This study examined the effects of orofacial myofunctional therapy (OMT) on speech intelligibility in adults with persistent articulation impairments. Six adults in the age range of 18-23 years were selected to receive orofacial myofunctional therapy for a period of six weeks. The results showed that five out of six clients made significant progress in oral postures and speech sound production across all three speech production tasks: single words, sentences, and spontaneous speech. Speech intelligibility increased significantly in all clients except the one diagnosed with developmental apraxia of speech. Orofacial myofunctional therapists, speech-language pathologists, and other professionals need to rule out underlying orofacial myofunctional variables when targeting speech sounds for intervention. Speech production tasks may be considered as important measures to understand the efficacy of OMT in clinical settings.

Purpose Children with Down syndrome (DS) demonstrate speech sound disorders, which impact speech intelligibility in academic and social settings across the life span. The purpose of this technical report is to determine the effects of electropalatographic (EPG) therapy on speech productions for children with DS receiving school-based speech-language therapy services. EPG is a form of visual feedback that displays timing and location of the tongue's contact with the palate during productions of individual phonemes. Method Authors completed a comprehensive literature search to locate articles that investigated the treatment effects of EPG for children with DS. Using the PICO (population, intervention, control, and outcomes) format, we sought in the research to answer “Do children 3–18 years of age with Down syndrome (P), who receive EPG therapy (I) compared to same-age peers with Down syndrome who do not receive EPG therapy (C) show improvement on speech sound production (O)?” Results Three studies met the inclusion criteria and were critically appraised. Two studies were single-subject designs, and one study was a randomized controlled study. Results revealed that EPG therapy improved speech sound production of target phonemes (/s/, /ʃ/, /k/, and /g/) with no transfer to speech intelligibility. Conclusions The results may further the understanding of EPG therapy as a treatment approach, particularly in older individuals with DS receiving school-based speech-language therapy services. Prior to adopting EPG as a treatment approach, clinicians should consider treatment intensity and students' cognitive abilities. Additional research is needed on the long-term effects and generalization of EPG therapy.

Abstract Nonspeech oral motor exercises (NSOME) are used often by speech-language pathologists to help children improve their speech sound productions. However, the phonology, articulation, and motor speech development and disorders literature does not support their use. This article presents five reasons (four theoretical, one empirical) why NSOME are not an appropriate therapeutic technique for treating children's speech sound production problems.

Purpose The purpose of this study was to determine the extent to which school-age children with speech sound disorder (SSD) exhibit concomitant reading difficulties and examine the extent to which phonological processing and speech production abilities are associated with increased likelihood of reading risks. Method Data were obtained from 120 kindergarten, first-grade, and second-grade children who were in receipt of school-based speech therapy services. Children were categorized as being “at risk” for reading difficulties if standardized scores on a word decoding measure were 1 SD or more from the mean. The selected predictors of reading risk included children's rapid automatized naming ability, phonological awareness (PA), and accuracy of speech sound production. Results Descriptive results indicated that just over 25% of children receiving school-based speech therapy for an SSD exhibited concomitant deficits in word decoding and that those exhibiting risk at the beginning of the school year were likely to continue to be at risk at the end of the school year. Results from a hierarchical logistic regression suggested that, after accounting for children's age, general language abilities, and socioeconomic status, both PA and speech sound production abilities were significantly associated with the likelihood of being classified as at risk. Conclusions School-age children with SSD are at increased risk for reading difficulties that are likely to persist throughout an academic year. The severity of phonological deficits, reflected by PA and speech output, may be important indicators of subsequent reading problems.

Objective: To collect information on the usage of nonspeech oral motor exercises (NSOMEs) by speech and language therapists (SLTs) for treating speech sound disorders (SSDs) in children in the Republic of Ireland. Method: SLTs who had worked with children with SSDs were invited to complete an online questionnaire adapted from a previous survey conducted in the US by Lof and Watson (2008). Main results:: 22/39 (56%) of the respondents reported using NSOMEs. Information from a colleague about the usefulness of NSOMEs, continuing education, and literature influenced the respondents the most to use NSOMEs. Most respondents used NSOMEs as a “warm up”, mainly with children with childhood apraxia of speech, dysarthria, and Down Syndrome. Conclusion: NSOMEs are used by over half of the respondents despite the lack of evidence that supports this treatment approach. Continuous effort to encourage the application of evidence-based practice in clinics is warranted.

Language delay (LD) and its relationship with later language impairment in preterm children is a topic of major concern. Previous studies comparing LD in preterm (PT) and full-term (FT) children were mainly carried out with samples of extremely preterm and very preterm children (sometimes with additional medical problems). Very few of them were longitudinal studies, which is essential to understand developmental relationships between LD and later language impairment. In this study, we compare the prevalence of LD in low-risk preterm children to that of FT children in a longitudinal design ranging from 10 to 60 months of age. We also analyze which variables are related to a higher risk of LD at 22, 30 and 60 months of age. Different language tests were administered to three groups of preterm children of different gestational ages and to one group of full-term children from the ages of 10 to 60 months. ANOVA comparisons between groups and logistic regression analyses to identify possible predictors of language delay at 22, 30 and 60 months of age were performed. The results found indicate that there were practically no differences between gestational age groups. Healthy PT children, therefore, do not have, in general terms, a higher risk of language delay than FT children. Previous language delay and cognitive delay are the strongest and longest-lasting predictors of later language impairment. Other factors, such as a scarce use of gestures at 10 months or male gender, affect early LD at 22 months of age, although their effect disappears as children grow older. Low maternal education appears to have a late effect. Gestational age does not have any significant effect on the appearance of LD.

Knowledge of vocal tract area functions is important for the understanding of phenomena occurring during speech production. We present here a new measurement method based on the external excitation of the vocal tract with a known pseudo-random sequence, where the area function is obtained by a linear prediction analysis applied to the cross-correlation between the sequence and the signal measured at the lips. The advantages of this method over methods based on sweep-tones or white noise excitation are (1) a much shorter measurement time (about 100 ms) and (2) the possibility of speech sound production during the measurement. This method has been checked against classical methods through systematic comparisons on a small corpus of vowels. Moreover, it has been verified that simultaneous speech sound production does not perturb significantly the measurements. This method should thus be a very helpful tool for the investigation of the acoustic properties of the vocal tract in various cases for vowels.
