Speech Quality Measurement and the Management of Stuttering

This paper reviews the various approaches that have been made toward the investigation of speech quality in stuttering treatment. The review takes into account the findings of relevant perceptual and acoustic investigations in the area of normal communication skills and voice disorders. Similar consideration is given to investigations on the stutter-free speech of stutterers where the contribution and relevance of this research to the search for a reliable and viable measure of speech quality is discussed. The review concludes with an overview of some promising findings from recent studies on the use of listener ratings of speech naturalness in the measurement and modification of untreated and treated stutterers. Some avenues for research and issues that have emerged from these studies are also discussed.

It is well known that unusual speech quality may result from stuttering treatments that are based on prolonged speech. However, empirical information concerning the speech quality associated with those treatments is lacking. The present study was designed to contribute such empirical information. Results indicated that speech quality assessments of posttreatment clients, using Martin, Haroldson, and Triden's (1984) speech naturalness scale, gave similar results regardless of whether they were based on monologues or conversations. The speech quality of those clients remained stable at the conclusion of their treatment program. Further, there was a significant, positive correlation between pretreatment speech measures and measures of speech naturalness made after the establishment of stutter-free speech. The subjects whose pretreatment stuttering was the most severe had posttreatment speech naturalness scores that were more than two scale values worse than the subjects whose pretreatment stuttering was the least severe. Speech naturalness scale scores are presented for nonstutterers and posttreatment stutterers and these data are compared with existing findings.

We investigated how standard speech coders, currently used in modern communication systems, affect the quality of the speech of persons who have common speech and voice disorders. Three standardized speech coders (GSM 6.10 RPELTP, FS1016 CELP, and FS1015 LPC) and two speech coders based on subband processing were evaluated for their performance. Coder effects were assessed by measuring the quality of speech samples both before and after processing by the speech coders. Speech quality was rated by 10 listeners with normal hearing on 28 different scales representing pitch and loudness changes, speech rate, laryngeal and resonatory dysfunction, and coder-induced distortions. Results showed that (a) nine scale items were consistently and reliably rated by the listeners; (b) all coders degraded speech quality on these nine scales, with the GSM and CELP coders providing the better quality speech; and (c) interactions between coders and individual voices did occur on several voice quality scales.

It is possible to implement help systems for diagnosis oriented to the evaluation of the fonator system using speech signal, by means of techniques based on expert systems. The application of these techniques allows the early detection of alterations in the fonator system or the temporary evaluation of patients with certain treatment, to mention some examples. The procedure of measuring the voice quality of a speaker from a digital recording consists of quantifying different acoustic characteristics of speech, which makes it possible to compare it with certain reference patterns, identified previously by a “clinical expert”. A speech acoustic quality measurement based on an auditory assessment is very hard to assess as a comparative reference amongst different voices and different human experts carrying out the assessment or evaluation. In the current bibliography, some attempts have been made to obtain objective measures of speech quality by means of multidimensional clinical measurements based on auditory methods. Well-known examples are: GRBAS scale from Japon (Hirano, M.,1981) and its extension developed and applied in Europe (Dejonckere, P. H. Remacle, M. Fresnel-Elbaz, E. Woisard, V. Crevier- Buchman, L. Millet, B.,1996), a set of perceptual and acoustic characteristics in Sweden (Hammarberg, B. & Gauffin, J., 1995), a set of phonetics characteristics with added information about the excitement of the vocal tract. The aim of these (quality speech measurements) procedures is to obtain an objective measurement from a subjective evaluation. There exist different works in which objective measurements of speech quality obtained from a recording are proposed (Alonso J. B.,2006), (Boyanov, B & Hadjitodorov, S., 1997),(Hansen, J.H.L., Gavidia-Ceballos, L. & Kaiser, J.F., 1998),(Stefan Hadjitodorov & Petar Mitev, 2002),(Michaelis D.; Frohlich M. & Strube H. W. ,1998),(Boyanov B., Doskov D., Mitev P., Hadjitodorov S. & Teston B.,2000),(Godino-Llorente, J.I.; Aguilera-Navarro, S. & Gomez-Vilda, P. , 2000). In these works a voiced sustained sound (usually a vowel) is recorded and then used to compute speech quality measurements. The utilization of a voiced sustained sound is due to the fact that during the production of this kind of sound, the speech system uses almost all its mechanisms (glottal flow of constant air, vocal folds vibration in a continuous way, …), enabling us to detect any anomaly in these mechanisms. In these works different sets of measurements are suggested in order to quantify speech quality objectively. In all these works one important fact is revealed; it is necessary to obtain different measurements of the speech signal in order to compile the different aspects of acoustic characteristics of the speech signal.

This study investigated the modification of speech naturalness during stuttering treatment. It systematically replicated an earlier study (Ingham & Onslow, 1985) that demonstrated that unnatural-sounding stutter-free speech could be shaped into more natural-sounding stutter-free speech by using regular feedback of speech-naturalness ratings during speaking tasks. In the present study, the same procedure was used with three persons who stutter—2 adolescent girls and 1 adult man—during rhythmic stimulation conditions. The two adolescent participants spoke only English, but Spanish was the first and English the second language (ESL) of the adult participant. For the 2 adolescents, it was demonstrated that their unnatural-sounding rhythmic speech could be shaped to levels found among normally fluent speakers without losing the fluency-inducing benefits of rhythmic speech. The findings indicate that speech-naturalness feedback may be a powerful procedure for overcoming a problematic aspect of rhythmic speech treatments of stuttering. However, it was not possible to deliver reliable speech-naturalness feedback to the adult ESL speaker, who also displayed a strong dialect. The study highlights the need to find strategies to improve interjudge agreement when using speech-naturalness ratings with speakers who display a strong dialect.
