be conducted in other aircraft deicing facilities.

It has been repeatedly suggested in the literature on generative phonology that the number of features which appear in phonological rules be used in an evaluation measure for alternative descriptions of the phonology of a language; thus Halle states (1962: 55): ‘Given two alternative descriptions of a particular body of data, the description containing fewer… symbols will be regarded as simpler and will, therefore, be preferred over the other.’ The purpose of the present paper is to examine critically the application of the proposed feature-counting evaluation measure to alternative analyses of a very restricted body of data in the phonology of Turkish. The data considered are, as far as I can see, self-contained in that the analyses to be discussed do not have any implications elsewhere in the system of phonological rules; the adequacy of the decision based on the evaluation measure in question can therefore be fully assessed in connexion with the minor problem we shall be examining.
