ABSTRACTParkinson’s disease is associated with a variety of neurotransmitter disturbances which may be further altered by its treatment with dopamine agonists. Based on this information a wide range of pharmacological approaches have been used in search of newer treatment alternatives and in hopes of reducing complications of long-term levodopa use. This paper reviews the various therapies which have had some success in the management of Parkinson’s disease, other than levodopa and dopamine agonists. Special emphasis is placed on the many unresolved questions and controversies that exist in this area of neuropharmacology.

AbstractLiterature documents the impact of Parkinson’s Disease (PD) on speech but no study has analyzed in detail the importance of the distinct phonemic groups for the automatic identification of the disease. This study presents new approaches that are evaluated in three different corpora containing speakers suffering from PD with two main objectives: to investigate the influence of the different phonemic groups in the detection of PD and to propose more accurate detection schemes employing speech. The proposed methodology uses GMM-UBM classifiers combined with a technique introduced in this paper called phonemic grouping, that permits observation of the differences in accuracy depending on the manner of articulation. Cross-validation results reach accuracies between 85% and 94% with AUC ranging from 0.91 to 0.98, while cross-corpora trials yield accuracies between 75% and 82% with AUC between 0.84 and 0.95, depending on the corpus. This is the first work analyzing the generalization properties of the proposed approaches employing cross-corpora trials and reaching high accuracies. Among the different phonemic groups, results suggest that plosives, vowels and fricatives are the most relevant acoustic segments for the detection of PD with the proposed schemes. In addition, the use of text-dependent utterances leads to more consistent and accurate models.

AbstractParkinson’s disease (PD) is diagnosed when patients exhibit bradykinesia with tremor and/or rigidity, and when these symptoms respond to dopaminergic medications. Yet in the last years there was a greater recognition of additional aspects of the disease including non-motor symptoms and prodromal states with associated pathology in various regions of the nervous system. In this review we discuss current concepts of two major alterations found during the course of the disease: cytoplasmic aggregates of the protein α-synuclein and the degeneration of dopaminergic neurons. We provide an overview of new approaches in this field based on current concepts and latest literature. In many areas, translational research on PD has advanced the understanding of the disease but there is still a need for more effective therapeutic options based on the insights into the basic biological phenomena.
