The Edwards SAPIEN transcatheter heart valve (Figure 1) is designed for heart valve replacement in patients with severe aortic stenosis without open-heart surgery. Physiological FEA analyses have been performed to provide an assessment of the fracture and fatigue resistance of the device during deployment and operation. The present study validates FEA frame models by comparing the crimping behavior of the FEA models with the results of crimping experiments.

Background: Open heart surgery is usually performed by connecting the heart to a cardiopulmonary bypass (CPB) machine. The use of the CPB machine may decrease the hemoglobin level and a very low hemoglobin levelcould cause seriouscomplications.This study aimed to explore the decrease in hemoglobin level after open heart surgery. Methods: A cross-sectional retrospective descriptive study was conducted on medical records of patients underwent coronary artery bypass graft (CABG) surgery and heart valve surgery in a tertiary hospital in Indonesia in 2018. The total sampling method was deployed to all medical records of patients underwent CABG surgery (n=25) and patients underwent heart valve surgery (n=3). Results: The decrease in hemoglobin level among young-adult female patients after heart valve surgery was 6.8 g/dl. The average decrease in mid-adult male and female patients after CABG surgery were 6 g/dl and 5.8 g/dl, respectively, and, after the heart valve surgery, the levels were 8.5 g/dl and 5.4 g/dl, respectively. The average decrease in Hb level among late-adult male and female patients after CABG surgery was 6.1 g/dl and 5.4 g/dl, respectively. Conclusion: Hemoglobin level decreases after an open heart surgery. Therefore, observation on the hemoglobin level to prevent complications and to facilitate early treatment is necessary.

Although percutaneous heart valve replacement is getting known amongst cardiovascular surgical procedures, as it was recently introduced, and reports of early clinical experience been published, this technique is limited to the replacement of pulmonary and aortic valves in old patients who cannot undergo open heart surgery. One of the reasons is the uphill challenges in the generation of an ideal design that would address anchorage and leakage issues. Stent anchorage and paravalvular leaks prove to the greatest challenge posed to biomedical design engineers. This paper describes a novel Nitinol based mitral valve stent that addresses migration and paravalvular leaks associated with the bioprosthetic mitral valve. The geometry-based model presented here specifically addresses issues of valve migration and paravalvular leaks This is of great interest to designers of new prosthetic heart valve models, as well as to surgeons involved in valve sparing surgery. Simulation results show that the studied stent design seemed to be good by virtue of its acceptable maximum crimping strain.

Different crystalloid cardiopulmonary bypass pump-priming fluids provide very different substrate loads to patients undergoing open-heart surgery. As a result they may modulate the endocrine milieu, and thus might be expected to alter postoperative nitrogen balance. To test this hypothesis, 24 adult patients undergoing open-heart valve surgery were randomized into four matched groups each to receive a different prime, namely: Hartmann's solution, Plasmalyte 148R, Solution 11 R and a prime consisting of equal volumes of Hartmann's solution and 5% dextrose. Accurate nitrogen balance studies were obtained each 24 hours from one day preoperatively to seven days postoperatively. The results obtained demonstrated that postoperative nitrogen balance was unaffected by the nature of the crystalloid pump prime. Nitrogen balance in the patients studied was better than that reported from other centres in similar patients.

SummaryA prospective study in 13 patients undergoing open-heart surgery with extracorporeal circulation revealed a marked decrease of the mean one-stage prothrombin time activity from 88% to 54% (p <0.005) but lesser decreases of factors I, II, V, VII and X. This apparent discrepancy was due to the appearance of an inhibitor of the extrinsic coagulation system, termed PEC (Protein after Extracorporeal Circulation). The mean plasma PEC level rose from 0.05 U/ml pre-surgery to 0.65 U/ml post-surgery (p <0.0005), and was accompanied by the appearance of additional proteins as evidenced by disc polyacrylamide gel electrophoresis of plasma fractions (p <0.0005). The observed increases of PEC, appearance of abnormal protein bands and concomitant increases of LDH and SGOT suggest that the release of an inhibitor of the coagulation system (similar or identical to PIVKA) may be due to hypoxic liver damage during extracorporeal circulation.
