While the prior objective of hydraulic structures is to remain in service, engineers must also be capable to adequately handle their failures. This paper gives guidance on this issue based on the European (mainly Dutch) and American practice. The discussion begins with terminology agreements on “accident” and “calamity”, followed by the notes on risk analysis and life safety risks in particular. Identifying and reducing such risks is an issue of much broader scope than structural analysis. Once an accident happens, proper investigations, handling the losses and planning the repair become major goals. The paper discusses the choice between repair and replacement of a gate, illustrated by examples from the USA and the Netherlands The discussion has an engineering rather than statistical character. The authors believe that accidents are events of infrequent nature and very diverse consequences, difficult to capture in databases.

<p>Hydraulic closures in dams, navigation locks and flood barriers belong to the most heavily loaded structures built by people. While ensuring their sufficient strength is the main engineers’ concern, one must also be prepared to adequately handle their failures. Identifying and reducing the risks of failures is an issue of wider scope than structural analysis alone. Once an accident happens, proper investigations, handling the losses and planning the repair become primary goals. This paper gives a general guidance on these issues reflecting the European (mainly Dutch) and American practice. The discussion includes both handling the situations immediately after the accidents, and the choice between repair and replacement of a damaged structure. Accidents are infrequent events of very diverse causes and consequences, therefore this discussion has an engineering rather than statistical character.</p><p>Both authors contributed to resolving accidents and failures of hydraulic structures, in the roles varying from investigation or design leader to repair manager. They were also consulted or made part of crisis teams in a number of other so-called “upset events”. This paper combines the highlights of their own experience and the practices being followed by the waterway administrations in the USA and the Netherlands. The selected examples are also from these countries, but can be seen as reflecting issues and concerns anywhere.</p>

Risk analysis in the water utility sector is fast becoming explicit. Here, we describe application of a capability model to benchmark the risk analysis maturity of a sub-sample of eight water utilities from the USA, the UK and Australia. Our analysis codifies risk analysis practice and offers practical guidance as to how utilities may more effectively employ their portfolio of risk analysis techniques for optimal, credible, and defensible decision making.

SUMMARYWe present the frequency and the nature of contact incidents of the Serotine bat,Eptesicus serotinus, with humans and with companion animals (specifically cats and dogs), in The Netherlands between 2000 and 2005. Out of 17 bats in bite contact with humans, five tested positive for European bat lyssavirus (EBLV) type 1a. Cats had the most numerous contacts with bats (49 times) but a relatively low number of these bats were EBLV positive (six times). We estimated that the average incidence of human bat rabies infection might be between once per year and once per 700 years, depending mainly on the number of infectious viral particles in bat saliva. The risk of bat rabies is higher between April and October, and in the northern half of the country. This is the first study in Europe describing the risk of human bat rabies after bat contact incidents.

This article discusses the basic safety questions to contemplate in a use-related risk analysis pertaining to the out-of-the-box experience, and focuses on how the most critical safety risks can be reduced or eliminated by conducting a usability study related to the out-of-the-box experience.

In many countries, including the UK, proposals are currently being made for the extension of legal deposit to electronic and other non-print material. Some countries such as Switzerland and the Netherlands have no national legal deposit legislation, though voluntary deposit works well in the latter. Norway has the most advanced legislation, requiring the deposit of all lands of media. In few countries is any range of material actively handled, and a very few deal with online publications. There is scope for international coordination of proposals through such bodies as CDNL, CENL, IFLA and UNESCO. The aim of totally comprehensive collecting of all published material may be accepted as unrealistic, and some selectively is likely to be necessary. The current situation with regard to deposit of non-print material in 11 west European countries, Australia, Canada and the USA is recounted.
