AbstractWhen all the remaining natural teeth are removed, there exists a space within the oral cavity that can be called the potential denture space. Within this denture space there is an area that has been termed the neutral zone. Neutral zone is defined as that area or position within the oral cavity where the forces between the tongue and cheeks or lips are equal. Thus making it the zone of minimal conflict, where the function of musculature will not unseat the denture and help in stabilizing the denture. In the present case the neutral zone was recorded using admix technique and impression compound as the material of choice.

The present prime concern in dentistry is on preservation of remaining natural teeth. Presence of few teeth in oral cavity help in preserving alveolar ridge integrity, maintain the proprioception, and gives psychological benefit to the patient. Overdenture has proven to be the mainstay of conservative prosthodontic treatment when proper diagnosis, treatment planning, and most importantly, patient compliance are achieved. This paper presents a case report of rehabilitation of a 65 year old patient with multiple missing teeth with tooth supported overdenture in both maxilla and mandible.

Patients with resorbed mandibular ridge poses a challenge for prosthodontic rehabilitation. The ongoing diminution of denture foundation defies the basic mechanism of denture stability and support. The technique of neutral zone to construct a denture that is shaped by muscle function and is in harmony with the surrounding oral structures can remarkably improve prognosis of such patients. The neutral zone is the area in the oral cavity where during function; the forces of the cheeks and lips pressing inward neutralize the forces of the tongue pressing outward. Thus physiologically optimal teeth arrangement and harmonious denture contours and can be achieved to maximize prosthesis stability, comfort, and function for patients. This clinical report describes rehabilitation of a patient with severely resorbed mandibular ridge using neutral zone impression technique.

Cu-Sil denture is a simplified approach which helps in preserving the few remaining natural teeth .It is a new type of transitional denture which clasps the neck of the tooth providing space for the remaining teeth to emerge into the oral cavity through the denture. Preserving the remaining teeth not only preserves the alveolar ridge but also maintains proprioception providing psychological benefit for the patient. Cu-Sil denture is one such simpler clinical approach which offers many advantages. This case report represents a simple chair-side clinical approach to fabricate Cu-Sil denture by preserving the few natural teeth. Key words: Cu-Sil denture, transitional denture, proprioception.

Nowadays one of the prime concern in dentistry is on preservation of remaining natural teeth. Many patients require replacement of missing teeth and associated structures to enhance appearance, improve masticatory efficiency, prevent unwanted movement of teeth (overeruption/ drifting), and/ or improve phonetics. Presence of few teeth in oral cavity helps in preserving alveolar ridge integrity, maintain the proprioception, and gives psychological benefit to the patient. This case report presents a case of a 54-years-old female with few remaining teeth, one remaining tooth in maxilla and two remaining teeth in mandible. Patient wants to get her masticatory function and aestethic back with maintain her remaining teeth. Cu-sil denture is one of the simplest removable partial denture that can replace missing teeth where only few teeth remain in oral cavity. A Cu-sil denture is a denture with holes, lined by a gasket, the holes that providing space for remaining natural teeth to emerge into the oral cavity through the denture.

Flabby ridge is an excessive movable fibrous tissue, usually affecting the maxillary and mandibular edentulous ridges. It is a typical finding frequently observed in the maxillary anterior region. It usually occurs when natural teeth oppose an edentulous ridge or in long-term denture wearers. The management of flabby ridges includes surgical intervention, implant-retained prostheses, and conventional dentures fabricated using the modified impression technique. This case report depicts a modified technique with the utilization of an aluminum mesh double tray and polyvinylsiloxane impression material for the management of a flabby ridge in the maxillary arch.

AbstractThe aim of this case report is to evaluate the efficacy of low-level laser therapy (LLLT) in the treatment of oral mucositis induced by methotrexate (MTX).A 52-year-old male patient, affected by rheumatoid arthritis and treated with corticosteroids and MTX, complained about severe oral pain and lesions for two months. He had been treated with topical and systemic corticosteroid therapy and chlorhexidine rinses with no significant improvement. He was not able to eat solid food or to wear his dental prosthesis. Examination of the oral cavity revealed retro-commissural and labial bilateral ulcerations and erythema and a 2-cm wide fibrous white lesion on the lower anterior vestibular ridge. Blood tests showed mild pancytopenia, and oral biopsies evidenced a “non-specific inflammatory condition” excluding any precancerous or paraneoplastic lesion or autoimmune diseases of the oral cavity. Previous medical records revealed that MTX had been discontinued many times due to mucositis since 2006, when the therapy had been started. This suggested a correlation between the onset of oral mucositis and MTX accumulation. LLLT was carried out for 4 consecutive days and three once-a-week follow-ups were performed.Lesions had completely healed during the second follow-up and the patient referred no pain at all from the 4th laser session on, therefore, the discontinuation of MTX had not been necessary.LLLT could represent an innovative technique to relieve pain related to MTX side effects thus avoiding dangerous discontinuation of therapy.
